Alameda County Death Certificate: Order Online, by Mail, or In Person

Authorized Copy vs. Informational Copy

California issues two versions of a death certificate, and the wrong one will be rejected by most institutions.

An Authorized Certified Copy is the version that carries full legal weight for closing accounts, collecting insurance, and transferring property. Only certain people qualify to request it: the deceased person’s spouse or registered domestic partner, parent, child, grandchild, grandparent, or sibling; an attorney handling the estate or someone appointed by a court; law enforcement or government officials acting in an official capacity; and funeral home employees ordering on behalf of an eligible family member.2California Legislative Information. An Informational Certified Copy is available to anyone and contains the same factual details, but it is stamped “INFORMATIONAL, NOT A VALID DOCUMENT TO ESTABLISH IDENTITY.”3California Legislative Information. What You Need Before You Apply

  • Full legal name (last, first, middle) as it appears on official records
  • Date of death (month, day, year)
  • City where the death occurred, which may not be the same as where the person lived
  • Number of copies you need

You will also provide your own contact information and, for an authorized copy, indicate your qualifying relationship. The Social Security number and mother’s maiden name are not required on the form, but having them ready can help staff locate the correct record when the name is common.

Payment options at the counter include cash, debit or ATM cards, personal or bank checks payable to “Alameda County Clerk Recorder,” and money orders. Ordering by Mail

Mail orders for an Authorized Certified Copy require an extra step. Ordering Online

Alameda County offers electronic ordering through its Clerk-Recorder website. Online orders carry the same $28 per copy fee plus a $2 convenience fee per order, and only Visa and Mastercard are accepted.1Alameda County Clerk-Recorder. Online processing runs longer than a walk-in visit, so if you are working against a deadline, go in person.

Ordering From the State Instead

The California Department of Public Health keeps its own permanent records of every death in the state going back to July 1905. You can request copies directly from the state at $26 per copy by mailing a completed VS 112 application and a check or money order payable to “CDPH-VR” to CDPH Vital Records in Sacramento.8California Department of Public Health. The authorized-copy rules are the same, because they come from the same statute. State-level processing typically takes longer than ordering from the county, so the CDPH route makes the most sense when the county office is inconvenient or the death occurred in a different California county.

How Many Copies to Order

This is where people consistently underestimate. Every bank, insurance company, brokerage, and government agency that needs proof of death will typically ask for its own certified original. Some return the copy after review; many do not. Common recipients include:

  • Life insurance companies: one per policy
  • Banks and credit unions: one per institution to close or transfer accounts
  • Brokerage and retirement accounts: one per financial firm
  • Social Security Administration: one to apply for survivor benefits
  • Mortgage lenders: one to address the outstanding loan
  • Real property transfers: one per property for the county recorder where the property sits
  • Vehicle title transfers: one per vehicle through the DMV
  • Probate court: one or more for estate filings

Ordering ten to fifteen copies upfront at $28 each sounds steep, but reordering later costs the same per copy plus additional processing time. Better to have a few extras in a folder than to stall an insurance payout because you ran out.

Fixing Errors on a Death Certificate

Misspellings, wrong dates, and missing information can block estate proceedings and insurance claims. California handles corrections through the Affidavit to Amend a Record, known as Form VS 24. The affidavit must be signed by two people and notarized. All amendment requests go through the California Department of Public Health by mail, not through the county. If the error goes beyond a simple typo, such as a dispute over the cause of death or a fundamental identity mistake, CDPH may require a court order rather than an affidavit. Once the amendment is processed, you can order updated certified copies reflecting the corrected information.
