California DMV Form INF 1100, titled “Commercial Employer Pull Notice Enrollment or Deletion of Drivers,” is the paper form a commercial employer uses to add drivers to, or remove them from, the state’s Employer Pull Notice (EPN) program.1California Department of Motor Vehicles. Commercial Employer Pull Notice Enrollment or Deletion of Drivers – INF 1100 Before you print it, check one thing: as of April 1, 2026, California Code of Regulations, Title 13, Section 350.47 requires EPN participants to submit documents, request and receive driver records, and pay invoices electronically through the DMV portal, so the paper INF 1100 may no longer be the right path for your account.2California Department of Motor Vehicles. What You Need Before You Start
Two pieces of information have to be on hand before the form makes sense. First, your company’s EPN requester code. The DMV assigns this code when you first set up your EPN account, and it links every enrolled driver record back to your account.3California Department of Motor Vehicles. Commercial Employers If you have never enrolled before, you do not yet have a requester code, and the INF 1100 alone will not open an account for you — start with the EPN program page and the companion enrollment forms described below.
Second, you need the California driver license number for each driver you are adding or removing. Transposed digits enroll the wrong person or push your submission back for correction, so verify each number against the physical license rather than a copy of a copy.
Filling Out the Form
Download the INF 1100 PDF from the California DMV website.1California Department of Motor Vehicles. Commercial Employer Pull Notice Enrollment or Deletion of Drivers – INF 1100 The form asks for your company’s legal name, your requester code, and the driver license number for each driver, along with a mark indicating whether you are enrolling or deleting that person. Enrollment and deletion share the same form, so one submission can do both if that reflects your current roster changes.
An authorized representative of the company has to sign the form with an original (wet) signature, certifying that the information is accurate and complete.1California Department of Motor Vehicles. Commercial Employer Pull Notice Enrollment or Deletion of Drivers – INF 1100 A scanned signature or typed name will not satisfy the paper process.
Deletions matter as much as enrollments. When a driver leaves the company or stops operating a covered vehicle, submit a deletion so the DMV stops generating reports (and billing) for that person. The statute requires timely notification when an enrolled driver’s employment ends, so deletion paperwork should move as quickly as hiring paperwork does.
Fee, Payment, and Where to Mail It
The fee is $5 for each driver you enroll. Pay by check or money order made out to the California Department of Motor Vehicles. Do not mail cash. One check covering the total is acceptable when you are enrolling multiple drivers on the same submission.1California Department of Motor Vehicles. The April 2026 Electronic Requirement
Starting April 1, 2026, all EPN participants have to handle document submissions, driver record requests, record delivery, and invoice payments through the DMV’s electronic portal under Title 13, Section 350.47 of the California Code of Regulations.2California Department of Motor Vehicles. Employer Pull Notice Program Once your account is enrolled electronically, you add and delete drivers online rather than mailing the paper INF 1100. The DMV has described its digital service as roughly ten times faster than the mail process.4California Department of Motor Vehicles. Employers can also authorize a DMV-approved EPN agent to manage the account and driver records. Authorizing an agent requires a separate form, the INF 2110 (EPN Agent Authorization). Approved agents use the DMV’s Secure File Transfer (SFT) website for submissions and record retrieval.2California Department of Motor Vehicles. Employer Pull Notice Program
If you are setting up a new EPN account in 2026, go directly to the electronic portal. The paper INF 1100 remains a valid document to reference and to understand what information you will be entering, but submission is now a digital workflow for commercial employers.
Companion Forms That May Need to Go With It
The INF 1100 rarely moves on its own. Depending on your situation, one or more of the following may be required with your submission:
- INF 1101, Authorization for Release of Driver Record Information, used when adding a driver who must authorize release of their record.
- INF 1102 and INF 1103/1103A, supporting forms that may be required during initial enrollment or when you change account details.
- INF 2110, EPN Agent Authorization, if you are routing your account through an approved agent.

Who Should Be Filing the INF 1100 at All
The form is for commercial employers subject to California Vehicle Code Section 1808.1. That section applies to employers whose drivers operate vehicles requiring a Class A or Class B license, a Class C license with a special endorsement such as hazmat or passenger transport, or any certificate for driving a school bus, youth bus, farm labor vehicle, or similar regulated transport. It also covers charter-party carriers, passenger stage corporations, and permitted taxicab companies. Motor carriers who contract with independent drivers in these categories carry the same enrollment obligation as direct employers.5California Legislative Information. Government agencies with drivers in covered roles also participate in EPN, but they enroll through a separate track and should not use the commercial INF 1100.2California Department of Motor Vehicles.
