CCSD Shared Housing Verification Form: How to Complete, Notarize, and File

The CCSD Shared Housing Verification form lets a parent or guardian who lives in someone else’s home — with no lease, mortgage, or utility bill in their own name — prove where their child lives so the district can assign the correct zoned school. You pick up the form through CCSD’s online registration, gather a photo ID and two recent supporting documents, have the form notarized, and upload everything through the Parent Portal. Plan on up to one week for processing after the district receives a complete packet.

Is This the Right Form for Your Situation

Clark County School District assigns students to schools based on the parent or guardian’s official residence. If you can produce a utility bill, rent receipt, or lease in your name at your current address, you use the standard enrollment process and skip this form entirely.1Clark County School District. Nevada law requires a parent or guardian to enroll a child aged 6 to 18 in a public school within the district where the child resides.3Nevada Legislature. Nevada Code 392 – Statute 392.040 CCSD policy adds that a student cannot claim a relative’s or friend’s address for school assignment purposes except through a process authorized by NRS Chapter 392.4Clark County School District. CCSD Regulation R-5112 The Shared Housing Verification form is CCSD’s approved way to do that.

Two situations call for a different path, not this form. If your family is in a shelter, motel, car, or moving between places, contact CCSD’s Title I Homeless Outreach Program for Education at 702-855-6682; the McKinney-Vento Act guarantees immediate enrollment without standard residency documents.1Clark County School District. What to Gather Before You Start

Pull everything together before you begin. A missing piece stalls the whole application.

  • The Shared Housing Verification Request Form itself, available through the online registration portal or from your child’s assigned school. It has to be notarized before you submit.
  • A government-issued photo ID showing your current address at the shared home — a Nevada driver’s license, state ID card, or passport. If the address on your ID is out of date, update it at the DMV first. An address mismatch will delay processing.
  • Two additional supporting documents in your name at the shared-housing address, each dated within 90 days. Acceptable sources include a bank or insurance company, a utility company (phone or gas), a medical provider, a religious institution, or a government agency.6Clark County School District. A few practical notes. The 90-day window is firm, so a document older than that will be rejected even if everything else is in order. You are not limited to utility bills; bank statements, insurance letters, medical correspondence, and government mail all count. What matters is that each document ties your name to the shared-housing address.

Filling Out and Notarizing the Form

The form asks for identifying details about you, your child, and the person whose home you share. Enter your full legal name, your child’s name and date of birth, and the complete street address of the shared residence. The homeowner or leaseholder fills in their own information and signs, confirming that you and your child live there.

Both signatures have to be notarized. A notary verifies each signer’s identity and stamps the form, turning it into a sworn statement. Under Nevada law, a notary may charge up to $15 to take an acknowledgment on the first signature and $7.50 for each additional signature. A jurat — the certification that you swore to the truth of the statement — also costs up to $15 per signature. Electronic notarization runs up to $25 per notarial act.7Nevada Legislature. Nevada Code NRS 240 – Notaries Public and Commissioned Banks, UPS stores, and shipping centers commonly offer notary services, and your local library or your child’s school may know of free or low-cost options.

Before you walk into the notary, cross-check every name and address on the form against your ID and the two supporting documents. A small discrepancy, like an abbreviated street name on one document and the full spelling on another, is enough for CCSD to send the packet back.

How to Submit

CCSD takes the Shared Housing Verification form through two online routes. You do not drop it off at a school office.

  • If you are registering your child for the first time or for a new school year, log in to the Parent Portal at campusportal.ccsd.net, choose the Shared Housing option during the registration workflow, and upload the notarized form along with your supporting documents.
  • If you already submitted an online registration and later moved into a shared-housing situation, use the Alternate Shared Housing Application link on the district’s shared housing page to submit the documents separately.6Clark County School District. CCSD Shared Housing Department

Allow up to one week for processing. Applications are handled in the order they arrive, so during peak registration in late spring and summer, expect the full week.

After the District Processes Your Application

Once CCSD completes its review, your child is assigned to the school zoned for the shared-housing address. The district may follow up to confirm that the living arrangement is genuine, with residency checks handled through its residency office.

Plan to repeat this process each year. CCSD registration generally applies to the current school year, so families in shared housing should expect to refile or confirm the arrangement when registering for the next year. Keep your ID current and hold onto fresh supporting documents so the renewal goes quickly.

If you move out of the shared home mid-year, tell the school registrar right away. A truthful filing can become a false one if the facts change and the records do not.

Penalties for False Statements

Because the form is notarized, it carries the weight of a sworn statement. Under Nevada law, signing an affidavit that contains a false statement is perjury, a category D felony.8Nevada Legislature. Nevada Code NRS 199 – Crimes Against Public Justice That exposure falls on both the parent and the homeowner or leaseholder who co-signs. On top of that, CCSD can withdraw a student found to be living outside the declared zone and may pursue reimbursement for the cost of educating a non-resident student.

Language Help

If English is not your first language, ask the school’s front office for help with the form. Under Title VI of the Civil Rights Act of 1964, CCSD cannot deny or delay enrollment because a parent cannot read English-language documents.9U.S. Department of Education. Education and Title VI CCSD routinely provides Spanish-language materials and offers assistance in other languages as well.
