The Collier County Subcontractor Affirmation Form is a one-page PDF that adds a licensed trade professional to an existing building permit, and you submit it through the county’s online portal at cvportal.collier.gov/cityviewweb or in person at 2800 Horseshoe Drive N, Naples, FL 34104. The form lets county staff confirm the subcontractor’s credentials before trade-specific inspections can be scheduled. Getting the fields right and verifying the license first is what keeps the permit moving.
When You Need to File One
You need an affirmation any time a subcontractor is added to a building permit after the initial application. If you listed the subcontractor when you first applied, the county can add those contact types at that stage and no separate form is required. Anyone brought in later needs the affirmation on file before their trade work can be inspected, and without it the permit cannot reach final approval.
Pick the Right Trade Box
The form has a checkbox for the subcontractor’s trade. Check only the one that matches the license the subcontractor actually holds:
- Electrical Contractor
- Mechanical Contractor
- Plumbing Contractor
- Roof Contractor
- Septic Contractor
- Low Voltage Contractor
- Pool Contractor (only when the permit was applied for by a general contractor)
- Contractor Other (with a blank line for trades not listed)
If one company performs work under two separate licenses, such as plumbing and mechanical, you’ll typically need a separate affirmation for each trade.
What Each Field Requires
The form is short, but staff cross-check what you enter against state licensing records, so accuracy matters.
- Permit Number: the number on the building permit the subcontractor is being added to. It appears on your permit card and in the online portal.
- Job Site Address: the street address exactly as it appears on the permit.
- Trade Category: the checkbox matching the subcontractor’s scope of work.
- Company Name: the subcontractor’s full legal business name as registered with the state.
- License Number: the subcontractor’s state or local contractor license number.
- Qualifier Name: the individual who holds the license on behalf of the company. This is the person whose technical competence qualifies the business to perform the work.
- Qualifier Signature and Date: the qualifier signs and dates the form.
The signer certifies that the trade information is true and correct to the best of their knowledge and belief. Because the certification goes to a government agency, false information can trigger penalties under Florida Statutes Section 837.06, which makes it a second-degree misdemeanor to knowingly make a false written statement intended to mislead a public servant. That carries up to 60 days in jail and a fine of up to $500.1Florida Senate. Florida Statutes 837.06 – False Official Statements
Verify the License Before You Submit
County staff will check the license after you submit, but two minutes of your own verification first prevents a rejection and resubmission. The Florida Department of Business and Professional Regulation keeps a free lookup at myfloridalicense.com, where you can search by name or license number. The results show the licensee’s name, profession, address, and current status.2Department of Business and Professional Regulation. How to Verify a License
You want to see “active” or “current.” An expired, suspended, or revoked license will cause the county to reject the affirmation. DBPR also offers a mobile app called “DBPR Mobile” and a phone line at (850) 487-1395 if you’d rather not search online.
How to Submit It
The county’s preferred method is uploading the completed form through the Growth Management Community Development public portal at cvportal.collier.gov/cityviewweb. The form itself notes that it can be uploaded once the subcontractor condition has been added to the permit.3Collier County Government. Collier County Subcontractor Affirmation Form If the portal is new to you, the county publishes a walkthrough called “Adding a Contractor and Qualifier to Contacts” on its E-Permitting help page.4Collier County, FL. Keep these upload requirements in mind:
- Files must be under 72 MB.
- Accepted formats include PDF, DOC, DOCX, XLS, and XLSX.
- If the document doesn’t fit a standard classification in the portal, upload it as “miscellaneous.”
You can also hand-deliver a physical copy to the permitting office at 2800 Horseshoe Drive N, Naples, FL 34104, or mail it in, though electronic uploads are processed faster.
What Happens After You Submit
Once the form is in, county staff verify the subcontractor’s license against state databases. When the credentials check out, the affirmation is linked to the permit and the trade’s inspections can be scheduled. If the license comes back expired or the information doesn’t match, the affirmation is rejected and you’ll need to correct and resubmit before inspections for that trade can move forward.
If You Pulled the Permit as an Owner-Builder
Owner-builders carry extra responsibility when adding subcontractors. Florida law requires owner-builders to personally supervise all construction on the property, and that supervisory role cannot be delegated to anyone who is not properly licensed for the work being performed.5The Florida Legislature. Florida Statutes 489.103 – Exemptions
The owner-builder disclosure you signed at permit application states this directly: you must verify that every person you hire holds the licenses required by law, and you may not hire an unlicensed person to act as your contractor. Anyone working on the project who is not independently licensed must be your direct employee, which triggers federal tax withholding and workers’ compensation obligations. If the county finds a violation, it can withhold final approval, revoke the permit, or pursue action for unlicensed activity against both you and the person doing the work.
Before the subcontractor starts, confirm their workers’ compensation coverage as well. Florida requires construction employers to carry it, and a gap in coverage can prompt a stop-work order on your project.6Florida Department of Financial Services. Construction Industry
