Farmington Office Hours and Appointments
The Farmington office is open Monday through Friday, 8:00 a.m. to 5:00 p.m., and closes on state holidays.2Utah Driver License Division. Office Locations One service it does not handle is commercial driver licenses. If you need a CDL, you’ll have to visit a DLD office outside the county.
Before you go, fill out the application online and schedule an appointment through the DLD’s scheduler.3Utah Driver License Division. Appointment Scheduler Walk-ins without a submitted application should expect delays. Check the DLD website for closures before your visit, especially around holidays.
What to Bring
Utah licenses are REAL ID compliant, and as of May 7, 2025, a REAL ID is required for domestic air travel and access to federal facilities. That makes it worth getting the documents right the first time. Everything must be an original or a certified copy with a raised seal, stamp, or signature. Photocopies, faxes, and laminated birth certificates aren’t accepted.4Utah Driver License Division. Required Documents
Plan to bring:
- Proof of identity, such as a U.S. passport or certified birth certificate
- Your Social Security card or another document showing your full Social Security number
- Two documents proving your Utah address, such as a utility bill and a bank statement
- Documentation of any name change since your identity documents were issued
Foreign documents must be translated into English by a certified translator. Document approval sometimes takes more than one day and can require a second visit, so bring more than you think you need.4Utah Driver License Division. Required Documents

Getting Your First Utah License
At your appointment, the DLD administers an eye exam. You need 20/40 vision and at least 90 degrees of peripheral vision in at least one eye.5Utah Driver License Division. Vision Requirements for Drivers Bring your glasses or contacts if you wear them; passing only with correction puts a corrective-lenses restriction on your license.
The written knowledge test has 50 questions, and you need at least 40 correct to pass. You can attempt it twice in the same day. After three failures, you’ll pay a second fee before testing again.6Utah Driver License Division. Written Knowledge Test Depending on the license type, you may also need a behind-the-wheel driving skills test.
Once you’ve passed and paid, the DLD issues a temporary paper license you can use immediately. Your permanent card is printed at a central facility and mailed to your home, typically arriving in four to six weeks. Confirm your mailing address on the application so it doesn’t land somewhere else.
Renewing Your License Online
You don’t always have to drive to Farmington. The DLD mails a letter with a PIN to eligible drivers about 90 days before their license expires.7Utah Driver License Division. Regular Driver License Renewal With that PIN, you can renew through the online portal for $42 instead of $52.
If you’ve moved since your last renewal, update your address online before starting the renewal. Doing so removes the requirement to provide two forms of address verification.7Utah Driver License Division. Regular Driver License Renewal If you don’t receive a PIN letter or aren’t eligible for online renewal, you’ll need to come in.
Moving to Utah with an Out-of-State License
If you’ve recently moved to Davis County, Utah expects you to get a Utah license once you establish residency. The state treats you as a resident if you set up a permanent home here, stay longer than six months, take a job other than seasonal work, or do things like register a vehicle or enroll children in school.
Bring the same identity and residency documents as any other applicant. A valid license from another U.S. state counts as proof of your driving privilege, but you’ll surrender it when you apply.8Utah Driver License Division. Regular Driver License Original Expect the standard vision screening and possibly the written knowledge test. The fee is the same $52 as any other original adult Class D license.
Teen Drivers
Utah uses a graduated system with three stages: learner permit, provisional license, and full license. A teen can apply for a learner permit at age 15.9Utah Driver License Division. Learner Permit Before moving to a provisional license, they must hold the permit at least six months and complete at least 40 hours of supervised driving, including a minimum of 10 hours at night.10Utah Legislature. Utah Code 53-3-211 Up to five of those hours can be on an approved simulator. Anyone 18 or younger must also complete a driver education course approved by the DPS commissioner.11Utah Legislature. Utah Code 53-3 – Uniform Driver License Act The minimum licensing age is 16.
Provisional license holders face two main restrictions. They cannot drive between midnight and 5:00 a.m. except in limited circumstances such as traveling to or from work or responding to an emergency. For the first six months, or until age 18, they also cannot carry passengers who aren’t immediate family unless a licensed driver age 21 or older is in the front seat.12Utah Driver License Division. Reinstating a Suspended License
If your license has been suspended, revoked, or denied, you’ll need to go through the DLD’s reinstatement process before driving again. Each suspension action may carry its own reinstatement fee. For a points-based suspension from accumulated traffic violations, the fee is $40.13Utah Driver License Division. Reinstate a License
If the suspension involved driving without insurance, you’ll also need an SR-22 filed by your insurer with the DLD.14Utah Driver License Division. SR22 Insurance Additional testing may apply depending on the circumstances. Driving on a suspended license is a separate offense, so finish reinstatement before getting back on the road.
Other Services at the Farmington Office
Motorcycle Endorsement
You can add a motorcycle endorsement to your Class D license at age 16 or older. It requires a separate 25-question written test and a rider skills test, though completing an approved motorcycle rider course waives the skills test.15Utah Driver License Division. Motorcycle Endorsement The endorsement costs $18. All riders under 21 must wear a helmet in Utah.
Mobile Driver License
Utah offers a mobile driver license (mDL) that lives on your smartphone. You’ll need a valid Utah license or ID already on file, then download the “Get Mobile ID” app from the Apple App Store or Google Play and complete the selfie verification.16Utah Driver License Division. mDL Registration The app is free for the first six months and then costs $1.99 per year. Acceptance varies, so keep your physical card as backup.
