Licensed dispensaries are the only legal retail source of cannabis in the state, and every recreational purchase is subject to a 9% state sales and use tax.²

The age rule is absolute. Military status, employment, or any other circumstance does not lower the threshold. If you are under 21, the only legal way to buy from a Maryland dispensary is through the medical program.¹

What a Medical Card Gets You That Walking In Does Not

The card is not just a formality for people with serious conditions. Four practical benefits set medical purchases apart from recreational ones.

No sales tax. Cannabis purchased with a medical card is fully exempt from the 9% sales and use tax. If you spend $300 a month, that is about $324 a year back in your pocket.²

Higher purchase limits. Adult-use buyers are capped at 1.5 ounces of flower, 12 grams of concentrate, or 750 milligrams of THC in other products, and those categories are combined rather than stacked. Medical patients can purchase up to 120 grams of dried flower or 36 grams of THC in other products on a rolling 30-day cycle. That is a substantial gap for anyone using cannabis routinely.³

High-potency products. Certain products with higher THC concentrations are reserved for registered patients and cannot be sold to recreational customers.²

Access at 18 to 20. The adult-use program starts at 21. The medical program starts at 18, and children under 18 can participate if a parent or legal guardian registers as their caregiver.¹⁴

There is also a service perk: dispensaries are required to provide patient-only operating hours or dedicated service lines, so registered patients are not waiting behind recreational foot traffic.¹

When the Card Is Worth Getting, and When It Is Not

If you buy cannabis a few times a year for personal use, the tax savings will not offset the provider fee and the time spent registering. A photo ID at the door is enough.

If you buy regularly, need more than 1.5 ounces of flower a month, are between 18 and 20, or want access to the high-potency products only patients can buy, the card starts paying for itself quickly. The 9% tax exemption alone can recover a provider evaluation fee within a few months of consistent buying.

How to Get a Maryland Medical Card

The Maryland Cannabis Administration runs the program, and the process has three steps.

Register With the MCA

Create an account through the MCA’s Onestop portal. You will need a valid government-issued photo ID and two documents proving Maryland residency, each from a different issuing agency and dated within the past 90 days. Acceptable proofs include a utility bill, bank statement, property tax bill, lease agreement, car insurance policy, pay stub, or W-2. Once approved, the MCA assigns you a patient license number.⁵

Get a Provider Certification

You need a written certification from a healthcare provider registered with the MCA, confirming a qualifying condition. Maryland’s list includes severe or chronic pain, severe nausea, seizures, severe or persistent muscle spasms, glaucoma, PTSD, cachexia, anorexia, wasting syndrome, or another chronic condition where other treatments have not worked. That last category is broad enough that many chronic conditions can qualify at a provider’s discretion.⁶

The provider uploads the certification to your MCA account. You pay the provider separately for the evaluation. The certification is valid for one year, but you have to make at least one dispensary purchase within 120 days of the certification date or it lapses and you will need a new one.⁵

Get Your ID Card

Once approved, log into your Onestop account and print a paper copy of your patient ID at no charge. A physical card costs $25. Either version works at the dispensary. When purchasing, bring both your state-issued photo ID and your patient ID or patient number.⁷⁵

Caregivers

If a patient cannot visit a dispensary in person, a registered caregiver can buy on their behalf. Caregivers must be at least 21, register with the MCA, and purchase a $25 caregiver ID card. One caregiver can serve up to five patients at a time. For patients under 18, only a parent or legal guardian can serve as the caregiver.⁸

Out-of-State Medical Cards Do Not Work Here

Maryland does not recognize medical cannabis cards from other states. A card from anywhere else will not get you the tax exemption, the higher purchase limits, or access to the medical-only product tier.⁹ You can still buy adult-use cannabis at any licensed Maryland dispensary as long as you are 21 or older with a valid photo ID. There is no residency requirement for recreational purchases.

Adult-Use vs. Medical Purchase Limits

The purchase caps are the clearest picture of what the card actually buys you.

Adult-use buyers 21 and older can purchase and possess up to 1.5 ounces of flower, 12 grams of concentrate, or products totaling 750 milligrams of THC. These are combined limits across product types.¹

Medical patients receive a 30-day supply based on their provider’s certification. The standard is 120 grams of dried flower or 36 grams of THC in processed products, on a rolling 30-day cycle. Providers can certify more or less depending on medical need.³

Exceeding the personal use amount but staying within the “civil use amount” is a civil offense with a maximum $250 fine. Going further is a misdemeanor: up to six months in jail and a $1,000 fine for a first offense, with steeper penalties on repeat convictions. For anyone under 21, possessing the personal use amount is a civil offense with a maximum $100 fine, not a criminal charge.¹⁰
