The Alabama DL-1-93 form is the school enrollment and exclusion document that every driver’s license or learner’s permit applicant under 19 must present to the Alabama Law Enforcement Agency (ALEA). You pick up a blank copy at your school or an ALEA driver license office, fill out your portion, have your school’s attendance officer certify the rest, and bring the original to your license appointment. The form exists because Alabama law ties driving privileges for anyone under 19 directly to school enrollment, satisfactory attendance, and good standing.
Who Has to Use the DL-1-93
If you are under 19 and applying for, renewing, or reinstating a learner’s permit or driver’s license in Alabama, you must present one of three things: a high school diploma, a GED certificate, or a completed DL-1-93.1Alabama Law Enforcement Agency. You don’t have to be sitting in a traditional classroom to qualify. Alabama Code Section 16-28-40 recognizes several other categories of young people who are eligible even without active enrollment:2Alabama Legislature. Alabama Code 16-28-40 – License Applicant Under 19 to Provide Documentation of School Enrollment, Etc. students making satisfactory progress in a GED or nontraditional high school diploma option (HSDO) program; participants in a job-training program approved by the State Superintendent of Education; minors who are gainfully and substantially employed; minor parents with care and custody of a child or expecting one; applicants whose parents depend on them as the sole source of transportation, as certified by a physician; and students kept from school by circumstances beyond their control, such as a physical or mental inability to attend or living more than two miles from school without public transportation. Suspension, expulsion, and imprisonment do not count as circumstances beyond the applicant’s control.1Alabama Law Enforcement Agency. Alabama DL-1-93 School Enrollment Form
Homeschool students use a different document. ALEA provides a separate Home School Student Affidavit instead of the DL-1-93, available on the same driver license forms page.3Alabama Law Enforcement Agency. Home School Affidavit
Where to Get the Form
Blank DL-1-93 forms are available at your school’s front office, at any ALEA driver license office, and as a printable PDF on the ALEA Driver License Forms page.4Alabama Law Enforcement Agency. Driver License Forms The official title on the document is “Enrollment / Exclusion Form,” and it cites Sections 16-28-40 through 16-28-45 of the Alabama Code.
How to Fill Out Each Part
The form has three parts. You complete Part I, the school completes Part II, and Part III applies only if you qualify under one of the statutory exclusion categories.
Part I: Your Information
Enter your full legal name, date of birth, Social Security number, sex, driver’s license number if you already have one, and home address.1Alabama Law Enforcement Agency. Alabama DL-1-93 School Enrollment Form Every entry needs to match your birth certificate and Social Security card exactly. A mismatch between the name on the form and the name on your other documents is one of the fastest ways to get turned away at the license counter.
Part II: School Certification
Part II has to be completed and signed by your school’s attendance officer or chief attendance administrator. The statute names those officials specifically, not principals or teachers.2Alabama Legislature. Alabama Code 16-28-40 – License Applicant Under 19 to Provide Documentation of School Enrollment, Etc. The front office or counselor will know who handles the form at your school. The official checks a box indicating whether you are currently enrolled, have withdrawn due to circumstances beyond your control, or have obtained a GED or HSDO diploma, and confirms that you have not accumulated disciplinary points that would delay your eligibility. They sign and include their title, the school’s name, address, and contact information. GED, HSDO, and virtual program students must also list the program name and date of enrollment or completion.1Alabama Law Enforcement Agency. Alabama DL-1-93 School Enrollment Form
Part III: Exclusion Categories
You only use Part III if you are not enrolled in any school or program but still qualify under one of the statutory exceptions described earlier. The appropriate authority checks the applicable box, whether that’s parent with custody, sole transportation provider, or gainfully employed, and signs. If the exclusion rests on a physician’s certification or a circumstance-beyond-control claim, the supporting documentation goes with the form.
Attendance and Discipline Can Block Certification
Your school cannot certify your enrollment if you have accumulated more than 10 consecutive or 15 total unexcused absences in a single semester. Under the statute, that level of absence counts as a withdrawal, and the attendance officer must report it to ALEA.2Alabama Legislature. Alabama Code 16-28-40 – License Applicant Under 19 to Provide Documentation of School Enrollment, Etc. If you are close to those thresholds, resolve the absences with your school before asking for the form.
Discipline matters too. Applicants must not have accumulated disciplinary points that would extend the age at which they become eligible, and students currently suspended or expelled cannot have the form certified during that period.
What Else to Bring with the Form
The DL-1-93 is one piece of a larger packet. For a learner’s permit at 15, bring a certified U.S. birth certificate issued by the Bureau of Vital Statistics (hospital copies are not accepted), your original Social Security card, and two proofs of your home address. For a driver’s license at 16 or older, bring two forms of identification with at least one photo ID from ALEA’s primary list, your Social Security card, and two proofs of residence.5Alabama Law Enforcement Agency. Document Requirements and Fees
Acceptable proofs of residence include a voter registration card, a utility bill less than 90 days old, a vehicle registration, a current lease, or last year’s tax return showing your address. If a utility bill is in a parent’s name, bring your birth certificate to connect you to that address.5Alabama Law Enforcement Agency. Document Requirements and Fees Photocopies are not accepted for any of these documents.6Alabama Law Enforcement Agency. STAR ID Document List Expect a $5 testing fee and $36.25 for the license, and note that the office does not accept checks.
Submitting the Form
Bring the original, completed DL-1-93 to the ALEA driver license examining office. The examiner needs the physical document because the school official’s original signature has to be verified; photocopies and faxed versions are rejected. Once your paperwork clears, you move on to vision screening and the written knowledge test for a learner’s permit, or the road test if you are applying for a full license.
Have the school complete the form as close to your appointment as you reasonably can. The information it certifies is a snapshot of your enrollment and attendance on the day of signing, and an older form invites questions about whether anything has changed since.
If You Stop Attending After You Get Your License
Once a student 16 or older crosses the 10-consecutive or 15-total unexcused absence threshold, the attendance officer notifies ALEA. Within five days of receiving that notice, ALEA mails the student a letter stating that the license or permit will be suspended on the 30th day after the notice was mailed.2Alabama Legislature. Alabama Code 16-28-40 – License Applicant Under 19 to Provide Documentation of School Enrollment, Etc. That 30-day window is your opportunity to re-enroll, resolve the absences, or document an applicable exclusion and send proof to ALEA before the suspension takes effect.
If the suspension does take effect, getting the license back requires the same documentation as a new application: a fresh DL-1-93 showing current enrollment or an applicable exclusion, presented at the license office with your other identification.1Alabama Law Enforcement Agency.
