CRNA Requirements in California: Education, Exam, and Licensure

To practice as a Certified Registered Nurse Anesthetist in California, you need national CRNA certification from the NBCRNA plus a Certificate of Nurse Anesthetist issued by the California Board of Registered Nursing. Getting there means a Bachelor of Science in Nursing, at least a year of adult critical care experience, graduation from a doctoral-level nurse anesthesia program accredited by the Council on Accreditation, a passing score on the National Certification Examination, and a $500 application to the BRN backed by a current California RN license and a clean fingerprint-based background check.¹²

What You Need Before Applying to a Program

A BSN (or higher) from an accredited institution is the baseline. Most nurse anesthesia programs expect a minimum cumulative GPA of 3.0 on a 4.0 scale, and competitive applicants often sit at 3.5 or above.³

You also need at least one year of full-time registered nurse experience in an adult critical care setting. Programs are specific about what counts. Time in a Medical ICU, Surgical ICU, Coronary Care Unit, or Trauma ICU qualifies. Time in the emergency department, post-anesthesia care unit, operating room, or pediatric ICU generally does not.³ Programs often want that experience to be recent, within the last two years, and many successful applicants work well beyond the one-year floor.

Completing a COA-Accredited Nurse Anesthesia Program

Any student entering a nurse anesthesia program on or after January 1, 2022, must earn a doctoral degree, either a Doctor of Nursing Practice (DNP) or a Doctor of Nurse Anesthesia Practice (DNAP).⁴ The program must be accredited by the Council on Accreditation of Nurse Anesthesia Educational Programs. California law tracks this by requiring graduation from a BRN-approved program that considers COA standards.²

Expect a full-time program of about 36 months that blends classroom work with clinical rotations. The COA sets a floor of 2,000 clinical hours. On case counts, students who matriculated before January 1, 2026, must complete at least 700 cases across a range of surgical and procedural areas; students entering on or after that date face a higher minimum of 750 cases.⁵ Tuition varies widely, from under $50,000 at some public institutions to over $165,000 at private universities.

Passing the National Certification Examination

After graduating, you take the National Certification Examination (NCE), administered by the National Board of Certification and Recertification for Nurse Anesthetists. A passing score earns the CRNA credential, and California will not process your state application without it.⁶

The NCE is a computer-adaptive test with 100 to 170 questions and a three-hour time limit. Because the difficulty of each question adjusts based on your previous answers, two test-takers see different exams.⁷

Retake Rules

There is no mandatory waiting period between attempts within a given year. You can take the NCE up to four times within one year of your program completion date. If you still have not passed, you get up to four more attempts in a second year. Each retake requires a new online application and fee; a prior eligibility notification cannot be reused.⁸ Exhausting all eight attempts without passing may mean completing additional education before becoming eligible again.

Applying to the California Board of Registered Nursing

Once you hold the CRNA credential, the final step is applying to the California Board of Registered Nursing for the Certificate of Nurse Anesthetist. You must already have a current, clear, and active California RN license before the BRN will process the application.⁹ If you hold an RN license from another state, you need to get California RN licensure first through endorsement.

The application goes through the BRN’s online Breeze system. Along with the completed form and the $500 fee, you must provide proof of graduation from your accredited program and proof of NBCRNA certification.¹ The BRN will not accept transcripts or supporting documents from you directly. Transcripts must be sent electronically by a certified vendor such as Parchment or the National Student Clearinghouse, and other required documents must come straight from the school or the NBCRNA.¹⁰ The application also requires your Social Security Number or Individual Taxpayer Identification Number, as called for by Business and Professions Code Section 30.

Fingerprinting and Background Check

Every applicant must complete a fingerprint-based criminal background check processed through both the California Department of Justice and the FBI. These clearances commonly take two to four months and are the longest bottleneck in the whole process.⁹

Temporary Certificate for Endorsement Applicants

If you are applying for California RN licensure by endorsement and do not yet have a permanent California RN license, you can apply for a Temporary Certificate of Nurse Anesthetist (TC/NA) so you can begin practicing while fingerprint clearances are processing. Eligibility requires a temporary California RN license with a complete endorsement application pending, plus a complete nurse anesthetist certification application on file with the BRN.⁹ If you already hold a permanent California RN license, the TC/NA does not apply; your certificate will simply be issued once the background check clears.

What California CRNAs Can Do

Under California Business and Professions Code Section 2826, anesthesia services include preoperative, intraoperative, and postoperative care and pain management for patients receiving anesthesia. CRNAs provide these services pursuant to an order from a physician, dentist, or podiatrist. The scope also covers selecting and administering medication under such an order and providing emergency, critical care, and resuscitation services.²

Medicare separately requires physician supervision of nurse anesthetists as a baseline rule, but state governors can opt out. California opted out in 2009, so CRNAs practicing in Medicare-participating facilities in California are not subject to the federal supervision mandate. Individual hospitals and health systems may still impose their own supervision policies, and the state-law requirement that CRNAs act pursuant to a physician, dentist, or podiatrist order remains in place.

Keeping Both Credentials Active

Practicing as a California CRNA means maintaining two separate credentials: your national CRNA certification and your underlying California RN license. Lose the national one and the state certificate goes with it.

NBCRNA Continued Professional Certification

The NBCRNA requires you to renew your CRNA credential every four years through the Continued Professional Certification (CPC) program. For CRNAs renewing in 2026 or 2027, two components are required: 60 MAC Ed credits and 40 MAC Dev credits, for a total of 100 credits over the four-year cycle.¹¹ Most CRNAs must also complete at least one Core Module from each of four clinical domains: Airway Management, Applied Clinical Pharmacology, Human Physiology and Pathophysiology, and Anesthesia Equipment, Technology, and Safety.¹²

California RN License Renewal

Your California RN license must be renewed every two years. Each renewal requires 30 contact hours of continuing education completed during the preceding renewal period. These are general nursing CE hours, separate from CPC credits, though some courses may count toward both. One thing that catches people off guard: during the first two years immediately following initial California licensure, you are exempt from the CE requirement.¹³ After that exemption window, failing to complete 30 hours before renewal will prevent you from renewing your active license.¹⁴
