Blue Cross Blue Shield of Mississippi does not cover Zepbound under its standard plans. The insurer’s GLP-1 agonist medical policy (L.5.01.428) treats GLP-1 medications as medically necessary only for type 2 diabetes, and Zepbound, which is approved for chronic weight management, is not on the list of covered GLP-1 drugs.1BCBS Mississippi. GLP-1 Agonist Medical Policy A small number of self-insured employer groups administered through BCBS Mississippi have added it, and manufacturer programs from Eli Lilly can bring the cash price down if your plan excludes it.
Why Standard Plans Exclude It
The BCBS Mississippi GLP-1 policy names four medications that may be considered medically necessary, all for type 2 diabetes: Mounjaro, Ozempic, Rybelsus, and Trulicity. Approval requires a confirmed diabetes diagnosis, typically an A1C of 6.5% or higher. There are no diagnosis codes in the policy for chronic weight management or obesity.1BCBS Mississippi. GLP-1 Agonist Medical Policy
The policy does not spell out a weight-loss drug exclusion by name. It doesn’t need to. By tying every covered GLP-1 to type 2 diabetes, it reaches the same result for weight management prescriptions. That holds whether Zepbound is prescribed for obesity or for its other approved indication, moderate-to-severe obstructive sleep apnea in adults with obesity.
Check Whether Your Specific Plan Is an Exception
Coverage can vary by benefit plan, and BCBS Mississippi directs providers and members to its formulary search tool with the member ID on the card. Self-insured employer groups fund their own claims and can customize their formularies beyond the standard medical policy. The policy document identifies two Mississippi employer groups that have done so for Zepbound:
- Glenn Machine Works Self-Insured Group covers Wegovy and Zepbound without prior authorization, effective December 1, 2025.1BCBS Mississippi. GLP-1 Agonist Medical Policy
- Structural Steel Self-Insured Group covers all GLP-1 agonist medications without prior authorization, effective June 16, 2025.1BCBS Mississippi. GLP-1 Agonist Medical Policy
If you work for a self-insured employer, ask your benefits administrator whether Zepbound has been added, or run your member ID through the BCBS Mississippi formulary search before assuming the standard exclusion applies.
If You’re a Mississippi State or School Employee
State and school employees are on a separate plan, with CVS Caremark as the pharmacy benefit manager. The 2026 plan document lists Zepbound in its Prescription Drug Program section.2Mississippi Department of Finance and Administration. 2026 Plan Document – State and School Employees Life and Health Insurance Plan Prior authorization through CVS Caremark requires a BMI of 30 or higher (or 27 or higher with a weight-related condition such as hypertension or type 2 diabetes), documented participation in a comprehensive weight management program for at least six months before starting drug therapy, and use alongside a reduced-calorie diet and increased physical activity. To continue coverage, the patient must show at least a 5% loss of baseline body weight or maintenance of initial weight loss after at least three months on a stable dose.3CVS Caremark. Mississippi PA Request Form A 2017 version of the plan excluded weight-loss drugs outright, so the current criteria represent a change, though they are strict.4George Washington University – STOP. Mounjaro Is Not a Workaround
Zepbound and Mounjaro contain the same active ingredient, tirzepatide. Under standard BCBS Mississippi plans, Mounjaro is covered with prior authorization, but only for type 2 diabetes. Approval requires a confirmed diabetes diagnosis with an A1C of 6.5% or higher, use as an adjunct to diet and exercise, no concurrent DPP-4 inhibitor or other GLP-1 receptor agonist, and no FDA-labeled contraindications.1BCBS Mississippi. GLP-1 Agonist Medical Policy A patient without type 2 diabetes cannot get Mounjaro approved on the standard plan as a stand-in for Zepbound.
Effective April 1, 2026, BCBS Mississippi began requiring pharmacy review for all new GLP-1 agonist prescriptions, and prior authorization is specifically required for members with type 2 diabetes who are not already on an oral antidiabetic agent. A 2026 update removed the earlier requirement that patients first try or show intolerance to metformin.1BCBS Mississippi. GLP-1 Agonist Medical Policy
What Zepbound Costs If Your Plan Excludes It
Eli Lilly offers several programs that can lower the price:
- If you have commercial insurance that does cover Zepbound, the manufacturer savings card may bring the cost down to as little as $25 for up to a three-month supply, subject to annual savings limits of $1,300.5Eli Lilly – Zepbound. Zepbound Savings
- If you have commercial insurance that excludes Zepbound, the same program may bring the single-dose pen down to as low as $499 per month.5Eli Lilly – Zepbound. Zepbound Savings
- Paying out of pocket through LillyDirect, the KwikPen starts at $299 per month for the 2.5 mg dose, $399 for 5 mg, and $449 for 7.5 mg through 15 mg, provided you refill within 45 days of your previous purchase. Missing that window pushes the cost up, to as much as $699 per month at higher doses.6Eli Lilly. LillyDirect – Zepbound
- The Lilly Cares Foundation provides certain Eli Lilly medications at no cost for up to 12 months to qualifying patients with financial need. Check whether Zepbound is included on the current list.7Eli Lilly. Lilly Cares Foundation Patient Assistance Program
Two eligibility traps are worth knowing. Lilly’s savings card programs are not available to people on government insurance, which includes Medicare, Medicaid, TRICARE, and VA plans.5Eli Lilly – Zepbound. Zepbound Savings And if your BCBS Mississippi plan uses an Alternate Funding Program or specialty drug vendor, that can also disqualify you from manufacturer coupons. Confirm both before counting on the savings card price.
Appealing a Denial
Providers can submit a prescription drug prior authorization appeal through the BCBS Mississippi online provider portal.8BCBS Mississippi. Provider Forms and Policies An appeal has a better chance when the issue is medical necessity criteria for a covered drug. When the denial is because Zepbound is not on the formulary at all, the odds are much worse. The policy itself states that medical policy guidelines do not override a member’s specific benefit plan language, and that a prescription from a doctor does not, by itself, make a drug medically necessary under the plan.1BCBS Mississippi. GLP-1 Agonist Medical Policy
If you’re on the state and school employee plan, prior authorization requests and formulary questions go through CVS Caremark rather than BCBS Mississippi directly, with submission handled through the Mississippi Department of Finance and Administration website.
If You’re on Mississippi Medicaid
Mississippi Medicaid is a separate payer and operates on different rules than BCBS Mississippi. The state Medicaid program began covering GLP-1 medications for obesity in 2023, but the preferred anti-obesity agents are Saxenda and Wegovy. Zepbound is not listed among the covered anti-obesity medications in the current prior authorization criteria.9Mississippi Division of Medicaid. Anti-Obesity Select Agents PA Criteria Medicaid enrollees also cannot use the Lilly savings card.
