Ambetter Georgia Claim Dispute Form: Levels, Fields, and Deadlines

To submit the Ambetter Georgia claim dispute form, a provider fills in the identification fields exactly as they appear in Ambetter’s records, checks the box that matches the reason for the dispute, attaches the supporting documents required for either Level I or Level II, and mails or faxes the package to Ambetter from Peach State Health Plan within 180 days of the original Explanation of Payment (90 days for non-participating providers).1Ambetter from Peach State Health Plan. Who the Form Is For

This is a provider document. Billing offices use it when Ambetter’s payment or denial on a specific claim looks wrong, whether the issue is a missing authorization, the wrong payment amount, or an untimely-filing denial applied in error.2Ambetter from Peach State Health Plan. Decide Level I or Level II Before You Start

Ambetter sorts provider challenges into two tiers, and the tier dictates what you attach and which attention line you use on the envelope.

Level I: Request for Reconsideration

Level I is the first step, used when Ambetter made an error during initial adjudication: applying the wrong fee schedule, denying a service that had proper authorization, or rejecting a claim as untimely when you can prove it was filed on time.1Ambetter from Peach State Health Plan. Provider Request for Reconsideration and Claim Dispute Form Attach medical records if the denial involved a code audit, code edit, or authorization issue. Do not attach the original claim form.

Level II: Claim Dispute

Level II is for situations where Ambetter’s answer to your Level I was unsatisfactory. You cannot skip Level I. For Level II, attach a copy of the Explanation of Payment with the disputed claim numbers clearly circled, plus Ambetter’s written response to your original reconsideration request.1Ambetter from Peach State Health Plan. Fields You Have to Complete

Every field in the identification section is mandatory. Miss one and the claims team cannot pull the right file, so the submission stalls. Have the following ready before you open the PDF:2Ambetter from Peach State Health Plan. Georgia Provider Claim Dispute Form

  • Provider name, matching Ambetter’s records for the rendering or billing provider.
  • Provider Tax ID number. The form asks for the federal TIN, not the NPI.
  • Control or claim number from the Explanation of Payment.
  • Date or dates of service, matching the original claim exactly.
  • Member name as it appears on the Ambetter card.
  • Member RID number from the card.
  • Requestor name and phone number. This can be a billing representative, not necessarily the treating provider.
  • Date of request, meaning the date you submit.

Checking the Reason for the Dispute

The form offers checkbox categories for the most common reasons. Pick the one that fits:2Ambetter from Peach State Health Plan. Georgia Provider Claim Dispute Form

  • Denied for no authorization, but authorization was obtained. Write the authorization number in the space provided.
  • Denied for no authorization, but none was required, meaning the service falls outside Ambetter’s prior-authorization list.
  • Denied for untimely filing in error. Attach proof of timely filing, such as a clearinghouse receipt or fax confirmation showing the original submission date.
  • Paid to the wrong provider or entity.
  • Paid for the incorrect amount, where the reimbursement does not match the contracted rate or expected allowed amount.
  • Other, a write-in option for things like duplicate-claim denials or coordination-of-benefits errors.

If you select Other, be specific. A line like “claim was processed incorrectly” forces the reviewer to come back for clarification, adding weeks. State what the payment or denial should have been and why, and point to the contract rate, benefit language, or coding logic that supports your position.

Where to Send It

Mail the completed form with attachments to the address printed on the form. The P.O. Box is the same for both tiers, but the attention line sends it to a different review team:1Ambetter from Peach State Health Plan. Provider Request for Reconsideration and Claim Dispute Form

  • Level I: Ambetter from Peach State Health Plan, Attn: Level I – Request for Reconsideration, PO Box 5010, Farmington, MO 63640-5010.
  • Level II: Ambetter from Peach State Health Plan, Attn: Level II – Claim Dispute, PO Box 5010, Farmington, MO 63640-5010.

Get the attention line wrong and the packet lands in the wrong queue. You can also fax to (833) 957-0438, which gives you a transmission confirmation you can use later as proof of delivery if a deadline question comes up.1Ambetter from Peach State Health Plan. One formatting trap catches offices off guard. Ambetter automatically rejects photocopied, black-and-white, or handwritten claim forms submitted alongside a dispute, whether the submission is a first-time claim, corrected claim, or dispute filing.1Ambetter from Peach State Health Plan. Provider Request for Reconsideration and Claim Dispute Form If the original claim needs correcting, send the corrected claim separately through the process in Ambetter’s Provider Manual. Do not bundle it with the dispute form.

Filing Deadlines

Participating providers have 180 days from the date on the original Explanation of Payment or denial notice to file. Non-participating providers have 90 days from the same date.1Ambetter from Peach State Health Plan. Provider Request for Reconsideration and Claim Dispute Form The deadlines are firm. A dispute received after the window closes will not be reviewed, regardless of the merits. If you are close to the cutoff and still gathering documentation, submit the form with what you have and note that additional records will follow.

If the dispute itself is about a late payment, Georgia law sets 15 working days for electronic claims and 30 calendar days for paper claims, with 12 percent annual interest on amounts the insurer pays late. Cite O.C.G.A. § 33-24-59.5 in the explanation section when that is the issue.3Justia. Georgia Code 33-24-59.5 – Definitions; Timely Payment of Claims

Mistakes That Slow Things Down

Incomplete documentation is the leading reason disputes stall. A Level II dispute without the Level I response letter attached gets kicked back before anyone reads it. Circling the wrong claim number on the Explanation of Payment, or failing to circle any, makes the reviewer guess which claim is in play.

Coding disputes need specifics. If the denial stems from a diagnosis-to-procedure code mismatch, state the correct pairing and explain why it is clinically appropriate. A generic “coding error” note does not give the reviewer enough to overturn the decision. For authorization disputes, the most useful single attachment is a screenshot or printout of the authorization number and approval date from the payer’s portal.

Watch the calendar. Filing in the last week of the 180-day or 90-day window leaves no room to resubmit if the form comes back for a formatting issue or missing field. Filing early, even with partial documentation, preserves the right to review.

If You Are the Member, Not the Provider

Members do not use this form. Ambetter uses a separate Member Grievance or Medical Necessity Appeal Form for coverage denials, benefit disputes, and complaints about care, and you have 180 calendar days from the denial letter to file an appeal.4Ambetter from Peach State Health Plan.
