The BCBSNC member claim form is what you file with Blue Cross and Blue Shield of North Carolina when you paid a provider directly and need to be reimbursed for covered care. You’ll use it most often after seeing an out-of-network provider who didn’t bill Blue Cross NC for you. The form goes in through the Blue Connect portal, by mail, or by fax, and you have 18 months from the date of service to send it.1Blue Cross Blue Shield of North Carolina. When You Actually Need to File
In-network providers submit claims to Blue Cross NC on your behalf, so there’s nothing for you to do. The member claim form exists for the situations where that doesn’t happen: an out-of-network provider with no participation agreement, an emergency visit at a non-participating facility, a specialist outside your network, or treatment while traveling inside the United States.2Blue Cross Blue Shield of North Carolina. Member Forms
Don’t file this form if your provider is in-network or is already filing for the same services. A duplicate just slows the real claim down.1Blue Cross Blue Shield of North Carolina. What to Gather Before You Start
The single most common reason a claim gets returned is a missing document. Pull everything together before you touch the form:
- Your Subscriber ID, including the letter prefix on the front of your member card and the two-digit suffix that follows the member’s name.3Blue Cross NC. Your Member ID Card
- An itemized bill from the provider. This is not a credit card slip or a payment receipt. It has to show the provider’s name and address, their tax ID number, the date of each service, a description of each service, the CPT procedure codes and ICD-10 diagnosis codes, and the individual charge for each service.1Blue Cross Blue Shield of North Carolina. Member Claim Form Requirements
- Proof of payment, if you already paid the provider.
- The Explanation of Benefits from any other health plan or Medicare that is primary. Blue Cross NC will not process your claim without it.1Blue Cross Blue Shield of North Carolina. Member Claim Form Requirements
Call the provider’s billing office and ask specifically for an itemized bill with procedure and diagnosis codes. Many offices default to a plain receipt, and a plain receipt alone will come back to you.
Filling Out the Form
Download the form from the Blue Cross NC website or Blue Connect portal.2Blue Cross Blue Shield of North Carolina. Member Forms If you’re filling it in by hand, use blue or black ink. Use a separate form for each family member and a separate form for each provider. Two family members who saw the same doctor on the same day means two forms.1Blue Cross Blue Shield of North Carolina. Patient Information
Enter the Subscriber Number with the letter prefix from your card. Add the patient’s full name, date of birth, sex, and relationship to the subscriber (self, spouse, child, or other), plus the two-digit suffix after the member’s name.
Mailing Information
Enter the address where you want correspondence and any reimbursement check sent. Check the ZIP code carefully. A wrong ZIP means the check lands somewhere else.
Other Insurance
Say whether the patient has any other health coverage. If yes, list the other insurer’s name, policy number, policyholder’s name, and policyholder’s employer. Note whether the patient is enrolled in Medicare Part A, B, or C, and give the Medicare claim number if there is one. Medicare Supplement members don’t need to complete a separate Coordination of Benefits questionnaire, because Blue Cross NC already treats Medicare as primary for them.4Blue Cross NC. The 18-Month Deadline
You have 18 months from the date you received the service to file. Anything later is denied.1Blue Cross Blue Shield of North Carolina. Member Claim Form Requirements When another insurer is primary and you had to wait for their EOB, count from the date on that EOB rather than the date of service. The window sounds generous, but chasing itemized bills and code lists from a billing office eats weeks. File as soon as the paperwork is in hand.
Care Received Outside the United States
Care received outside the US, Puerto Rico, and the US Virgin Islands does not use this form. It goes through the Blue Cross Blue Shield Global Core program, which has its own International Claim Form available on the BCBS Global Core website.6Blue Cross Blue Shield of Illinois. How Do I Submit a Claim for Services Received Internationally This only applies if your Blue Cross NC plan includes a travel benefit.
The original itemized bill from each provider has to be attached, on letterhead, showing the provider’s name and address, the patient’s full name, the date of each service, a description of each service, and the charge in local currency. Collect every invoice and medical record before you leave the country. Getting them later from abroad is hard and sometimes impossible. You can request payment in US dollars or another currency, but if the currency you pick isn’t available you’ll be paid in dollars.7Blue Option SC. International Claim Form
What Happens After You File
Watch for an Explanation of Benefits in the mail or in your Blue Connect account. The EOB shows what was covered, what was applied to your deductible, and how much you’ll be reimbursed.
North Carolina law backs up the timeline. Under N.C. Gen. Stat. ยง 58-3-225, Blue Cross NC has 30 calendar days from receipt to pay the claim, deny it, or tell you something is missing. If part of the claim is undisputed and part is contested, the insurer must pay the undisputed part within that same 30 days and send notice on the rest. When Blue Cross NC asks you for more information, the clock resets, and the insurer gets another 30 days once you reply. Late payments accrue interest at 18 percent annually from the day after payment was due.8North Carolina General Assembly. Why Claims Get Returned or Denied
Most member-filed problems are preventable. The form itself warns that claims come back if provider receipts aren’t attached.1Blue Cross Blue Shield of North Carolina. Member Claim Form Requirements Beyond that:
- Missing procedure or diagnosis codes. A dollar amount and a date aren’t enough. Blue Cross NC needs CPT and diagnosis codes to decide whether the service is covered.
- No coordination of benefits documentation when another insurer or Medicare is primary.
- Services that aren’t covered or that required prior authorization you didn’t get.9Blue Cross Blue Shield of North Carolina. Understanding the Appeals Process
- Filing after the 18-month window.
- Duplicate submissions when the in-network provider already billed.
Small errors like a misspelled name, a wrong date, or a mistyped ID number often don’t require a formal appeal. The provider’s office can usually fix and resubmit.9Blue Cross Blue Shield of North Carolina. Understanding the Appeals Process
If Your Claim Is Denied
You have 180 days from the date on the denial letter to file a formal appeal.10Blue Cross NC. The Appeals Process An appeal is the right step when a service was denied as not medically necessary, called experimental or cosmetic, denied for a missing referral or prior authorization, or denied because the provider was out-of-network.9Blue Cross Blue Shield of North Carolina. Understanding the Appeals Process
Pull together medical records, referrals, prescriptions, and any provider correspondence that supports why the service was necessary. You can submit a letter or use the Member Appeal Form from the Blue Cross NC website. Appealing on someone else’s behalf requires an authorization form.9Blue Cross Blue Shield of North Carolina. Keep notes on every call you make to customer service: the representative’s name, the date, what they said, and a reference number. That paper trail matters if the appeal moves beyond internal review.
