Does Apple Health Cover Dental Implants? Exceptions and Alternatives

Washington Apple Health does not cover dental implants for adults. The exclusion is written into state regulation and applies to the implant, the abutment, any implant-supported crown, and even the maintenance or removal of implants you already have.1Washington State Legislature. WAC 182-535-1100 Enrollees age 20 and younger may have a narrow path to coverage through a federal benefit called EPSDT, but approval is decided case by case. Adults who need implants will almost always need to use a covered alternative or pay privately.

Why Adults Are Excluded

Under WAC 182-535-1100, the Washington Health Care Authority bars all implant services for adults age 21 and older. That includes the implant itself, abutments, implant-supported crowns and retainers, and the maintenance, repair, or removal of existing implants.1Washington State Legislature. WAC 182-535-1100 The HCA’s own summary of adult dental benefits lists implants under “not covered services.”2Washington State Health Care Authority. Apple Health Adult Dental Coverage

Switching managed care plans does not help. The exclusion is set at the state level, and no Washington Medicaid fee schedule lists reimbursement for implant-specific procedure codes.3American Dental Association. The Exception to Rule Request

Apple Health has a mechanism called an Exception to Rule, or ETR, that can occasionally authorize a service the program normally excludes. To qualify, the enrollee has to show that the service is medically necessary, cost-effective, consistent with good medical practice, and that no covered alternative can meet the patient’s needs as effectively.5Washington Law Help. Exception to Rule for Washington Apple Health

The provider submits a written request with clinical documentation explaining why the patient’s situation is distinct from the general population and why dentures or other covered options will not work. Fee-for-service members submit directly to the HCA; managed care members go through their plan’s prior authorization process. A written decision must be issued within 15 working days, or within five business days if additional information was requested. If the request is denied, you have 90 days from the denial notice to appeal.5Washington Law Help. Exception to Rule for Washington Apple Health

The bar is high. Scenarios that might support an ETR include severe bone loss that prevents the use of conventional dentures, or implants needed to anchor a facial prosthesis after trauma or cancer treatment. There is no public data on how often dental implant ETRs succeed in Washington, so plan for an uphill effort rather than a reliable route to coverage.

Coverage for Children Under 21

The implant exclusion does not automatically apply to enrollees age 20 and younger. These patients are covered under the Early and Periodic Screening, Diagnostic and Treatment program, a federally mandated benefit that requires states to cover medically necessary care for children even when the service sits outside the standard adult benefit. WAC 182-535-1100 itself carves out EPSDT from its exclusions.1Washington State Legislature. WAC 182-535-11006Washington State Health Care Authority. EPSDT Noncovered Services

A dental provider submits a prior authorization request, and the HCA reviews it case by case using the medical necessity definitions in WAC 182-500-0070 and WAC 182-501-0165.6Washington State Health Care Authority. EPSDT Noncovered Services This is not an automatic approval; EPSDT opens the door to consideration. Approval is more plausible when the child has a congenital dental condition, has lost teeth from trauma, or has anatomy that makes conventional prosthetics unworkable. If the request is denied, families can choose to pay privately, but a waiver for noncovered services must be signed.

What Apple Health Will Pay For Instead

For adults, Apple Health covers these tooth replacement options with prior authorization:

  • Complete dentures.
  • Resin-based partial dentures.
  • Denture replacements, if the existing dentures are at least three years old.
  • Rebasing and relining, once in a three-year period, provided at least six months have passed since the original dentures were placed.4National Indian Health Board. Washington Adult Dental Coverage

Children have broader dental benefits, including crowns, fillings, sealants, fluoride treatments, and orthodontic services when medically necessary.7DentistLink. Apple Health Covers Dental Care

Paying for Implants on Your Own

A single dental implant in Washington runs roughly $2,400 to $4,900, depending on the source and what the estimate includes. Spokane tends toward the lower end at around $3,500, while the Seattle-Bellevue area runs $4,800 to $5,200. Bone grafts, crowns, and other components can add hundreds to thousands more.8CareCredit. Several Washington resources can soften that cost:

  • The University of Washington School of Dentistry’s teaching clinics in Seattle charge roughly 30 to 40 percent less than private practices and accept Apple Health patients. Treatment is provided by supervised dental students, so appointments take longer and waits can stretch to several weeks. The clinic does offer implant services, but patients must meet the school’s educational case criteria during a screening appointment.9University of Washington School of Dentistry. • DentistLink, a free referral service from the Arcora Foundation, connects Apple Health enrollees and uninsured patients to providers. Referral specialists are available at 844-888-5465 and can help with transportation and other logistics.11DentistLink. Dentists That Accept Apple Health
  • Community health centers such as SeaMar Community Health Centers and Community Health Centers of Snohomish County serve low-income patients on a sliding fee basis.12Snohomish County Dental Society. Low Cost Dental Care
  • Dialing 211 or visiting win211.org connects callers to public clinics and reduced-cost dental services in their area.13Washington State Dental Association. Reduced Cost Dentistry

Third-party financing through services like CareCredit, which many dental practices accept, can spread the cost into monthly payments. Health Savings Accounts and Flexible Spending Accounts let you pay with pre-tax dollars if you have access to one.
