Washington Apple Health does cover GLP-1 medications, but only for specific medical conditions. The state’s Medicaid program pays for drugs like Ozempic, Mounjaro, Wegovy, and Zepbound when they’re prescribed for type 2 diabetes, cardiovascular risk reduction, moderate-to-severe MASH, or moderate-to-severe obstructive sleep apnea in adults with obesity. It does not cover any GLP-1, or any other drug, when the prescription is for weight loss alone.
That exclusion sits in Washington Administrative Code 182-530-2100(1)(b)(i), which says the Medicaid agency “does not cover a drug prescribed for weight loss or gain.”1Washington State Legislature. A May 2026 court ruling has begun to test whether that blanket approach can stand, but as of now the exclusion is still in force.
Coverage for Type 2 Diabetes
The most common path to Apple Health coverage for a GLP-1 is a type 2 diabetes diagnosis. Medical policy 27.17.00 covers dulaglutide (Trulicity), exenatide, liraglutide (Victoza), semaglutide (Ozempic, Rybelsus), and tirzepatide (Mounjaro).2Coordinated Care Health. To qualify, you generally need a type 2 diabetes diagnosis or an HbA1c of 6.5% or higher within the past 12 months. Age minimums are 10 for dulaglutide, exenatide, liraglutide, and tirzepatide, and 18 for semaglutide. The medication cannot be combined with another GLP-1 agonist or a DPP4 inhibitor.
If the prescribed drug is preferred on the Apple Health Preferred Drug List, meeting those basic criteria is typically enough. Non-preferred agents require step therapy: documented failure of metformin at the maximum tolerated dose and failure of at least two preferred GLP-1 products, each tried for 90 continuous days or more, to bring HbA1c below 7%.2Coordinated Care Health. Patients with type 2 diabetes plus a comorbid condition, such as chronic kidney disease, cardiovascular disease, MASH, or obstructive sleep apnea, may qualify under expanded criteria that have their own documentation requirements.3Molina Healthcare. Antidiabetic GLP-1 Form
Reauthorization after the initial approval period requires showing that HbA1c has either dropped to 7% or below, or improved from baseline.2Coordinated Care Health. Antidiabetics GLP-1 Agonists Policy
Coverage for Heart Disease and MASH Without Diabetes
Wegovy (semaglutide) has its own Apple Health policy, No. 61.25.20.AA, for patients who do not have diabetes. Effective February 1, 2026, coverage is available in two situations.4Washington State Health Care Authority. Semaglutide Medical Policy
The first is cardiovascular risk reduction. Adults 18 or older with a BMI of at least 27, established cardiovascular disease (such as a prior stroke, heart attack, or symptomatic coronary or peripheral artery disease), and an HbA1c no higher than 6.5% may qualify. The patient must also be on concurrent secondary prevention therapy such as antiplatelet, blood pressure, or cholesterol medication. The purpose is preventing major adverse cardiovascular events, not weight loss.
The second is MASH treatment. Adults 18 or older with moderate to severe noncirrhotic MASH (fibrosis stage F2 or F3, confirmed by biopsy or validated scoring tools) and at least one cardiovascular risk factor may qualify. HbA1c must be 6.5% or below. Patients with diabetes are routed to the diabetes policy instead.

Coverage for Obstructive Sleep Apnea
Zepbound (tirzepatide) has its own policy, No. 61.25.25.AA, for moderate to severe obstructive sleep apnea in adults with obesity. Weight-loss-only use is excluded. Qualifying criteria: age 18 or older, BMI above 30, a polysomnogram confirming moderate to severe sleep apnea (AHI or RDI of 15 or more events per hour without a positive airway pressure device), and an HbA1c no higher than 6.5%. The patient must also document consultation about positive airway pressure therapy and whether they declined it, could not tolerate it, or are actively using it.5Washington State Health Care Authority. Why Weight Loss Alone Is Not Covered
The state rule barring coverage for weight-loss drugs applies uniformly, and every Apple Health managed care plan follows it. Most enrollees receive coverage through a managed care organization, and those plans must follow the Apple Health Preferred Drug List and the Health Care Authority’s clinical pharmacy policies.6Wellpoint Washington. - Wellpoint (formerly Amerigroup) follows the statewide Apple Health drug list and clinical pharmacy policies.6Wellpoint Washington. Pharmacy Information
Switching Apple Health plans will not get you around the exclusion, because the rule sits at the state level.
A Court Ruling That Could Loosen the Weight-Loss Exclusion
On May 4, 2026, the Washington Court of Appeals decided Simonton v. Washington State Health Care Authority (No. 86988-4-I). Judge Ian Birk, writing for the court, held that health insurance plans in Washington cannot maintain blanket exclusions for medications treating a recognized disability just because the treatment is for that disability. Obesity is a protected disability under Washington law, so the court found that categorically refusing to cover obesity medications may violate the state’s nondiscrimination statute, RCW 48.43.0128.10Washington Courts. The ruling reversed the dismissal of claims brought by Jeannette Simonton, a nurse insured through the Health Care Authority who had been denied coverage for GLP-1 medications.11The Spokesman-Review. Insurers can still deny coverage for a particular drug with a “reasonable justification,” such as a finding that it is experimental, not effective, or not cost-effective. What they cannot do is refuse to cover an entire category of treatment for no reason beyond longstanding policy.10Washington Courts. Simonton v. HCA, No. 86988-4-I
The case was sent back to the lower court to decide whether the Health Care Authority had a reasonable, nondiscriminatory justification for denying Simonton’s coverage. Until that plays out, the WAC exclusion is still what plans apply. But the ruling gives members a legal argument they did not have before.
What to Do If You Are Denied
If your Apple Health plan denies a GLP-1 prescription, you have several ways to push back.
Start with an appeal to your managed care plan. Instructions come with the denial letter. Submit the appeal in writing. If you want to keep receiving an existing treatment while the appeal is pending, file within 10 days of the denial notice. If the plan upholds the denial, request a fair hearing through the Office of Administrative Hearings, either in writing or by calling 1-800-583-8271. To keep benefits during the hearing, file within 10 days of the letter upholding the first denial.12Washington Law Help. Appeal Denial Your Health Plan
For a denial that rests on a benefit exclusion rather than a clinical criterion, you can also submit an Exception to Rule request. That is a written request asking the plan or the Health Care Authority to cover a service not normally covered. File within 90 days of the denial. After Simonton, members denied GLP-1 coverage for obesity have a new argument available: that a categorical exclusion is discriminatory under Washington’s nondiscrimination statute. The legal question is not fully resolved, but the door is no longer shut.
