CCSD Sports Physical Form: Packet, Aktivate Upload, and Fees

To clear a student for sports at a Clark County School District school, you complete the CCSD sports physical form packet — NIAA Physical Form B, Form D, and (when applicable) Form E, plus the CCSD Authorization and Release and a Parent/Student Residency Affidavit — and upload the signed documents through the Aktivate online portal. The school’s athletic director reviews the upload and changes the student’s status from “Pending” to “Cleared.” No student may try out, practice, or compete until that clearance posts, and a new packet is required every school year.

The Forms in the Packet

The packet pulls together forms from two sources: the Nevada Interscholastic Activities Association and CCSD itself. A partial submission leaves the student stuck in Pending, so gather everything before you start the online registration.

  • NIAA Physical Form B (Pre-Participation History). A health history completed by the parent or guardian covering past injuries, surgeries, allergies, medications, and family medical history. The doctor reviews this before the hands-on exam.
  • NIAA Physical Form D (Physician Form). The actual exam, filled out and signed by a licensed physician (MD or DO), physician assistant, or advanced practice registered nurse. The provider checks vitals, heart and lung function, vision, musculoskeletal range of motion, and any conditions flagged on Form B.
  • NIAA Physical Form E (Health Questionnaire Interim Form). A shorter update used when a student’s full physical is still valid but a new school year has started. It captures health changes since the last exam.
  • CCSD Authorization and Release Form. A district form in which parents acknowledge the risks of athletic participation and authorize emergency medical treatment during practices or games.
  • Parent/Student Residency Affidavit. A sworn statement that the student lives within the school’s designated attendance zone.

The forms are posted on the NIAA’s website under its forms page and through individual school athletic department pages.1Nevada Interscholastic Activities Association. Getting the Physical Done

The exam is the step that trips most families up, usually because they wait until the week before tryouts and can’t get a doctor’s appointment. Schedule it early. Form D is valid for one calendar year from the date the provider signs it, so a physical done in May covers a student through the following May, spanning fall, winter, and spring sports with no repeat visit.

Start with Form B at home. Fill in every section of the health history, including questions about fainting, chest pain during exercise, asthma, prior concussions, and family history of sudden cardiac death. Don’t leave blanks. Unanswered items slow down the provider’s review and can get the upload rejected by the school. Bring the completed Form B to the appointment so the provider can review it alongside the exam.

The provider then completes Form D. It’s a standard head-to-toe sports screening: blood pressure, pulse, heart and lung auscultation, vision check, abdominal exam, and musculoskeletal assessment of the major joints. If the provider finds something that needs follow-up, such as a heart murmur, they may mark the student “cleared with conditions” or “not cleared” pending further testing. Only a signed Form D with a “cleared” determination satisfies the packet requirement.

If the student already has a valid physical on file but is entering a new sport season within the same school year, Form E can take the place of a full repeat exam. The parent notes any health changes, and if nothing significant has changed, the existing physical stays valid.

Uploading Through Aktivate

All completed forms go through Aktivate, the online platform CCSD uses district-wide for athletic registration. Older materials call it “Register My Athlete,” and the login page is still at registermyathlete.com.2Bonanza High School. Athletics – Register My Athlete

  • Create an account. A parent or guardian sets up a household account, then adds a student profile linked to the CCSD school.
  • Select the sport and season. Choose the specific sport and the current school year. Each sport needs its own registration even when the underlying physical and residency forms are the same.
  • Upload documents. Scan or photograph every page of Forms B, D, and E (if applicable), the CCSD Authorization Form, and the Residency Affidavit. Upload as a PDF or clear image. Every page must be legible. Blurry uploads and missing pages are the most common reason athletic directors send a packet back.
  • Complete the online acknowledgments. The portal may include digital consent forms covering topics like concussion awareness or sudden cardiac arrest information, depending on the sport and current state requirements.
  • Pay any applicable fees. Some fees are collected inside the portal during registration.

After submission, the student’s status shows as “Pending.” The athletic director reviews the uploaded documents against district records. Once everything checks out, the status changes to “Cleared,” and only then can the student step onto a field or court for that sport. Check the portal regularly. If a document is rejected, the reason appears in the student’s profile, and you can re-upload a corrected version without starting over.

Why the Residency Affidavit Matters

The Residency Affidavit isn’t a formality. Nevada’s athletic eligibility rules under NAC 385B.712 tie a student to the school in their attendance zone, and one address is designated as the student’s home for eligibility purposes when they first enroll, regardless of how many addresses the family may use.3Nevada Legislature. Nevada Administrative Code Chapter 385B – Residency of Pupil General Requirements

If the NIAA’s executive director determines that a student or parent falsified any records related to the student’s school assignment, the student becomes ineligible for all sports for two full years after the last game in which they competed. The school forfeits every contest in which the ineligible student played, and any team awards earned during that period must be returned.4Nevada Legislature. Nevada Administrative Code Chapter 385B – Penalties for Ineligible Pupils Students enrolled at a school based solely on an affidavit of residency, rather than actually living in the zone, are ineligible for 180 school days from the enrollment date, which effectively wipes out an entire sports year.

Concussion and Cardiac Acknowledgments

Nevada law requires that a student suspected of sustaining a concussion during practice or competition be removed from play immediately and not return until a qualified health-care provider gives written clearance. No coach, parent, or the student can override this; the written medical release is mandatory before the student participates in any physical activity again.

As part of registration, families may be asked to sign acknowledgments confirming they have reviewed information about the warning signs of concussion and sudden cardiac arrest. These acknowledgments are increasingly built into the Aktivate workflow rather than requiring a separate paper form. If you see them in the portal, complete them promptly. They count as clearance items just like the physical forms.

Fees

Playing a sport at a CCSD school usually involves several out-of-pocket costs, though amounts vary by school and sport. Most schools charge a participation or activity fee and a separate transportation fee that helps offset busing for away games. These charges are typically collected through the school’s online webstore or inside the Aktivate portal during registration. Some programs also charge for a “spirit pack” that includes a team jersey, practice gear, or other equipment. Each school sets its own schedule, so check with your athletic department for current amounts. Payment confirmation is often the final step before full clearance posts; an unpaid balance can hold up an otherwise complete packet.

After the Packet Is Cleared

Clearing the paperwork gets a student onto the team, but it doesn’t guarantee they can stay there. Nevada’s administrative code also sets academic thresholds that are checked at the end of each grading period, including a minimum 2.0 cumulative GPA and passing grades in at least two units during the preceding grading period.5Nevada Legislature. Nevada Administrative Code 385B.7515 – Passing Grades and Minimum Grade Point Average A student who transferred schools within 180 school days before a sport’s season may also face a separate NIAA eligibility review before competing.6NIAA. NIAA Eligibility Information for Students and Parents If either situation applies, raise it with the athletic director before you spend time on the physical, because a cleared packet won’t override an academic or transfer hold.
