Blue Cross and Blue Shield of Minnesota does not cover Wegovy on its standard individual and small-group formulary. The drug is absent from the BasicRx covered drug list that most BCBS MN individual and small-group plans use, and the plan’s policy is that drugs not on the list are typically not paid for.1MyPrime. 2025 MN 4T BasicRx Individual Small Group Formulary That said, coverage can look very different from one plan to the next, especially for employer-sponsored plans, and members can ask for a formulary exception when the drug is excluded.
How to Check Your Own Plan
Your specific plan’s drug list is the only reliable answer. BCBS MN publishes covered drug lists through its pharmacy benefit manager, Prime Therapeutics, and members with individual and family plans can search theirs at the MyPrime website through the BCBS MN “Find a pharmacy or search drug list” page.2Blue Cross and Blue Shield of Minnesota. Find a Pharmacy or Search Drug List You’ll need the name of your formulary, which appears in your health plan materials or can be found by calling the number on the back of your member ID card.

Why Employer Plans Can Be Different
BCBS MN offers many plan types, and each can carry its own pharmacy network and covered drug list. An individual or family plan bought on the marketplace will not have the same formulary as an employer plan. With employer-sponsored coverage, the employer itself usually decides whether weight-loss medications are a covered benefit, and BCBS MN tells members with employer plans to check with their human resources department because that coverage may be administered by a different company altogether.3Blue Cross and Blue Shield of Minnesota. When Wegovy is covered, it tends to sit on a higher formulary tier, which means a bigger share of the cost falls on the member.4SingleCare. Is Wegovy Covered by Blue Cross Blue Shield
Prior Authorization If Your Plan Covers It
Even on plans that do include Wegovy, prior authorization is almost always required. Your doctor has to submit documentation showing you meet the clinical criteria before the plan will pay. Across BCBS affiliates, those criteria generally track the FDA label:
- Adults with a BMI of 30 or higher, or a BMI of 27 or higher with at least one weight-related condition such as high blood pressure, type 2 diabetes, or high cholesterol.
- Adolescents aged 12 and older with a BMI at or above the 95th percentile for their age and sex.
Plans often also want documentation that you’re following a reduced-calorie diet and exercise program, and some require you to have tried a non-GLP-1 weight-loss medication within the previous six months. Renewal usually requires evidence of at least 5% weight loss from baseline.5SingleCare. Wegovy Prior Authorization Criteria Blue Cross Blue Shield
Step therapy may also apply, meaning you could be required to try a lower-cost alternative first. Whether it applies depends on your formulary, and BCBS MN points members to its online drug search tool to check.6Blue Cross and Blue Shield of Minnesota. Step Therapy Drug Program
Requesting a Formulary Exception
If Wegovy isn’t on your plan’s covered drug list, BCBS MN allows members and their doctors to file a drug list coverage exception. This is a formal request asking the plan to cover a medication that isn’t normally on the formulary, backed by a doctor’s explanation of why the drug is medically necessary for you.7Blue Cross and Blue Shield of Minnesota. Prior Authorization for Prescription Drugs
There are a few ways to submit:
- Online, using the Prime Therapeutics drug coverage exception form.
- By paper, with the Minnesota Uniform Form completed by your doctor and sent by fax or mail.
- Electronically through CoverMyMeds.
Members with questions about the process can call BCBS MN at 1-800-262-0823.7Blue Cross and Blue Shield of Minnesota. Prior Authorization for Prescription Drugs
One caveat matters here. If your plan categorically excludes weight-loss drugs, you usually don’t have appeal rights for a weight-loss indication. However, when a doctor is prescribing Wegovy to reduce cardiovascular risk in a patient with established heart disease, a letter of medical necessity may still be submitted even when weight loss is otherwise excluded from the plan.8Novo Nordisk. If You’re on Minnesota Medical Assistance
If your coverage is Medical Assistance rather than a private BCBS MN plan, the answer flips. Wegovy is a covered medication under Minnesota Health Care Programs, subject to prior authorization.12Minnesota Department of Human Services. Anti-Obesity Medications PA Criteria The state program requires documentation of a reduced-calorie diet and increased physical activity, with BMI thresholds that match the FDA label: 30 or higher for adults without risk factors, or 27 or higher with a weight-related condition. For adolescents aged 12 to 17, the threshold is a BMI of 30 or higher with body weight above 60 kg.
Initial approval runs six months. Renewal for another 12 months requires evidence of at least 5% weight loss for adults, or a 5% reduction in baseline BMI for adolescents. Renewals beyond 18 months require evidence that the weight loss is being maintained.12Minnesota Department of Human Services. Anti-Obesity Medications PA Criteria
If You Have Medicare
Traditional Medicare Part D does not cover Wegovy prescribed solely for weight loss. It does cover the drug for a narrower use: reducing the risk of major cardiovascular events in adults with established heart disease who also have obesity or are overweight.13Centers for Medicare & Medicaid Services. Information for Part D Plans – Medicare GLP-1 Bridge
Beginning July 1, 2026, a CMS demonstration called the Medicare GLP-1 Bridge will cover Wegovy for weight management for qualifying Medicare beneficiaries, running through December 31, 2027. Eligible beneficiaries will pay a $50 copay, and eligibility requires a prior authorization from the prescribing provider confirming the patient meets clinical criteria tied to BMI and specific comorbidities such as heart failure, hypertension, chronic kidney disease, or pre-diabetes.13Centers for Medicare & Medicaid Services.
