Does AHCCCS Cover Sports Physicals? Well-Child Billing and Age Limits

AHCCCS does cover sports physicals, but only for members under 21, and only when the exam happens as part of a regular well-child visit. There is no separate sports physical benefit and no out-of-pocket cost for the family. For adults 21 and older, sports physicals are specifically excluded from coverage.

Getting a Sports Physical for a Child on AHCCCS

Call your child’s primary care provider and schedule a well-child visit. Tell the scheduler your child also needs sports clearance paperwork completed for school athletics. The provider handles both in the same appointment, and there is no extra charge for filling out the forms.1Mercy Care. Prevention and Wellness Provider Outreach Manual

Bring the forms with you. Arizona high school athletes governed by the Arizona Interscholastic Association need four documents completed: a parent/student evaluation form, the medical provider’s examination form, an acknowledgement form, and a consent-to-treat form.2AIA Online. Physical Forms The evaluation form asks for a detailed medical history covering heart symptoms, prior injuries, medications, family cardiac conditions, and mental health questions, so filling in what you can at home shortens the visit.3AIA Online. Some community health centers also hold seasonal sports physical events that accept AHCCCS. Why It’s Billed as a Well-Child Visit

AHCCCS does not reimburse providers for a standalone sports physical. Instead, sports clearance is folded into the Early and Periodic Screening, Diagnostic, and Treatment benefit (EPSDT) that federal Medicaid law requires every state to provide to enrolled children and young adults under 21.6Medicaid.gov. Early and Periodic Screening, Diagnostic, and Treatment A well-child visit already covers everything a sports physical requires: heart and blood pressure checks, vision, hearing, flexibility, bone and muscle health, height, and weight, along with immunizations, developmental screenings, and behavioral health checks.7Arizona Complete Health.
