Does Alabama Medicaid Cover Dental for Adults?

Alabama Medicaid covers dental care for adults only in one situation: if you are 21 or older, have full Medicaid eligibility, and are pregnant or within 60 days postpartum. Everyone else over 21 is excluded, with one narrow exception for hospital charges when a dental problem is making a serious medical condition worse. Because Alabama has not expanded Medicaid, many low-income adults can’t enroll in the program at all.

The Pregnant Adult Dental Benefit

Alabama Medicaid began paying for dental services for pregnant adults on October 1, 2022. To qualify, you must be 21 or older, have full Medicaid eligibility (not a limited-benefit category), and be currently pregnant or in the postpartum window.1Alabama Medicaid Agency. The benefit is broader than many people expect. Covered procedures include:2Alabama Medicaid. Dental Program Overview

  • Checkups and cleanings every six months, fluoride treatments, sealants, and x-rays
  • Fillings, crowns and buildups, and root canals
  • Extractions for diseased or painful teeth
  • Scaling and root planing (often requires prior authorization from Medicaid, which your dentist’s office arranges)3Alabama Medicaid Agency. Postpartum Extension, Dental Coverage, and Other Changes

There is no annual dollar cap. As long as the procedure is medically necessary, falls within the covered categories, and happens during your eligibility window, Medicaid pays. That compares favorably with many private dental plans, which commonly cap annual benefits around $1,000 to $2,000.4Alabama Medicaid. FAQ Dental for Pregnant Recipients

What the Benefit Doesn’t Cover

Even for pregnant recipients, Alabama Medicaid excludes:2Alabama Medicaid. Dental Program Overview

  • All-porcelain crowns
  • Periodontal (gum) surgery

The 60-Day Postpartum Cutoff

Dental coverage ends at the close of the month in which your 60th day postpartum falls.5Alabama Medicaid Agency. ALERT Maternity Dental Benefit Coverage 60 Days Postpartum This trips people up because Alabama extended general maternity Medicaid eligibility from 60 days to 12 months postpartum.6Alabama Medicaid. Reminder About Maternity Medicaid Eligibility The 12-month extension covers medical care. The dental benefit still ends at 60 days.

If you need dental work, schedule it during pregnancy or in the first few weeks after delivery. Waiting several months postpartum will likely put the appointment outside your dental eligibility window.

What You Pay

Nothing. Alabama Medicaid waives copayments for anyone receiving pregnancy-related services, so you should owe zero out of pocket for covered procedures.7Alabama Medicaid Agency. Alabama Medicaid Covered Services and Copayments Providers can’t bill you beyond that copayment amount for covered care.

Finding a Dentist Who Takes It

Services only get paid for when performed by a dentist enrolled with Alabama Medicaid. The agency maintains a page with provider directories and public health clinic listings, and recipients who need help can call Medicaid at 1-800-362-1504.8Alabama Medicaid. Find a Medicaid Dental Provider When you call an office, confirm two things: that the practice is currently enrolled with Alabama Medicaid, and that it accepts the pregnant adult dental benefit specifically. Not every Medicaid-enrolled dentist treats adults under this benefit.

The Narrow Hospital Exception for Everyone Else

Outside the pregnant adult benefit, Alabama Medicaid lists adult dental care as a non-covered service. One limited exception exists: when an adult’s dental problem is actively worsening an underlying medical condition serious enough to require hospitalization, Medicaid may pay the hospital facility fee and anesthesia charges. The dental procedure itself is still on the patient.9Alabama Medicaid Agency. Dentist 13 – Provider Manual

So if a dental infection sends you to the hospital and your doctor determines it is threatening your overall health, Medicaid can cover the hospital stay. The oral surgeon’s bill for the extraction performed during that stay still falls on you. This provision does not function as emergency dental coverage in any practical sense.

Why Many Adults Can’t Even Enroll

Alabama has not expanded Medicaid under the Affordable Care Act. Most non-disabled, non-elderly adults without minor children cannot qualify for Medicaid at any income level. Parents and caretaker relatives of minor children qualify only if household income is below 18% of the federal poverty level. Pregnant women qualify up to 146% of the federal poverty level.

If you are not pregnant, not disabled, and have no minor children in your household, you have no path to Alabama Medicaid, and therefore no path to any Medicaid dental benefit.

Low-Cost Dental Options Without Medicaid

Federally Qualified Health Centers

Alabama has roughly three dozen Federally Qualified Health Centers that offer dental services. Federal law requires them to see patients regardless of ability to pay and to use a sliding fee scale based on income. If your household income is at or below the federal poverty level, you qualify for a full discount and may owe only a nominal charge. Partial discounts apply between 100% and 200% of the poverty level, with at least three discount tiers in that range.10Health Resources & Services Administration. Dental School Clinics

The University of Alabama at Birmingham School of Dentistry runs clinics where students and residents treat patients under faculty supervision. Fees run up to roughly 50% below private practice, depending on whether you’re seen in the student clinic, a specialty resident clinic, or a faculty practice.11School of Dentistry.
