Pick Your Doctor Before You Pick the Network

Your primary care provider determines which network you join, so start with the doctor. CalOptima’s online provider directory at caloptima.org/en/find-a-provider lets you search by plan, location, specialty, and language spoken. Printable directories exist too: Volume 1 covers primary care doctors, hospitals, and general providers, and Volume 2 covers specialists. As you browse, write down the doctor’s full name, the health network they belong to, and whether they are accepting new patients. Because your PCP also controls specialist referrals within the network, it pays to pick someone whose network already includes the specialists you see.

CalOptima’s contracted Medi-Cal health networks currently include AltaMed Health Services, AMVI Care Health Network, CHOC Health Alliance, Family Choice Health Network, Noble Mid-Orange County, Optum, Prospect Medical Group, Regal Medical Group, and United Care Medical Network. What to Write on the Form

The form is short. The trouble is precision: a wrong digit or a missing signature sends it back and delays your enrollment. You will need to fill in:

  • Your full legal name as it appears in state Medi-Cal records.
  • Your CalOptima Member ID number, printed on the ID card in your welcome packet.
  • Your date of birth and current home address, so the system matches you to the correct record.
  • The health network name and its provider identification number. The guide in your welcome packet lists these codes, and the provider directory has them too.
  • Your chosen PCP’s name. If the form asks for the doctor’s National Provider Identifier, you can find it in the directory or ask the doctor’s office.
  • Your signature and the date. Under Title 22 of the California Code of Regulations, Section 53882, enrollment is not valid until the member voluntarily signs and dates the form and the state verifies Medi-Cal eligibility.4Legal Information Institute. Adding family members to the same network means entering each dependent’s name, date of birth, and Member ID. Check every Member ID before signing. A transposed digit is the most common reason forms come back.

Three Ways to Submit It

You have three options for turning in your selection:

  • Register at member.caloptima.org with your Member ID number, log in, and use the portal’s built-in feature to change your network or PCP. The portal is available around the clock and creates an immediate digital record of the change.6CalOptima Health. Portals

Whichever route you take, keep a copy of the signed form or a note of the date you called and the person you spoke with. If processing goes sideways, that record is your proof.

If You Do Not Return the Form

CalOptima will assign a doctor for you, typically one within 10 miles of your home address.1CalOptima Health. CalOptima Health New Member Orientation: Medi-Cal Auto-assignment might place you with a PCP who does not speak your language, is inconvenient to your schedule, or belongs to a network that does not include the specialists you already see. Sending in the form first avoids the change process later.

When Your Choice Takes Effect

Once CalOptima receives your selection, the change generally takes effect on the first day of the following month. This tracks Section 53882, which provides that a member’s name is added to the approved enrollment list effective the first day of a given month.4Legal Information Institute. CalOptima then mails a confirmation letter and a new ID card showing your health network and assigned PCP. Check the details before your first appointment. If anything looks wrong, call Customer Service instead of waiting until you arrive at the office.

Changing Networks or Doctors Later

You are not locked in. CalOptima allows you to change your health network and PCP every 30 days.7CalOptima Health. The same first-of-the-month effective date applies. If you switch mid-month, keep seeing your current PCP until the new assignment starts.

Help in Your Language

If English is not your primary language, you have the right to language assistance at no cost. Providers that receive federal Medicaid funds are required to make language services available to people with limited English proficiency under Title VI of the Civil Rights Act.8Medicaid.gov.
