Can I Get My CDL Back After a Downgrade in PA?

If your Pennsylvania commercial license was downgraded within the last two years for a routine reason like an expired medical card, you can get it back by filing a current Self-Certification Form (DL-11CD) and making sure a new DOT medical certificate reaches PennDOT electronically. If the downgrade is older than two years, or if it was triggered by a Drug and Alcohol Clearinghouse prohibition, the path is longer. Here is how to get your CDL back after a downgrade in PA, step by step, based on why it happened and how long it has been off your license.

Start by Checking Your Eligibility

Before you spend money on a physical or testing fees, confirm three things.

Your underlying non-commercial license must be valid and active. PennDOT will not restore commercial status on top of a license that is suspended or revoked, so clear any non-commercial issue first.

Your driving record has to support restoration. Major violations such as DUI or leaving the scene can trigger federal disqualification periods that block a CDL return no matter how clean your paperwork is.

If the downgrade came from the federal Drug and Alcohol Clearinghouse, your status in that database must read “not prohibited” before PennDOT will process anything. That requires the full return-to-duty process described below.

Why Your CDL Was Downgraded Determines the Path Back

The most common trigger is an expired Medical Examiner’s Certificate. PennDOT changes your medical status to “not certified” and removes your commercial designation within 45 days if a new certificate is not provided.1Commonwealth of Pennsylvania. Self-Certification/Medical Examiner’s Certification FAQs If a medical examiner rescinds your certification outright, the downgrade happens immediately.

Failing to file a current Self-Certification Form (DL-11CD) has the same effect. Without a self-certification on file telling PennDOT what kind of commercial driving you do, the agency treats you as it would a driver with no valid medical status and begins downgrade procedures.1Commonwealth of Pennsylvania. Self-Certification/Medical Examiner’s Certification FAQs

Since November 2024, a third cause is in play. State licensing agencies are now required to downgrade any CDL holder flagged as “prohibited” in the FMCSA Drug and Alcohol Clearinghouse.2Drug & Alcohol Clearinghouse. Clearinghouse II and CDL Downgrades: State Compliance Begins Restoration after this type of downgrade is significantly more involved than fixing an expired medical card.

The Paperwork Path: Downgraded Less Than Two Years

If your CDL has been downgraded for under two years and the trigger was medical or self-certification, you can restore it without retesting. You need two things in PennDOT’s system: a current DL-11CD and a current medical certificate.

Filing the Self-Certification Form (DL-11CD)

The DL-11CD tells PennDOT how you intend to operate commercially.3Pennsylvania Department of Transportation. Self-Certification Form DL-11CD You pick one of four categories. Non-Excepted Interstate (NI) is the most common choice and requires a Medical Examiner’s Certificate. Non-Excepted Intrastate (NA) also requires a medical card but limits you to Pennsylvania routes with a “K” restriction. The two excepted categories, Excepted Interstate (EI) and Excepted Intrastate (EA), cover specific exempt activities and do not require a medical card.

If you are not actively driving but want to keep your CDL alive while you schedule a physical, you can select EI or EA, then submit a new DL-11CD changing your category when you return to work that requires medical certification.3Pennsylvania Department of Transportation. Getting the DOT Physical to PennDOT

If you select NI or NA, you need a current DOT physical from a medical examiner on the FMCSA National Registry. You can find one by city or ZIP on the Registry site.4FMCSA National Registry. Welcome to the National Registry of Certified Medical Examiners

This is where restorations most often stall. The examiner transmits your results electronically to PennDOT through the National Registry; you do not mail the card yourself.3Pennsylvania Department of Transportation. Self-Certification Form DL-11CD Before you leave the clinic, confirm the examiner has actually submitted the certificate. If it never lands in PennDOT’s system, your restoration sits with no explanation.

A DOT physical typically runs $100 to $150. The certificate is generally valid up to two years, though the examiner may issue a shorter one if a health condition requires more frequent monitoring.

The Retest Path: Downgraded More Than Two Years

Once a downgrade exceeds two years, PennDOT treats restoration as a new application. You will need to retest.

That means obtaining a Commercial Learner’s Permit first by completing the DL-31CD application, submitting a DL-11CD, paying the permit fee, and passing the knowledge tests covering vehicle inspection, cargo handling, safe driving, hours of service, and other core topics.5Commonwealth of Pennsylvania. Commercial Driver License CDL Learners Permit FAQs You must hold the CLP at least 15 days before taking the skills test, and you get three attempts at the skills test on that permit. The full sequence has to be completed within one year of your initial Knowledge Test Authorization. Skills tests are offered at PennDOT branch offices and through third-party programs statewide.6Commonwealth of Pennsylvania. CDL Skills Test

One useful boundary on entry-level driver training (ELDT): if you held a valid CDL before February 7, 2022, the ELDT rules are not retroactive, and you are not required to complete training for the class you previously held.7Federal Motor Carrier Safety Administration. Entry-Level Driver Training ELDT Anyone qualifying for a skills test exemption under 49 CFR Part 383 is also exempt from ELDT.8Commonwealth of Pennsylvania. If the Downgrade Was Triggered by the Clearinghouse

A Clearinghouse-triggered downgrade requires the full return-to-duty process before PennDOT will touch your file.2Drug & Alcohol Clearinghouse. Clearinghouse II and CDL Downgrades: State Compliance Begins

You select a DOT-qualified Substance Abuse Professional (SAP) from a list provided by your employer or a consortium/third-party administrator. The SAP conducts an initial assessment and prescribes education or treatment. After you complete that program, the SAP re-evaluates you and decides whether you are eligible for a return-to-duty test. Only a negative result on that test changes your Clearinghouse status from “prohibited” to “not prohibited.”9Federal Motor Carrier Safety Administration. The Return-to-Duty Process and the Clearinghouse

Even after your status clears, PennDOT will query the Clearinghouse before processing your restoration. State licensing agencies are required to check the database before issuing, renewing, upgrading, or restoring any commercial license.10Drug & Alcohol Clearinghouse. CDL Downgrades If the query still shows “prohibited,” the transaction is blocked. Depending on the SAP’s treatment recommendations and testing schedules, the process can take weeks or months. Start early.

Getting Endorsements Back

Restoring your base CDL does not automatically restore endorsements. Hazmat (H), tanker (N), doubles/triples (T), and passenger (P) each have their own requirements.

Hazmat is the most involved. It requires a separate TSA Security Threat Assessment, which in Pennsylvania you apply for through your local PennDOT driver license center rather than the standard TSA online portal. The assessment includes fingerprinting and a background check. The fee is $85.25, or $41.00 if you hold a valid Transportation Worker Identification Credential (TWIC). TSA recommends starting at least 60 days before you need the endorsement, since processing can exceed 45 days.11Transportation Security Administration. HAZMAT Endorsement The assessment is valid for five years.

If you are inside the no-retest window, endorsements like tanker or doubles/triples can be restored along with your base CDL through the same paperwork. If you are retesting, you will typically need to pass the relevant knowledge test for each.

One ELDT boundary worth knowing: ELDT applies to H, P, and S endorsements only when you are seeking them “for the first time.” If you previously held the endorsement, the retroactivity exception protects you.

Where to Send Your Paperwork

Self-Certification/Medical Examiner’s Certification FAQs If a fee applies, include a check or money order payable to PennDOT as indicated in your restoration requirements letter.

If you need to retest, start at a PennDOT driver license center to take your knowledge tests and apply for the CLP. After the 15-day hold, schedule the skills test through PennDOT’s exam scheduling system or at a third-party testing location.6Commonwealth of Pennsylvania. CDL Skills Test

Before gathering documents, request a Restoration Requirements Letter from PennDOT. It lays out the exact steps, fees, and conditions for your specific case, which can vary from the general rules above.12Commonwealth of Pennsylvania. Request a Drivers License Restoration Requirements Letter

What It Will Cost

For a simple expired-medical-card restoration inside the two-year window, plan on roughly $150 to $200: the DOT physical plus a $42.50 duplicate license fee.13Commonwealth of Pennsylvania. Payments and Fees

If you have to retest, PennDOT charges separate fees for the permit and license issuance. A CDL issued 25 to 48 months before expiration runs between $87.50 and $109.50.13Commonwealth of Pennsylvania. Payments and Fees

Hazmat restoration adds the TSA threat assessment: $85.25, or $41.00 with a valid TWIC.11Transportation Security Administration. HAZMAT Endorsement ELDT training, when it applies, runs from a few hundred dollars for endorsement courses to several thousand for full CDL programs. PennDOT may also charge a separate restoration fee depending on your circumstances; the exact amount appears in your Restoration Requirements Letter.

Drivers who need to retest or clear a Clearinghouse violation should budget considerably more, potentially $500 to $1,000 or above once training, testing, SAP evaluations, and multiple fees stack up.
