For a WIC appointment, bring:
- A government-issued photo ID, or another photo ID plus a bill or letter addressed to you
- Proof of income such as a recent pay stub, tax return, or employer letter; if you receive SNAP, Medicaid, or TANF, documentation of that benefit works instead
- The child, or a note from your doctor confirming pregnancy
- Immunization records for any children being enrolled
For immunizations, bring records of previous vaccinations and the parent or guardian’s ID. If a non-parent adult brings a child under 18, that adult needs a notarized letter signed by the parent or guardian. Florida Code R. 64D-3.046 – Immunization Requirements for Public and Nonpublic Schools Uninsured children, Medicaid enrollees, and those whose insurance lacks first-dollar vaccine coverage may qualify through the federal Vaccines for Children program.
