Blue Cross Blue Shield of Michigan does cover Ozempic when it is prescribed to treat Type 2 diabetes, and it does not cover Ozempic or other GLP-1 drugs prescribed solely for weight loss. That single distinction decides most coverage questions across BCBSM’s PPO plans and its Blue Care Network HMO.1BCBSM MiBluDaily. Why We Are Changing Coverage of GLP-1 Drugs for Weight Loss
Ozempic for Type 2 Diabetes
BCBSM and Blue Care Network cover Ozempic, Mounjaro, Rybelsus, and Victoza for Type 2 diabetes treatment. There is no published difference in formulary placement or prior authorization criteria between the PPO and HMO plans on these drugs.2Michigan Collaborative for Type 2 Diabetes. BCBSM New Prior Authorization Criteria for GLP-1 RAs
Prior authorization has been required since August 2023. The prescriber must confirm a Type 2 diabetes diagnosis, and the member must either have tried at least one generic or preferred diabetes medication (not counting metformin or another GLP-1) or have a recent pharmacy or medical claim showing a Type 2 diabetes diagnosis on file. No lab results or detailed records are required for the submission itself, and BCBSM has said approvals typically come back the same day.2Michigan Collaborative for Type 2 Diabetes. BCBSM New Prior Authorization Criteria for GLP-1 RAs
Documentation standards can vary by plan. One BCBSM plan document reviewed in a state appeal specifically required medical records showing a hemoglobin A1c level of at least 6.5% to confirm the diagnosis, so check your own plan’s criteria before assuming an attestation alone will suffice.3Michigan Department of Insurance and Financial Services. Weight Loss Is No Longer Covered
Starting January 1, 2025, BCBSM eliminated coverage for GLP-1 drugs prescribed for weight loss in its fully insured large group commercial plans. The specific drugs named in the exclusion are Wegovy, Zepbound, and Saxenda.4Fierce Healthcare. Blue Cross Blue Shield of Michigan Pulling Back GLP-1 Coverage The exclusion applies regardless of BMI, sleep apnea, or other obesity-related conditions. A Michigan Department of Insurance and Financial Services ruling in May 2025 upheld a BCBSM denial on exactly that ground, confirming that the plan “does not cover GLP-1 medications used to treat obesity… including for those with, and those without, obstructive sleep apnea.”5Michigan Department of Insurance and Financial Services. BCBSM External Review, File No. 235583
BCBSM’s formulary also broadly excludes compounded products, so compounded semaglutide (the active ingredient in Ozempic and Wegovy) is not an in-network workaround.6Blue Cross Blue Shield of Michigan. Clinical Drug List Formulary
How Your Plan Type Changes the Answer
The weight loss exclusion applies to fully insured large group commercial plans. Other plan categories work differently.
Self-funded employer groups. Employers that self-fund their health benefits and use BCBSM only as the administrator may continue covering GLP-1 drugs for weight loss under their own prior authorization rules. The call belongs to the employer, not BCBSM.7BCBSM Provider Information. Changes to Weight Loss Drugs – Commercial Alert
MESSA (Michigan school employees). MESSA stopped covering Wegovy, Zepbound, and Saxenda for weight loss as of January 1, 2025, according to a 2025 state regulatory ruling. MESSA does cover Ozempic and Mounjaro for diabetes treatment.8Michigan Department of Insurance and Financial Services. BCBSM External Review, File No. 2343179MESSA. Weight Loss
Medicare Advantage. Medicare does not cover GLP-1 drugs for weight loss, and BCBSM Medicare Advantage plans follow that federal rule. For Type 2 diabetes, most BCBSM Medicare Advantage plans list Ozempic as a preferred brand.10Michigan Collaborative for Type 2 Diabetes. MCT2D Coverage Guide
Michigan Medicaid. The state’s 2026 budget legislation restricted Medicaid coverage of GLP-1 weight loss drugs to members who are morbidly obese, have failed other interventions, and need the drug to avoid bariatric surgery. Diabetes coverage remains in place.11Michigan Public. Michigan Cuts Off Weight Loss Drugs for Most Medicaid Patients, Saving $240 Million
Individual and family (ACA marketplace) plans. BCBSM publishes separate HMO and PPO drug lists for 2026. If you have an individual plan, check the drug list on the BCBSM website or call the number on the back of your member ID card before filling an Ozempic prescription.12Blue Cross Blue Shield of Michigan. Appealing a Denial
If BCBSM denies Ozempic, two levels of appeal are available.
The internal appeal is filed with BCBSM within 180 days of the denial. The appeal form can be mailed or faxed to BCBSM in Detroit. BCBSM must respond within 30 days for prior authorization denials and within 60 days for post-service claims.13Blue Cross Blue Shield of Michigan. BCBSM Member Appeal Form
If the internal appeal fails, you can request an external review through the Michigan Department of Insurance and Financial Services within four months of the denial. An independent review organization then issues a decision within 72 hours, or within 24 hours in urgent cases where health is at stake.14Blue Cross Blue Shield of Michigan. External Drug Review
External reviews have gone both ways. In an April 2025 case, the DIFS director reversed a Wegovy denial after finding BCBSM’s written documents still listed the drug as a nonpreferred benefit and had not been updated to reflect the exclusion.15Michigan Department of Insurance and Financial Services. BCBSM External Review, File No. 234270 In an October 2025 case, an independent reviewer found that BCBSM’s requirement of a BMI of 35 or higher for Wegovy was “not consistent with the standard of care,” and coverage was ordered.16Michigan Department of Insurance and Financial Services. BCBSM External Review, File No. 239983 In other cases, DIFS upheld denials where the plan language clearly excluded GLP-1 drugs for weight loss.5Michigan Department of Insurance and Financial Services. Paying Out of Pocket
If your plan does not cover Ozempic, or you are bridging a coverage gap, Novo Nordisk offers direct pricing through its NovoCare program. New patients can get the injectable pen at $199 per month for the first two months; existing patients pay $349 per month for the standard doses, or $499 for the 2 mg dose. An oral tablet formulation starts at $149 per month for new patients.17Ozempic. Save on Ozempic
If you do have commercial coverage for Ozempic, a savings card may reduce the copay to as little as $25 per month for up to 48 months. The NovoCare Patient Assistance Program also provides free medication to qualifying low-income patients. Medicare and Medicaid beneficiaries are not eligible for either offer.17Ozempic.
