Most BlueCross BlueShield of South Carolina plans do not cover Wegovy. The insurer classifies it as an anti-obesity agent, and the majority of its commercial and ACA Marketplace plans exclude weight-loss medications outright. That exclusion holds even when a doctor prescribes Wegovy for its FDA-approved cardiovascular use, because BCBS SC bases coverage on how a drug is classified, not on the diagnosis written on the prescription.1BlueCross BlueShield of South Carolina. Reminder: GLP-1 Utilization Management
Whether you can get Wegovy paid for comes down to the plan your employer bought or the plan you picked on the Marketplace. A narrow set of employer groups have added weight-loss drugs as an optional benefit; everyone else is looking at an appeal, a manufacturer savings program, or paying cash.
Check Your Specific Plan First
Coverage varies by plan design, so start by confirming what your plan actually excludes. BCBS SC tells members to contact their HR department or review their benefit documents to see whether weight-loss medications are included or excluded.2BlueCross BlueShield of South Carolina. Why the Cardiovascular Approval Doesn’t Change the Answer
In March 2024, the FDA expanded Wegovy’s approved uses to include reducing the risk of cardiovascular death, heart attack, and stroke in adults with established cardiovascular disease who are overweight or obese.5U.S. Food and Drug Administration. FDA Approves First Treatment to Reduce Risk of Serious Heart Problems Specifically in Adults with Obesity or Overweight That approval has not changed how BCBS SC handles the drug. The insurer has stated that coverage rests on “drug classification, not specific indication,” so Wegovy remains subject to any weight-loss exclusion in the member’s plan regardless of whether it is prescribed for obesity, cardiovascular risk reduction, or obstructive sleep apnea.4BlueCross BlueShield of South Carolina. The One Commercial Exception: Employer-Elected Coverage
There is a single pathway for commercial coverage. Self-funded employer groups that use the insurer’s Lowest Net Cost (LNC) formulary can add weight-loss drug coverage as an optional benefit category. When a group elects it, all drugs in the weight-loss category become covered for its employees, including Wegovy, Zepbound, Saxenda, Qsymia, and Contrave.6BlueCross BlueShield of South Carolina. Optional Drug Coverage List
This is the employer’s decision, not the employee’s, and it is all or nothing. If your employer declined, no appeal or doctor’s letter will get Wegovy covered for weight loss under that plan. Even when the benefit is elected, prior authorization still applies.4BlueCross BlueShield of South Carolina. BlueNews for Providers – May 2025
If You Have BlueChoice HealthPlan
BlueChoice HealthPlan, an affiliate within the BCBS SC family, takes a stricter position. It states that weight-loss medications are typically not covered, and that it will not cover GLP-1 medications for weight loss even when a member’s plan includes a weight-loss drug benefit, because the plan treats GLP-1s as not FDA-approved for that purpose. Any GLP-1 coverage requires prior authorization documentation showing a Type 2 diabetes diagnosis.7BlueChoice HealthPlan. Weight Loss Coverage
BlueChoice also requires that patients have tried and failed metformin or a metformin combination product for at least 90 days before a GLP-1 will be authorized.8BlueChoice HealthPlan. Glucagon-Like Peptide-1 Agents Utilization Management Update
If You’re on the State Employee Plan
The South Carolina State Health Plan, which covers more than 540,000 public employees, spouses, and dependents, does not cover anti-obesity medications. GLP-1 drugs are covered only for Type 2 diabetes. Even that limited coverage has been expensive; the plan spent $106.4 million (after rebates) on GLP-1 prescriptions for diabetes between January and September 2024.9SC Daily Gazette. States Consider High Costs, Possible Savings of Covering Weight Loss Drugs for Their Workers
Rather than expanding coverage, the Public Employee Benefit Authority has tightened it. In 2024, the board approved 30-day fill limits and a new prior authorization process for GLP-1 prescriptions, estimated to save $18 million annually.10South Carolina Public Employee Benefit Authority. If You’re on South Carolina Medicaid
South Carolina Medicaid briefly covered GLP-1 weight-loss drugs starting in November 2024.11KFF Health News. If You Have Medicare: The GLP-1 Bridge Program
Federal law prohibits Medicare Part D from covering drugs prescribed for weight loss. CMS has created a time-limited workaround: the Medicare GLP-1 Bridge Program, running from July 1, 2026 through December 31, 2026, which provides eligible Medicare beneficiaries access to Wegovy and Zepbound for weight loss outside of the standard Part D benefit.14Centers for Medicare & Medicaid Services. Medicare GLP-1 Bridge
The program is open to anyone enrolled in a Medicare Advantage plan with drug coverage or a standalone Part D plan. Individual plan sponsors do not need to opt in, because CMS runs the program centrally. Beneficiaries pay a fixed $50 monthly copay, which does not count toward Part D out-of-pocket limits.15Medicare Rights Center. GLP-1 Weight Loss Drug Demonstration Begins July 2026 For BCBS SC Medicare Advantage members, this is a pathway that did not previously exist.
How to Appeal a Denial
If Wegovy is denied, BCBS SC allows members to file a written appeal within 180 days of the date on the Explanation of Benefits. The appeal must include the member’s name, ID number, patient name, claim number, and the name and relationship of the person filing. A Designation of Authorized Representative form is required if someone other than the covered member files.3BlueCross BlueShield of South Carolina. Appeal a Denied Claim
An appeal has better odds when the denial is based on medical necessity rather than a flat plan exclusion. A plan exclusion is a contract term, and appeals rarely overturn it. For medical necessity denials, a letter from the prescribing physician documenting weight history, prior failed lifestyle attempts, and obesity-related conditions such as cardiovascular disease or diabetes can support the request.16Medical News Today. Paying Without Coverage
Novo Nordisk runs several programs that lower Wegovy’s out-of-pocket cost:
- Commercially insured patients whose plan covers Wegovy may pay as little as $25 per month through the Wegovy Savings Offer, with maximum savings of $100 per month. Enrollment is available by texting “SAVE” to 83757.18NovoCare. Wegovy Savings Offer
- Self-pay patients can buy Wegovy directly through NovoCare Pharmacy at $149 per month for the 1.5 mg and 4 mg doses through August 31, 2026, after which the 4 mg dose rises to $199 per month.17Wegovy. What to Pay for Wegovy

Medicare and Medicaid beneficiaries are generally not eligible for the commercial savings offer. People on ACA Marketplace plans, FEHB plans, and state employee plans are not considered government beneficiaries for this purpose and can participate.18NovoCare. Wegovy Savings Offer
