Blue Cross Blue Shield of North Carolina does not cover Zepbound as a standard benefit, and the drug appears on the insurer’s list of medications with “limited or no plan coverage.” Most fully insured individual and group plans exclude weight-loss medications as a category, so the question of whether BCBSNC covers Zepbound usually comes down to the specific plan design. Some self-funded employer plans administered by Blue Cross NC do cover it through a dedicated GLP-1 program, and Medicare beneficiaries will have access through a separate federal demonstration starting in mid-2026.
Why Most Plans Exclude It
Zepbound (tirzepatide) was approved by the FDA in November 2023 for chronic weight management in adults with obesity or overweight with at least one weight-related condition, and in December 2024 for moderate-to-severe obstructive sleep apnea in adults with obesity.1Eli Lilly and Company. FDA Approves Zepbound (Tirzepatide) First and Only Prescription
Blue Cross NC’s provider-facing materials categorize Zepbound under medications “not covered as a standard benefit.”2Blue Cross NC. Prior Authorization for Prescription Drugs The member site reinforces the point, noting that “some plans may exclude coverage for certain categories of drugs, such as those for weight loss.”3Blue Cross NC. Prescription Drugs The formulary itself states that “coverage of drugs used for weight loss may be subject to benefit exclusion based on member benefit.”4MyPrime. Blue Cross NC Essential QHP-S 4-Tier Formulary
The word “exclusion” is important here. A non-formulary drug is one that isn’t on the preferred list but can sometimes be approved through an exception request. A benefit exclusion means the plan was built not to cover that category at all. Blue Cross NC’s documentation does not indicate that the standard non-formulary exception process can override a weight-loss exclusion, so a provider letter or appeal that works for other off-formulary drugs will not generally produce coverage for Zepbound on an excluded plan.
The Exception: Self-Funded Employer Plans
The best chance of getting Zepbound covered through Blue Cross NC is a self-funded employer plan, sometimes called Administrative Services Only or ASO. In these arrangements the employer, not Blue Cross NC, decides what benefits to include and bears the claims cost; Blue Cross NC administers the plan.
Effective January 1, 2025, Blue Cross NC began offering a GLP-1 for Weight Management program powered by Vida Health, available only to ASO plans whose employers choose to add it.5Blue Cross NC. New Expanded Member Programs 01-01-2025 Members must meet one of two clinical thresholds:
- BMI of 30 or above without weight-related comorbidities, or
- BMI of 27 or above with weight-related comorbidities
Members must also be enrolled in a Blue Cross NC pharmacy plan and must participate in the Vida program, which is led by registered dietitians and uses cognitive behavioral therapy principles to support medication adherence, nutrition, physical activity, and emotional regulation.5Blue Cross NC. New Expanded Member Programs 01-01-2025 If you already have an active prior authorization for a GLP-1, Vida will honor the remainder of it as long as you enroll in the program.
Whether your employer has added this benefit is something only your plan can confirm. Call the customer service number on your member ID card and ask specifically whether your plan includes GLP-1 coverage for weight management.
If You’re on the NC State Health Plan
North Carolina state employees and retirees are covered under the NC State Health Plan, which operates separately from Blue Cross NC’s commercial plans. The State Health Plan’s board of trustees ended coverage for all obesity-related GLP-1 medications, including Zepbound, effective April 1, 2024.6News & Observer. State Treasurer Brad Briner has pushed to restore the benefit. In October 2025, the State Health Plan reached an agreement with its pharmacy benefit manager, CVS Caremark, that lets the plan negotiate directly with GLP-1 manufacturers.8NC Department of State Treasurer. Successful Negotiation with CVS Caremark Briner requested $100 million from the legislature to restore the benefit for an estimated 14,000 state workers with a BMI of 38 or higher.9WRAL. NC State Health Plan Requests Funding for GLP-1 Coverage As of May 2026, that funding has not been appropriated, and Briner has said he does not expect the upcoming budget to include it.10WRAL. North Carolina Expected Budget Won’t Restore Weight Loss Coverage Coverage could potentially be restored without legislative funding if manufacturer negotiations yield large enough discounts, but no timeline has been set.
Medicare Members and the GLP-1 Bridge Program
Medicare Part D rules generally prohibit coverage of weight-loss medications, and Blue Cross NC’s 2026 Medicare Rx Enhanced formulary does not list Zepbound.11Blue Cross NC. Blue Medicare Rx Enhanced PDP Formulary 2026 A new federal demonstration changes that for some beneficiaries.
The Medicare GLP-1 Bridge Program runs from July 1, 2026, through December 31, 2027, and covers the Zepbound KwikPen formulation at a flat $50 monthly copayment.12Medicare Rights Center. GLP-1 Weight Loss Drug Demonstration Begins July 2026 It is administered centrally by Medicare through Humana as the processor rather than through individual Part D or Medicare Advantage plans.13CMS. Medicare GLP-1 Bridge – Information for Providers Blue Cross NC Medicare members do not need their plan to add Zepbound to its formulary to participate; prior authorization requests go to the central processor, not to Blue Cross NC.
Eligibility requires Medicare prescription drug coverage and one of three clinical tiers:
- BMI of 35 or higher, with no additional condition required
- BMI of 30 or higher, with heart failure with preserved ejection fraction, uncontrolled hypertension despite two medications, or chronic kidney disease at stage 3a or above
- BMI of 27 or higher, with pre-diabetes, a previous heart attack, a previous stroke, or symptomatic peripheral artery disease14CMS. Medicare GLP-1 Bridge
Beneficiaries with type 2 diabetes, obstructive sleep apnea, or certain liver conditions should seek coverage through their regular Part D plan instead, because those indications are already covered separately.13CMS. Medicare GLP-1 Bridge – Information for Providers Only the KwikPen is covered under the Bridge Program; single-dose vials and single-dose pens are excluded.
Can an OSA Diagnosis Get Zepbound Covered?
Because the FDA approved Zepbound for moderate-to-severe obstructive sleep apnea in adults with obesity in December 2024, some members ask whether an OSA diagnosis can route around the weight-loss exclusion. For commercial Blue Cross NC plans, the answer appears to be no. Blue Cross NC’s medical policy for sleep apnea diagnosis and management, last reviewed in March 2026, does not list tirzepatide, Zepbound, or any GLP-1 medication as a covered OSA treatment.15Blue Cross NC. Sleep Apnea: Diagnosis and Medical Management The policy limits covered OSA treatments to weight loss counseling, oral appliances, positive airway pressure therapy, and lifestyle measures. As of mid-2026, Blue Cross NC has not created a separate coverage pathway for Zepbound under the sleep apnea indication.
Paying Without Insurance Coverage
If your plan excludes Zepbound, Eli Lilly’s LillyDirect pharmacy offers tiered self-pay pricing:16Eli Lilly. Zepbound Coverage and Savings
- 2.5 mg: $299 per month
- 5 mg: $399 per month
- 7.5 mg through 15 mg: $449 per month, provided the refill is purchased within 45 days of the previous delivery
Retail pharmacy pricing is higher. Average retail prices for a 28-day supply were approximately $1,291 for single-dose pens or vials and around $600 for KwikPens as of May 2026.17GoodRx. Zepbound Cost
Lilly also offers a savings card for patients with commercial insurance that does cover Zepbound, which can reduce costs to as little as $25 per month.16Eli Lilly. Zepbound Coverage and Savings A separate card exists for commercially insured patients whose plans don’t cover the drug, but it requires paying entirely out of pocket without any insurance reimbursement. Neither card is available to Medicare or Medicaid beneficiaries, and both expire on December 31, 2026. Blue Cross NC’s formulary documents note that weight-loss drugs may be eligible expenses for Health Savings Accounts or Flexible Spending Accounts.
