California CTC Form 41-4: Fingerprints, Transcripts, and Fees

California CTC Form 41-4, the Application for Credential Authorizing Public School Service, is the Commission on Teacher Credentialing’s paper application for teaching credentials, specialist permits, and related documents. Most applicants cannot use it. The Commission requires online filing through an educator account for the common credential types, and reserves the paper form for a narrow set of unique situations. If your credential falls into the online-required group, a mailed 41-4 will not be processed.

When You Can Use the Paper Form

Online submission is mandatory for Single Subject and Multiple Subject Teaching Credentials, Education Specialist Teaching Credentials, Child Development Permits at all levels, and 30-Day Substitute Permits. The paper Form 41-4 is limited to situations the online system doesn’t accommodate: extensions by appeal, American Indian languages credentials, and eminence credentials for Single Subject authorization, along with extensions by appeal for Multiple Subject and Education Specialist credentials.1Commission on Teacher Credentialing. Check the CTC’s online portal for your credential type first. Paper is the fallback.

Set Up Your Educator Account

Online applicants need a CTC educator account before filing. The Commission’s “Your Educator Account” page has a link to create one and a separate help page for login problems.3Commission on Teacher Credentialing. Get Your Fingerprints Cleared

Every applicant must obtain fingerprint clearance from the California Department of Justice and the FBI, processed through the Commission specifically. Clearance from another employer, agency, or state does not count.4Commission on Teacher Credentialing. The method depends on where you live. California residents use Live Scan, submitting Form 41-LS at an authorized site. DOJ results typically come back in three to seven business days, and you pay the Live Scan operator directly for rolling and processing fees. Out-of-state residents cannot use Live Scan. They must submit ink-rolled prints on the specific FD-258 cardstock provided by the CTC, with a $49 processing fee by check or money order payable to the CTC. DOJ processing for cards runs 10 to 12 weeks, and the DOJ will reject any other cardstock even when a fingerprinting agency substitutes its own.4Commission on Teacher Credentialing. On Form 41-LS, leave the pre-filled fields alone (ORI, Type of Application, Job Title, Agency Address, Mail Code, Level of Service). Enter your home address and personal descriptors in the fields marked with an asterisk. Sign the form; the DOJ requires your signature to confirm you have read the Privacy Notice and Privacy Act Statement. If you are resubmitting after a prior rejection, record the original ATI number from your previous Live Scan receipt in the field provided.5Commission on Teacher Credentialing. The Commission will not review your fingerprint history until you have submitted an application and paid the required fees. If you’re filing from out of state, start the FD-258 process early; the 10-to-12-week wait can stall an otherwise complete file.

Completing Form 41-4

The application collects the same information whether you file online or on paper: personal identification, educational history, professional fitness disclosures, and the specific credential you are requesting.

Personal Information

You must provide your full Social Security Number or Individual Tax Identification Number. Under 42 USC §666 and California Family Code §17520, an application without it will be returned.6Commission on Teacher Credentialing. Professional Fitness Questions

Six professional fitness questions cover arrests, criminal convictions, changes in education employment status resulting from misconduct, and disciplinary actions taken against any professional license you hold.8Commission on Teacher Credentialing. Answer honestly even if a conviction was later expunged or dismissed. California Penal Code Section 1203.4 states that a dismissal after probation does not relieve you of the obligation to disclose the conviction on any application for licensure by a state or local agency.10California Legislative Information. For each “yes” answer, attach a written explanation and supporting documentation such as court records or sentencing reports. Include evidence of rehabilitation or completion of court-ordered requirements where applicable. The CTC cross-references your disclosures against DOJ and FBI fingerprint results, so omissions tend to surface during review.

Credential Type

The final section asks you to specify the exact credential, certificate, or permit you’re seeking. California Education Code Section 44225 authorizes basic teaching credentials for kindergarten through grade 12, credentials for adult and vocational education, specialty credentials in areas like bilingual and special education, and service credentials for administrators, counselors, psychologists, library media teachers, and other positions.11California Legislative Information. Transcripts

Official transcripts from regionally accredited colleges or universities are a standard attachment for most credential applications. The CTC accepts paper and electronic transcripts, each with its own rules.

Paper transcripts must be printed on security paper or carry an official raised seal. If you ordered through a clearinghouse, include a cover letter from the degree-granting institution confirming the physical copy was sent directly to you. Electronic transcripts must be sent to the Commission directly from a digital credential service. Parchment users select the Commission as the authorized recipient. Your transcript must show a conferral date, degree granted date, or degree issued date. Enrollment or matriculation dates alone don’t prove degree completion. Coursework must be non-remedial, with a minimum grade of C- for general courses and B- for courses satisfying the Basic Skills Requirement. Fees

The standard application fee for most credentials, certificates, and permits is $100. Some documents cost less:

  • Certificate of Clearance: $50
  • Certificate of Completion of Staff Development: $45
  • Upgrading a Certificate of Eligibility to a Preliminary Credential: $50
  • Upgrading a Child Development Permit within 3 years of initial issuance: $50
  • Adding or changing an employment restriction: $50
  • Name change (Form 41-NC): no fee

For paper applications, pay by check or money order made out to the Commission on Teacher Credentialing (or “CTC”). No cash. A bounced check triggers a $20 service charge, and the Commission may void your credential until all fees are paid. Before you drop it in the mail, confirm the fee is correct, your fingerprint clearance is complete or in progress, and any required transcripts have been sent through the channels above.

Processing Times and Tracking

Paper applications take two to three weeks just to be logged into the CTC’s system after arrival.14Commission on Teacher Credentialing. Track progress through your CTC educator account. The Commission contacts you at the email on file when it needs more information. Once approved, your credential appears in the CTC’s public “Search for an Educator” database, which districts use to verify authorization.16Commission on Teacher Credentialing. If Your Application Is Denied

When the Committee on Credentials recommends denial or adverse action, you can request reconsideration. If you disagree with the Committee’s recommendation after that formal review, you can request an administrative hearing.15Commission on Teacher Credentialing.
