BCBSTX Medical Claim Form: Filing, Deadlines, and Appeals

The BCBSTX medical claim form is a one-page document you fill out and mail (or upload) when your provider does not bill Blue Cross and Blue Shield of Texas directly, so the insurer can either reimburse you or apply your cash payment toward in-network cost sharing. Download the fillable PDF from the BCBSTX website, attach an itemized bill from your provider, and send it to P.O. Box 660044, Dallas, TX 75266-0044.

When You Need to File It Yourself

At the top of the form are two checkboxes, and you have to pick one before filling in anything else. “To Pay Insured/Subscriber” is the traditional reimbursement route: you already paid the provider out of pocket, usually because they were out of network or didn’t bill your insurance, and you want BCBSTX to pay back whatever your plan covers. “To get in-network credit for your cash payment to a provider” is the newer option created after Texas House Bill 2002, which lets PPO members apply cash payments to out-of-network providers toward their out-of-pocket maximum.1Blue Cross and Blue Shield of Texas. Do not file this form if your provider is already billing BCBSTX. The form says so explicitly: a duplicate submission from the member creates confusion and delays both claims.2Blue Cross and Blue Shield of Texas. What to Gather Before You Start

The form itself is short. The itemized bill you attach is where most problems happen, so get both pieces right the first time.

From Your Member ID Card

Copy your Group Number and Identification Number exactly as they appear on your BCBSTX card. The form warns that these must match the card precisely; a single transposed digit can cause the claim to be rejected at eligibility verification.3Blue Cross and Blue Shield of Texas. From Your Provider

Ask the office or facility for an itemized bill on their official letterhead. A receipt or credit card statement will not work. The itemized bill needs to show all of the following:3Blue Cross and Blue Shield of Texas. BCBSTX Medical Claim Form

  • Provider name and address, printed on the letterhead.
  • Patient name (which may differ from the subscriber).
  • Date of each service, listed individually.
  • Description of each service or supply.
  • Charge for each service, broken out line by line.
  • Diagnosis code identifying the condition treated.

If anything on that list is missing, call the office and ask for a corrected bill before you submit. Sending an incomplete bill is the fastest way to get the whole claim kicked back.

Filling Out the Form

The form fits on one page and moves from your personal details through the clinical encounter to your signature.

Subscriber and Patient Information

Enter the subscriber’s full name, mailing address, and employment status (employed, not employed, or retired, with a retirement date if applicable). Then fill in the patient’s full name, sex, date of birth, and relationship to the subscriber: self, spouse, child, or other. When the patient is a dependent, both names need to appear in their respective fields.3Blue Cross and Blue Shield of Texas. BCBSTX Medical Claim Form

Treatment Type and Date

Check exactly one box: injury, illness, pregnancy, or preventive care. Write the matching date next to it: the date of the accident for an injury, the date symptoms first appeared for an illness, the date of conception for pregnancy, or the date the service was received for preventive care.3Blue Cross and Blue Shield of Texas. BCBSTX Medical Claim Form You can attach multiple itemized bills to one form only if they all fall under the same treatment type. A routine physical and a specialist visit for a shoulder injury are two separate forms.

Diagnosis or Description

Write a diagnosis or a brief description of your symptoms. Plain English works here; formal coding is not required on this line. For preventive care, state the type: routine physical, hearing exam, vision exam, immunization, and so on.3Blue Cross and Blue Shield of Texas. BCBSTX Medical Claim Form The detailed codes go on the provider’s bill.

Work-Related and Motor Vehicle Questions

The form asks whether the illness or injury was work-related and, if so, the employer’s name and address. It also asks whether a motor vehicle was involved in an injury. Answer honestly. Workers’ compensation claims follow a different process, and BCBSTX uses the motor vehicle question to figure out whether another insurer should pay first.

Other Coverage and Medicare

If the patient has any other health insurance besides BCBSTX (not counting Medicaid, Medicare, or CHAMPUS), list the other carrier’s name, address, policy number, and effective date. A separate Medicare section asks whether the patient has Part A, Part B, or qualifies through disability, along with the Medicare ID. These coordination-of-benefits fields determine which plan pays first.3Blue Cross and Blue Shield of Texas. BCBSTX Medical Claim Form

Total, Signature, and Phone

Enter the total dollar amount for everything on the attached bills. Sign, date, and add a daytime phone number. Your signature certifies the information is correct and authorizes BCBSTX to request medical information from your providers.3Blue Cross and Blue Shield of Texas. If you’re on a non-HealthSelect BCBSTX plan, check your plan materials or call member services to confirm online submission is available for your policy.

Filing Deadline

The BCBSTX provider manual requires claims to be submitted within 365 days of the date of service, after which they are denied as untimely.5Blue Cross and Blue Shield of Texas. TX PPO Provider Manual Filing Claims – General Information That deadline is written for providers; your specific plan may set a different window for member-submitted claims, so check your benefits booklet or summary plan description. Either way, file as soon as you have the itemized bill in hand. The longer you wait, the harder it is to go back to the provider for a corrected version if something is missing.

What Happens After You File

Texas law gives insurers 45 days to decide on paper claims and 30 days on electronic submissions from participating providers.6State of Texas. Texas Code Insurance Code 843.338 – Deadline for Action on Clean Claim Member-submitted paper claims can run longer than electronic provider claims, so plan on the full 45-day window.

Track the claim by logging in to Blue Access for Members and checking the claims history section. The status will show as pending, in review, or finalized. When BCBSTX finishes, you’ll get an Explanation of Benefits showing the total billed, what the plan covers, and what you owe or have already paid. Read the EOB carefully. If something looks wrong, you have a limited window to dispute it.

If the Claim Is Denied

BCBSTX has a two-level appeals process.

Level 1: Internal Appeal

You have 180 days from the date you receive the denial notice to file a Level 1 appeal. Send a written appeal to the Claim Review Section at the same Dallas P.O. Box, or call 1-866-355-5999 to start the process by phone.7Teacher Retirement System of Texas. How to File Medical Claims Appeals Include supporting evidence: updated lab results, additional medical records, or a letter from your doctor explaining why the treatment was necessary. A different BCBSTX physician who was not involved in the original denial reviews it, and you’ll get a written decision.

Level 2: External Review

If the internal appeal is denied, you can request external review within four months of that denial. An accredited Independent Review Organization (IRO), separate from BCBSTX, examines the case. BCBSTX has five business days after the request is approved to send the IRO all relevant documentation, and the IRO must issue a written decision within 45 days.7Teacher Retirement System of Texas. How to File Medical Claims Appeals The IRO’s decision is final. For urgent medical situations, an expedited external review can produce a decision within 72 hours.8HealthCare.gov. External Review
