Does BCBSTX Cover Weight Loss Drugs? Employer Opt-In, GLP-1s, and Costs

Most Blue Cross and Blue Shield of Texas plans do not cover weight loss drugs. Coverage for GLP-1 medications such as Wegovy, Zepbound, and Saxenda exists only when a plan sponsor, usually an employer, has specifically added a weight management benefit. BCBSTX states on its member FAQ that “most plans don’t cover GLP-1s for weight loss” and that members with a weight loss prescription typically pay the full cost out of pocket.1BCBSTX. Why Coverage Depends on Your Specific Plan

BCBSTX does not apply one blanket rule to weight loss medications. The standard drug lists note that “some plans may exclude coverage for certain agents or drug categories, like those used for erectile dysfunction or weight loss.”2BCBSTX. Multi-Tier Basic Drug List Whether a particular member has coverage comes down to the plan the employer or government entity bought.

Three patterns dominate. Large self-funded employers can choose to add weight loss drug coverage as an optional benefit, and many do not. Certain Texas public-employee plans exclude obesity drugs outright. Individual marketplace plans operate from closed formularies that often leave weight loss categories off entirely. A drug that appears on one BCBSTX drug list is not automatically covered under another.

Employer Plans and the Opt-In Benefit

For Administrative Services Only (ASO) employer groups, BCBSTX treats weight loss medication as a “custom benefit option” that employers elect. Groups that add it get a dedicated prior authorization program for chronic weight management, managed separately from GLP-1 coverage for type 2 diabetes.3BCBSTX Communications. Employers that opt in may also layer on supply-management programs through BCBSTX’s pharmacy benefit manager, Prime Therapeutics. Options launched in September 2024 cap initial fills at a 30-day supply to reduce waste and cost, covering drugs that can include Wegovy, Zepbound, Saxenda, Ozempic, Mounjaro, Trulicity, Qsymia, Phentermine, and Orlistat.4BCBSTX. If your employer has not elected the weight management benefit, the drugs are not covered even if your coworkers at another company with the same insurer have access to them. Check your own plan documents before assuming either way.

Prior Authorization Criteria

Even when a plan covers weight loss drugs, prior authorization is almost always required.1BCBSTX. GLP-1s for Weight Loss The clinical criteria Prime Therapeutics applies for BCBSTX’s weight management program (effective April 2026) require:

  • A BMI of 30 kg/m² or higher for patients with obesity, or 27 kg/m² or higher for patients who are overweight and have at least one weight-related condition such as type 2 diabetes, high blood pressure, high cholesterol, cardiovascular disease, or obstructive sleep apnea. Lower thresholds (BMI 23 kg/m² or higher) apply for South Asian, Southeast Asian, and East Asian populations.6Prime Therapeutics. Weight Management Prior Authorization Program Summary

State Employees and Teacher Retirees Are Excluded

Two large Texas public-employee plans administered through BCBSTX networks exclude weight loss drugs outright. HealthSelect of Texas, which covers state employees and retirees, lists “drugs prescribed for the treatment of obesity” as a plan exclusion.7Express Scripts. HealthSelect of Texas Prescription Drug Program

The Teacher Retirement System of Texas (TRS) health plans do not cover drugs with an FDA indication for weight loss, weight reduction, or dietary control, naming Saxenda and Wegovy as examples. The exclusion applies even when a participant has medical conditions that could benefit from weight loss and even when a physician has written the prescription. TRS does cover bariatric surgery for members on its Primary and Primary+ plans who meet qualification criteria, and it offers obesity counseling, nutrition counseling, one-on-one health coaching, and discounted gym memberships at no extra cost.8Teacher Retirement System of Texas.
