Does BCBSTX Cover Wegovy for Weight Loss? Plans, Approval, Appeals

Most Blue Cross and Blue Shield of Texas plans do not cover Wegovy for weight loss. BCBSTX states plainly that “most plans don’t cover GLP-1s for weight loss,” and members whose plans lack this benefit pay the full cost themselves if their doctor writes the prescription.1BCBSTX. GLP-1s for Weight Loss Whether your plan is one of the exceptions depends on choices your employer made, so the only reliable answer comes from your own plan documents.

Why Most BCBSTX Plans Leave Wegovy Out

BCBSTX treats weight-loss drug coverage as a “custom benefit option” rather than a standard part of its plans. Employer groups that sponsor BCBSTX coverage decide whether to add it. Employers that skip the option effectively exclude GLP-1 medications like Wegovy, Zepbound, and Saxenda when prescribed for chronic weight management.2BCBSTX Communications. GLP-1 Agonist Medications The drug lists say the same thing, noting that “some plans may exclude coverage for certain agents or drug categories, like those used for… weight loss.”3BCBSTX. Multi-Tier Basic Drug List April 2026

Wegovy does not appear on the April 2026 Performance Drug List or the April 2026 Basic Drug List.4BCBSTX. Performance Drug List April 2026 It is also absent from the April 2025 Balanced Drug List.5BCBSTX. Balanced Drug List April 2025 That absence does not automatically mean the drug is unavailable under every plan, because employers that did opt in run weight-loss drugs through a separate prior authorization program. But Wegovy is not part of the standard formulary most members see.

How To Check Your Specific Plan

BCBSTX gives members three ways to confirm whether Wegovy is covered:

  • Log in to Blue Access for Members and search for Wegovy under the Pharmacy tab to see coverage status under your plan.
  • Use the Drug Finder at MyPrime.com and search for Wegovy. The guest version works without signing in.
  • Call the customer service number on the back of your member ID card to confirm coverage, payment tier, and any prior authorization requirements.

What Approval Looks Like When Your Plan Does Cover It

If your employer added weight-loss drug coverage, BCBSTX manages those medications through a dedicated weight-loss prior authorization program, which runs separately from the prior authorization process for GLP-1s prescribed for type 2 diabetes.2BCBSTX Communications. GLP-1 Agonist Medications You will almost certainly need approval before starting treatment, even with a covered benefit.1BCBSTX. GLP-1s for Weight Loss

Prime Therapeutics, the pharmacy benefit manager BCBSTX uses for commercial plans, sets the clinical criteria. For adults 17 and older, initial approval generally requires:

  • A BMI of at least 30, or at least 27 with a weight-related condition such as hypertension, type 2 diabetes, or obstructive sleep apnea. Lower thresholds of 25 or 23 apply to patients of South Asian, Southeast Asian, or East Asian descent.
  • At least six months of a reduced-calorie diet, increased physical activity, and behavioral changes, during which the patient did not lose at least one pound per week.
  • No history of pancreatitis.
  • No concurrent use of another GLP-1 receptor agonist or targeted weight-loss drug.

Initial approvals last 12 months. To renew, adult patients must have reached or maintained at least a 5% reduction from baseline body weight.7Prime Therapeutics. Texas State Employees Covered by ERS

If you are enrolled in HealthSelect of Texas through the Employees Retirement System, Wegovy is not an option through your prescription benefit. The HealthSelect Prescription Drug Program, administered by Express Scripts rather than Prime Therapeutics, lists “Drugs Prescribed for the Treatment of Obesity” as a flat exclusion in its benefit plan document.9Express Scripts. Federal Employees on an FEP Plan

Federal employees in the Blue Cross Blue Shield Federal Employee Program have a different path. FEP plans do not automatically cover weight-loss GLP-1s, but a member’s prescriber can submit a formulary exception request. If approved for 2026, Wegovy sits at Tier 2 (Preferred) under the FEP Blue Focus plan.10FEP Blue. Pharmacy FAQs FEP’s weight management page says the program covers “many FDA-approved weight-loss medications” for members who meet eligibility criteria and obtain prior approval, though it does not name Wegovy on that page.11FEP Blue. If Your Plan Denies Coverage

When Wegovy is not on your drug list, you or your doctor can ask for a drug list exception. Call the number on your ID card or submit a Prescription Drug Coverage Exception form through MyPrime.com. BCBSTX responds to a standard request within 72 hours and an expedited request within 24 hours.6BCBSTX. Drug Lists

If the exception is denied, you can file a formal appeal. For commercial plans, the internal pharmacy appeal must be filed within 60 days of the denial notice. You can submit it by phone (1-888-657-6061), fax (1-855-212-8110), or mail to Prime Therapeutics’ appeals department. If a delay would seriously harm your health, you or your doctor can request an emergency appeal, which requires a decision within 72 hours. To keep receiving the medication while the appeal is pending, you must request that within 10 days of the denial.12BCBSTX. Appeals and Grievances

If the internal appeal also fails, you can request an external medical review by an independent review organization or a state fair hearing through the Texas Health and Human Services Commission. Either must be requested within 120 days of the internal appeal decision.12BCBSTX. Appeals and Grievances
