How You Get the Form
You don’t download the CFWB-049. Before you can finish the form, you need a provider lined up. That can be a licensed day care center, a licensed or registered family or group family day care home, or a legally exempt provider such as a relative, neighbor, or nanny. Licensed providers must be approved by ACS to accept vouchers; legally exempt providers must be enrolled and monitored through the Women’s Housing and Economic Development Corporation (WHEDco), which New York State contracts for that purpose.3Administration for Children’s Services. Filling Out the Parent and Provider Sections
Complete your section first. It covers your household information and identifies the child who will receive care. Your provider then fills in their portion, which confirms their licensing or enrollment status and the details of the care arrangement. Submitting the Form Through the Portal
The signed CFWB-049 goes to ACS through the Child Care Voucher Submission Portal. The portal accepts voucher uploads only, not other forms or general inquiries. You’ll be asked for your name, your child’s name, whether the voucher is new or a transfer, your case number, a phone number, and an email address, and then you’ll upload the signed form.4NYC Administration for Children’s Services. A few things worth watching:
- If you have more than one child receiving a voucher, submit a separate portal entry for each child.
- Don’t resubmit. Duplicate submissions can delay enrollment. If you’re not sure whether an upload went through, wait rather than sending it again.
- The portal will also accept companion enrollment forms if your situation calls for them, including the CFWB-048, CFWB-050, and OCFS-LDSS-4699.
The portal page does not list a mailing address or fax number as alternatives. If you need to submit a physical copy because you don’t have internet access, contact ACS directly or visit a local ACS office for guidance.

What Happens After You Submit
ACS reviews your voucher and supporting documents, then mails a child enrollment notice to both you and your provider once enrollment is finalized. Expect up to six weeks, depending on the type of provider and whether they’re already in the ACS system.4NYC Administration for Children’s Services. A brand-new provider still finishing their own enrollment with ACS adds time.
The date your child can officially be enrolled with the provider depends on three things: the issue date of the voucher, when the child actually started attending care, and whether any additional approvals are still pending.4NYC Administration for Children’s Services. Switching Providers or Reporting Changes
You can change providers at any time during your eligibility period by requesting a transfer voucher from ACS.1Administration for Children’s Services. Support for Families With Vouchers A new CFWB-049 gets completed with the new provider and submitted through the portal the same way as the first one, with “transfer” indicated when the portal asks.
Report other changes to ACS promptly, including schedule adjustments and income changes. Failing to disclose a change that affects your eligibility, such as receiving additional income, can be treated as deliberate concealment under Social Services Law and may trigger repayment obligations or criminal penalties.5New York State Senate.
