  • Official educational transcripts sent directly from your accredited institution to the DPH. Copies you forward yourself won’t be accepted.

  • A signed authorization allowing the state to review your criminal history and any prior disciplinary actions.
  • Verification of any out-of-state license, sent directly from that state’s licensing authority.
  • Professional references or employment history where the profession requires them.

How to Verify a Connecticut Health Care License

Renewing Your License

Continuing education requirements are set by profession. Physician assistants, for example, must meet the continuing medical education requirements of the National Commission on Certification of Physician Assistants, plus at least one contact hour every two years on prescribing controlled substances and pain management.7Justia. What Happens If You Miss the Deadline

A missed renewal date does not immediately shut down your practice. A void license can’t just be renewed. You have to apply for reinstatement, which is more involved. Physicians seeking reinstatement must submit a current curriculum vitae, verification of at least 25 hours of continuing medical education completed in the year before applying, verification of all out-of-state licenses (current or expired), and a letter from a recent clinical employer or colleague confirming safe practice.8Connecticut Department of Public Health. Practicing Across State Lines

Connecticut belongs to two interstate compacts that matter if you work in more than one state.

Complaints and Disciplinary Actions

The DPH investigates complaints against licensed practitioners, but its authority is limited to action against the license itself. A Note on Practicing Without a License

Providing health care services in Connecticut without a valid license is a criminal offense. Classification and penalties depend on the profession, and consequences can include fines and imprisonment. Practicing on a void or expired license can also damage future licensure elsewhere: every state licensing board asks about prior unauthorized practice, and a “yes” answer complicates every application that follows.
