Does BCBSNC Cover Weight Loss Drugs? Exceptions and Appeals

Blue Cross and Blue Shield of North Carolina does not cover weight loss drugs as a standard benefit, so medications like Wegovy, Zepbound, Saxenda, Contrave, Qsymia, and phentermine are excluded across its plans.1Blue Cross NC. Prior Authorization for Prescription Drugs A narrow exception exists for members whose employers self-fund their health benefits and have opted into a separate GLP-1 program, and a few other pathways exist outside the insurer’s standard pharmacy benefit.

What Blue Cross NC Excludes

The exclusion is broad. It covers newer GLP-1 receptor agonists, older appetite suppressants, and combination therapies. The medications listed as not covered as a standard benefit include:

  • GLP-1 drugs prescribed for weight loss: Wegovy (semaglutide), Zepbound (tirzepatide), and Saxenda (liraglutide)
  • Combination drugs: Contrave (naltrexone/bupropion) and Qsymia (phentermine/topiramate)
  • Appetite suppressants: Adipex-P, phentermine, phendimetrazine, diethylpropion, benzphetamine, Evekeo (when used for weight loss), and Lomaira
  • Xenical (orlistat)

The exclusion appears on Blue Cross NC’s provider-facing prior authorization page and is reflected in formulary documents for marketplace plans, which note that coverage of weight loss drugs may be subject to a benefit exclusion depending on the member’s plan.2MyPrime. Same Drug, Different Indication: The Diabetes Line

The exclusion targets weight loss specifically. GLP-1 medications prescribed for type 2 diabetes, such as Ozempic (semaglutide) and Mounjaro (tirzepatide), are not on the excluded list. Mounjaro appears on the 2025 Essential Drug List as a Tier 2 medication under “Antidiabetics — Hormones,” subject to prior authorization and quantity limits for new starts.3Formulary Navigator. Mounjaro Drug Search Results Several of these drugs are FDA-approved for both diabetes and weight management, but Blue Cross NC treats the two indications very differently for coverage.

The Self-Funded Employer Plan Exception

The one meaningful path to coverage runs through Administrative Services Only (ASO) plans, where an employer self-funds the benefits and Blue Cross NC administers claims. Those employers can design their own coverage, and some have opted into a GLP-1 weight management program Blue Cross NC offers in partnership with Vida Health.4Blue Cross NC. New and Expanded Member Programs Effective January 1, 2025

The program launched January 1, 2025, and is available only to ASO plans. To qualify, members must be enrolled in a Blue Cross NC pharmacy plan and meet clinical thresholds: a BMI of 30 or higher without weight-related comorbidities, or 27 or higher with them. Registered dietitians lead the program, which is built around cognitive behavioral therapy principles and includes support for medication adherence, nutrition, physical activity, and emotional regulation.4Blue Cross NC. New and Expanded Member Programs Effective January 1, 2025

Whether the drug itself gets paid for, and how, varies by employer. Some have moved GLP-1 coverage off the Blue Cross NC pharmacy plan entirely and instead subsidize the drugs through a separate channel, with employees required to be enrolled in Vida Health and prescribed by a Vida provider.5Currituck County NC. Bariatric Surgery Is Covered When Medically Necessary

Blue Cross NC excludes the drugs but covers bariatric surgery when medically necessary. Patients generally must have a BMI of 40 or higher, or a BMI of 35 or higher with at least one significant obesity-related comorbidity such as type 2 diabetes, hypertension, or sleep apnea. The surgery has to be part of a comprehensive program with pre-surgical evaluation, the procedure, and post-surgical follow-up.7Blue Cross NC. Bariatric Surgery Policy

Covered procedures include Roux-en-Y gastric bypass, sleeve gastrectomy, and adjustable gastric banding, among other surgical approaches. Prior authorization is required. Endoscopic weight loss procedures, including intragastric balloons and endoscopic sleeve gastroplasty, are considered investigational and are not covered.7Blue Cross NC. Bariatric Surgery Policy

Can You Request an Exception or Appeal a Denial?

Blue Cross NC has a non-formulary exception process, but it is designed for drugs that are simply off the formulary, not drugs that are explicitly excluded. A prescribing provider submits a request through CoverMyMeds, the MHK Provider Portal, SureScripts, or by fax, with clinical documentation showing why the specific medication is medically necessary and why covered alternatives are inadequate. Decisions typically come within three business days, or 24 hours for urgent requests.8Blue Cross NC. Non-Formulary Drugs

For drugs on the weight loss exclusion list, the insurer’s documentation does not spell out an exception pathway. The prior authorization page lists them as not covered, without the criteria or step therapy requirements that would apply if coverage were possible under some conditions.1Blue Cross NC. Prior Authorization for Prescription Drugs

After a denial, members can file an appeal within 180 days of the adverse benefit determination using the Member Appeals Form, submitted by mail or fax to Blue Cross NC’s Member Rights and Appeals department.9Blue Cross NC. Appeals Medicare plan members instead request a coverage determination first, then appeal within 65 calendar days of a denial, with the option to escalate to an Independent Review Entity if the plan upholds its decision.10Blue Cross NC. Part D Coverage Determinations, Exceptions, Appeals, and Grievances An appeal for a drug that is categorically excluded from the plan benefit is unlikely to succeed, because the exclusion is a plan design decision rather than a medical necessity judgment.

Medicare Members and the GLP-1 Bridge

Blue Cross NC’s Medicare Advantage plans (Blue Medicare) face an extra restriction. Federal law has historically excluded drugs used for weight loss from Medicare Part D coverage, meaning anti-obesity medications have not been payable through the Part D benefit structure regardless of what the plan sponsor might want.11Blue Cross NC. Medicare Prescription Drug Coverage Limitations and Exclusions

A temporary workaround now exists. CMS launched the Medicare GLP-1 Bridge, a demonstration program running from July 1, 2026, through December 31, 2026, that provides eligible Medicare beneficiaries access to Wegovy, Zepbound, and Foundayo for $50 per month. The program operates outside the Part D benefit, with providers submitting prior authorization requests to a central processor (Humana) and pharmacies billing through a separate channel.12CMS. Medicare GLP-1 Bridge

To qualify, a beneficiary must have Medicare Part D coverage, must not have type 2 diabetes, moderate-to-severe sleep apnea, or fatty liver disease, and must meet BMI thresholds: 35 or higher, or 30 or higher with conditions like heart failure or chronic kidney disease, or 27 or higher with prediabetes or a history of heart attack or stroke.13Medicare.gov. Medicare GLP-1 Bridge: GLP-1 Drugs for $50 a Month The Bridge is designed as a precursor to the BALANCE Model launching in January 2027, though continued access depends on being in a plan that opts into that model.12CMS. How to Confirm Your Own Plan

Coverage depends on the type of plan you have and, if your coverage comes through an employer, on how that employer has designed its benefits. Call the customer service number on the back of your member ID card to verify your specific prescription drug benefits. The online drug search tool, available through the Blue Cross NC website’s “Find Care” feature and Prime Therapeutics, lets you look up a specific medication under your plan by entering the Rx letter code from your ID card.1Blue Cross NC. Prior Authorization for Prescription Drugs Ask specifically whether your plan is self-funded (ASO) and whether your employer has opted into the Vida Health GLP-1 weight management program, because that is where the real variation lives.
