Alabama Vital Records Phone Number and Business Hours

The main Alabama vital records phone number is (334) 206-5418, which reaches the Alabama Center for Health Statistics in Montgomery. The line carries recorded information at all hours and connects to a live representative during business hours. The state office does not take credit or debit cards directly, so if you want to pay by card over the phone, call VitalChek instead at 1-888-279-9888.1Alabama Department of Public Health. Which Number to Call

The two lines handle different things, and calling the wrong one wastes time.

  • (334) 206-5418 is the Center for Health Statistics customer service line. Use it for questions about record availability, who is eligible to order, the status of a pending request, or anything about the process itself.2Centers for Disease Control and Prevention. Where to Write for Vital Records – Alabama
  • 1-888-279-9888 is VitalChek, the state’s authorized vendor for card payments. The toll-free line is open around the clock and handles both the phone order and the payment. VitalChek adds a service fee on top of the state’s certificate fee.1Alabama Department of Public Health. Birth Certificates
  • (334) 206-2637 is the Birth Amendment Clerk, a separate line for correcting or changing a birth or death certificate rather than ordering a copy.3Alabama Department of Public Health. Application to Change an Alabama Birth or Death Certificate

Have This Ready Before You Dial

Representatives can’t pull a record without the basics. Missing any of these usually means you’ll be asked to call back.

  • Full legal name of the person on the record
  • Date of the event (birth, death, marriage, or divorce)
  • County where the event took place
  • Parents’ names for a birth certificate, including the mother’s maiden name and the father’s full name

For a death certificate, the decedent’s Social Security number and approximate age at death help if the name is a common one. For marriage or divorce records, have the full names of both spouses.

If you plan to order through VitalChek by phone, you’ll also need your card and a valid photo ID handy, and you’ll be asked for your relationship to the person on the record.

Who Can Order

Alabama restricts access to recent vital records. Birth certificates less than 125 years old and death certificates less than 25 years old can only be issued to someone who proves they are legally entitled to the copy.5Alabama Administrative Code. Alabama Administrative Code 420-7-1-.22 – Who May Obtain Certified Copies of Vital Records Eligible requesters include:

  • The person named on the record (at least 14 years old, or an emancipated minor, for a birth certificate)
  • A parent listed on the record
  • An adult sibling
  • An adult child of the person named
  • A spouse of the person named

Acceptable ID is decided by the State Registrar. Primary options include an Alabama driver’s license, an out-of-state driver’s license, or a state-issued non-driver ID, and the document must be current or expired no more than 60 days.6Alabama Department of Public Health. What It Costs

The search fee is $15.00 for a birth, death, or marriage certificate. That fee covers one certified copy, or a “Certificate of Failure to Find” if no matching record turns up. Each additional copy of the same record ordered at the same time is $6.00.7Alabama Department of Public Health. Death Certificates8Alabama Department of Public Health. Marriage Certificates

Ordering through VitalChek adds service and processing fees on top of the state fee, with the exact surcharge depending on record type and shipping speed. Paying by check or money order through the mail avoids those extra charges.1Alabama Department of Public Health. Birth Certificates

Amendments run on a separate fee schedule. Correcting a birth or death certificate costs $20.00, which includes one certified copy of the corrected record. Extra copies are $6.00 each, and expedited service is an optional $15.00. All amendment fees are non-refundable.3Alabama Department of Public Health.
