California Medical Board License Renewal Requirements: CME, Fees, and Deadlines

To keep practicing in California, you renew your medical license with the Medical Board of California every two years, pay the $1,206 biennial fee, and certify that you have completed 50 hours of approved continuing medical education. There is no grace period for practice: the moment your license expires, seeing patients becomes illegal, even though you have a short window to submit the paperwork without a late penalty.

When Your License Expires

Your license expires at midnight on the last day of the month in which it was originally issued, on a two-year cycle.1California Legislative Information. California Business and Professions Code BPC 2423 The Board starts sending email reminders 180 days out, so if your inbox is current, you have roughly six months of runway.2Medical Board of California. P&S – Renew

Two different clocks matter here. You cannot see patients one day past expiration. Separately, you have 30 days after expiration to submit your renewal and fee without a late penalty. The first clock governs your right to practice; the second governs how much the paperwork costs you.

Continuing Medical Education

You need at least 50 hours of approved CME during each two-year cycle, all completed within the 24 months before your expiration date.3Medical Board of California. Continuing Medical Education Courses must come from a provider the Board recognizes, such as one accredited by the ACCME or the California Medical Association.

California law also requires that CME courses incorporate curriculum on cultural and linguistic competency.4California Legislative Information. California Business and Professions Code 2190-1 Research-only courses with no direct patient-care component are exempt, as are out-of-state providers. If you take courses from California-based, ACCME-accredited providers, this content is usually baked in.

One-Time Pain Management Course

Every California physician must complete a one-time, 12-hour CME course in pain management and the treatment of terminally ill patients. Those 12 hours count toward the 50-hour biennial total, not on top of it.3Medical Board of California. Continuing Medical Education If you completed it in a prior cycle, you are done with it for good.

Keep Your Records

Hold onto certificates for at least four years. The Board audits for compliance, and you will need to produce the course title, dates, length, sponsoring organization, and accrediting body.3Medical Board of California. Continuing Medical Education

Disclosures and Your Address of Record

The renewal form asks whether any disciplinary action has been taken against your license and whether you have been convicted of a crime since your last renewal.2Medical Board of California. P&S – Renew Incomplete or inaccurate answers can delay processing or trigger a Board investigation.

Separately, Business and Professions Code Section 2021 requires you to report any change to your Address of Record within 30 days, which you can do through BreEZe.5Medical Board of California. P&S Practice Information An outdated address means you may miss renewal notices, and missing a notice does not excuse a late filing.

How to Submit the Renewal

The fastest route is the Department of Consumer Affairs’ BreEZe online portal, where you answer the disclosure questions, certify CME compliance, and pay by credit card in one session with immediate confirmation.6Medical Board of California. BreEZe Resources Center

Paper renewal still exists, but allow four to six weeks for processing.2Medical Board of California. P&S – Renew A payment sitting in a mail queue does not extend your right to practice. If you are inside the last few weeks of your window, use BreEZe.

Fees

The current biennial renewal fee is $1,206 for licenses expiring through December 31, 2026.7Medical Board of California. Fees It breaks down as:

  • Base renewal fee: $1,151
  • Steven M. Thompson Physician Corps Loan Repayment Program surcharge: $25
  • CURES (Controlled Substance Utilization Review and Evaluation System) surcharge: $30

For licenses expiring on or after January 1, 2027, the base rises to $1,255, bringing the total with surcharges to $1,310.8California Legislative Information. California Business and Professions Code 2435

Late Fees and the Five-Year Cliff

The penalty structure escalates the longer you wait:

  • Within 30 days of expiration: pay only the standard $1,206, but do not practice.
  • After 30 days: a delinquency fee of 10 percent of the renewal fee (about $115) is added.8California Legislative Information. California Business and Professions Code 2435
  • After 90 days: a penalty fee of 50 percent of the renewal fee (about $576) is added on top.7Medical Board of California. Fees

At the 90-day-plus mark, you are looking at roughly $1,897 to restore a license that would have cost $1,206 on time. You still cannot see patients until the Board processes the renewal and reactivates the license.

If your license has been expired less than five years, you can still renew online through BreEZe.9Medical Board of California. Inactive License Renewal Past five years, the standard online path closes and you face a formal reinstatement process with additional documentation and Board review.

Inactive Status If You Are Not Practicing

If you are retired, on extended leave, or practicing only in another state, you can renew on inactive status instead of letting the license lapse.9Medical Board of California. Inactive License Renewal Inactive status waives the CME requirement, but you still renew every two years and pay the fee. You cannot practice medicine in California while inactive. When you are ready to go active again, you submit a restoration application and show CME compliance, which is far easier than reinstating a license that has sat expired past the five-year mark.

Federal Credentials to Check at the Same Time

Your California license is not the only credential on a clock. Review these when you renew:

DEA registration. If you prescribe controlled substances, your DEA registration runs on a separate three-year cycle with its own fee.10Federal Register. Registration and Reregistration Fees for Controlled Substance and List I Chemical Registrants The MATE Act, effective June 2023, also requires a one-time, eight-hour training on treating patients with opioid or other substance use disorders, which you must complete before your next DEA renewal.11SAMHSA. Waiver Elimination (MAT Act) This federal training does not override California’s own pain management and substance use CME rules; both apply.

NPI record. Federal regulations require you to notify the National Plan and Provider Enumeration System within 30 days of any change to your required data, including practice address.12Federal Register. National Plan and Provider Enumeration System (NPPES) Data Changes No one sends a reminder. When you update your Address of Record with the Board, update NPPES the same day.

OIG exclusion list. If you participate in Medicare, Medicaid, or other federal health programs, confirm periodically that you do not appear on the OIG’s List of Excluded Individuals and Entities. Exclusion bars you from any federal health care program payment, and employers who hire excluded individuals can face civil monetary penalties.13U.S. Department of Health and Human Services, Office of Inspector General. Exclusions The search is free on the OIG website and takes a few minutes.
