Whether Blue Cross Blue Shield of Texas covers a GLP-1 medication depends on why it is being prescribed and which plan you carry. For type 2 diabetes, most BCBSTX plans cover GLP-1 drugs once prior authorization is approved. For weight loss, coverage is optional on employer-sponsored plans and is included only if the employer has specifically elected it, and marketplace plans often exclude weight-loss drugs entirely. If your plan does not include the benefit, you can still fill a prescription, but you pay the full cost.
Weight Loss Coverage Is Optional and Set by Your Employer
BCBSTX states on its own site that “most plans don’t cover GLP-1s for weight loss.”1BCBSTX. GLP-1s for Weight Loss For self-funded employer groups, GLP-1 coverage for weight management is an optional add-on. Employers that elect it are subject to utilization management rules, and BCBSTX pairs the drug benefit with metabolic health programs.2BCBSTX. New GLP-1 Coverage 2026 If the employer has not opted in, the plan will not pay for the drug when it is prescribed for weight loss, no matter how clinically appropriate.
Even when the benefit is included, prior approval from the health plan is normally required before treatment starts. To confirm what your specific plan does, sign in at MyPrime.com or call the customer service number on your ID card.1BCBSTX. GLP-1s for Weight Loss
Diabetes Coverage Is Broader, but Prior Authorization Applies
When a GLP-1 is prescribed for type 2 diabetes, BCBSTX plans generally cover it, but the insurer requires prior authorization to confirm the diagnosis. Since June 2023, BCBSTX has used integrated medical and pharmacy claims data to automate some approvals. If you already have a documented type 2 diabetes diagnosis and a claims history for diabetes medications, the prior authorization requirement may be satisfied automatically.3BCBSTX. GLP-1 Agonist Medications
If the diagnosis is not already in the claims system, your prescribing physician must submit chart notes or lab results. Accepted lab documentation includes an A1C of 6.5% or higher, a fasting plasma glucose of 126 mg/dL or higher, a two-hour oral glucose tolerance test result of 200 mg/dL or higher, or a random plasma glucose of 200 mg/dL or higher with symptoms of hyperglycemia.4MyPrime. GLP-1 Agonists Prior Authorization Form Prescriber attestation alone is no longer accepted; actual chart notes have been required since November 2023.3BCBSTX. GLP-1 Agonist Medications
Some older or less-preferred GLP-1 agents also carry step therapy. Approval for Byetta, exenatide, liraglutide, or Victoza requires documentation that the patient had an inadequate response, intolerance, or contraindication to semaglutide (Ozempic or Rybelsus), dulaglutide (Trulicity), or tirzepatide (Mounjaro).4MyPrime. Which GLP-1 Drugs Are on the BCBSTX Drug Lists
As of May 2026, BCBSTX’s drug lists for self-funded groups include several GLP-1 medications, but availability of the weight-loss indication still depends on whether the employer has elected that coverage.
- Wegovy (semaglutide injection), including Wegovy HD, is covered for groups that have elected GLP-1 coverage for weight management, with utilization management applied.
- Zepbound (tirzepatide auto-injection) is covered for groups that elect GLP-1 coverage.
- Zepbound KwikPen is excluded from BCBSTX coverage entirely and is available only through Eli Lilly’s direct-to-consumer channel.
- Ozempic tablets in 1.5 mg, 4 mg, and 9 mg strengths have been available since May 4, 2026, for type 2 diabetes, replacing the older Rybelsus formulation. Rybelsus is expected to be phased out by the end of 2026.
- Foundayo (orforglipron) tablets, the newly FDA-approved oral GLP-1 for weight loss, are under clinical review for drug list placement. Members whose groups already have weight management coverage can submit coverage exception requests in the meantime.2BCBSTX. New GLP-1 Coverage 2026
BCBSTX marketplace (ACA) plans use a closed formulary, meaning only listed drugs are covered, and these formularies may exclude weight loss drugs depending on plan design. Supply Limits You May Run Into Even When Covered
BCBSTX has rolled out optional pharmacy benefit programs that cap fill sizes. The “GLP-1 New to Therapy” program, available since April 2024 for large and self-funded groups, limits initial prescription fills to a 30-day supply for members who have not filled a GLP-1 in the previous 120 days. After the initial fill period, members may become eligible for up to a 90-day supply depending on the plan, and the 30-day limit can reapply when a member switches drugs or doses.6BCBSTX. Separate “GLP-1 30-Day Max” and “Anti-Obesity 30-Day Max” programs took effect in September 2024 and cap all fills at a 30-day supply for both oral and injectable medications. Affected drugs can include Mounjaro, Ozempic, Orlistat, Phentermine HCL, Qsymia, Saxenda, Trulicity, Wegovy, and Zepbound, with the specific list depending on employer selection.7BCBSTX. What You Pay if the Plan Does Not Cover It
If your plan excludes GLP-1s for weight loss, the full cost falls on you. Wegovy’s list price is roughly $1,349 for a 28-day supply of the injectable, though Novo Nordisk offers a self-pay introductory price of $199 per month for the lowest starting doses. An oral version of Wegovy launched in early 2026 with starting doses priced at $149 to $299 per month.8FindHonestCare. Wegovy Cost
Zepbound’s list price for a 28-day supply ranges from $499 to $1,086 depending on the dose, and without insurance or discounts the annual cost can exceed $14,000. Eli Lilly’s LillyDirect platform offers lower prices, starting at $299 per month for the 2.5 mg vial.9GoodRx. Zepbound Cost Foundayo is priced between $149 and $349 per month for self-pay patients, depending on the dose.10CNBC. Eli Lilly GLP-1 Pill Foundayo Approved for Obesity
Members with commercial insurance coverage who use manufacturer savings cards can pay as little as $25 per month for Zepbound or Wegovy.8FindHonestCare. Wegovy Cost Those savings cards are not available to anyone on a government-sponsored plan such as Medicare or Medicaid.
If You Have HealthSelect or Medicare
Texas state employees on HealthSelect of Texas should know that the plan’s prescription drug program, administered by Express Scripts, excludes drugs prescribed for the treatment of obesity.11Express Scripts. Medicare Part D has historically been prohibited from covering drugs prescribed for weight loss. The “Medicare GLP-1 Bridge” demonstration, running from July 1, 2026, through the end of 2027, lets eligible Medicare beneficiaries access Wegovy or Zepbound for weight reduction at a fixed $50 copayment per month.13CMS.
