Does Blue Cross Blue Shield of Michigan Cover Wegovy?

Blue Cross Blue Shield of Michigan does not cover Wegovy when it is prescribed for weight loss. The exclusion took effect January 1, 2025 for fully insured large group commercial members and remains in place through 2026, though coverage depends on your specific plan type and on the reason the drug is prescribed.¹ The June 2026 BCBSM Clinical Drug List continues to list Wegovy, along with Saxenda and Zepbound, as a nonformulary exclusion.²

Which BCBSM Plans Exclude Wegovy

Not every BCBSM member is affected the same way. Where you land depends on how your plan is structured.

Fully insured large group commercial plans exclude Wegovy, Saxenda, and Zepbound.³
Blue Care Network HMO plans follow the same policy as BCBSM.⁴
Self-funded group plans vary. The employer, not BCBSM, decides. Some have adopted the exclusion; others still cover Wegovy under existing prior authorization rules.⁵
MESSA (Michigan Education Special Services Association) members were initially exempt, but as of October 2025 MESSA also no longer covers Wegovy for weight loss or for other diagnoses such as sleep apnea. Coverage is limited to diabetes treatment.⁶
Michigan state employee plans (SHP PPO and State HDHP) stopped covering GLP-1 drugs for weight management or sleep apnea on January 1, 2026, while continuing coverage for diabetes and cardiovascular diagnoses.⁷

BCBSM’s formulary documents do not specifically address individual or small group plan coverage of these drugs. To confirm your situation, call the number on the back of your member ID card or log in at bcbsm.com.²

Diabetes vs. Weight Loss

The exclusion applies only to GLP-1 drugs used for weight loss. BCBSM and Blue Care Network continue to cover GLP-1 medications prescribed for Type 2 diabetes, including Ozempic, Mounjaro, Rybelsus, and Victoza.⁴

Wegovy itself is a different story. The 2024 provider alert explicitly stated that Saxenda, Wegovy, and Zepbound “aren’t covered for members with Type 2 diabetes” under the weight-loss drug policy.⁵ A diabetes diagnosis does not open a path to Wegovy specifically; it opens a path to the GLP-1 drugs that remain on the formulary for diabetes care.

What If You Have Michigan Medicaid

Medicaid rules are set at the state level and work separately from BCBSM’s commercial exclusion. Effective January 1, 2026, Michigan Medicaid limits GLP-1 weight loss coverage to beneficiaries with a BMI of 40 or greater. A physician must document that other weight-loss medications such as phentermine or Qsymia have failed and attest that the GLP-1 is necessary to avoid bariatric surgery.⁸ Wegovy may also be authorized for Medicaid beneficiaries with established cardiovascular disease or severe liver disease.⁹ Coverage for Type 2 diabetes continues.

What Wegovy Costs Without Coverage

The list price for Wegovy is roughly $1,350 for a 28-day supply. Novo Nordisk, the manufacturer, offers reduced self-pay pricing through its NovoCare program for people who are uninsured or whose plans do not cover the drug.

Novo Nordisk’s copay savings card, which brings the monthly cost down to as little as $25, only works if the underlying commercial plan covers Wegovy. If your BCBSM plan excludes the drug, you pay the self-pay price, and that spending does not count toward your insurance deductible or out-of-pocket maximum.¹⁰ Patients covered by Medicaid or Medicare are not eligible for the manufacturer’s savings programs.¹²

Can You Appeal a Wegovy Denial

You can, but so far appeals have not succeeded. Multiple BCBSM members have taken Wegovy denials to the Michigan Department of Insurance and Financial Services, and the state has consistently upheld the insurer.

In a March 2025 case, a member with a BMI of 38.4 and coronary stenosis sought Wegovy for obesity and cardiovascular risk reduction. The independent review organization acknowledged that BCBSM’s blanket exclusion of Wegovy for obesity was “inconsistent with the standard of care” given the drug’s FDA approval, but concluded that the member did not meet the specific clinical trial criteria for Wegovy’s cardiovascular indication. DIFS upheld the denial.¹³

A June 2025 DIFS decision again sided with BCBSM, holding that GLP-1 weight loss drugs are plan exclusions and that the insurer is “not contractually obligated to pay for services or drugs excluded from the plan,” regardless of whether a physician recommends the medication or it has worked for the patient.¹⁴ If DIFS rules against you, you have 60 days to seek judicial review in the appropriate circuit court.

How to Confirm Your Coverage and What to Do Next

Start by confirming where your plan actually falls. Call the customer service number on the back of your BCBSM or BCN member ID card, or log into your account at bcbsm.com and check your pharmacy benefits.² If you work for a self-funded employer, confirmation matters most of all, because your employer may have chosen a different path than the standard BCBSM policy.

If your plan does not cover Wegovy, the “Check My Benefits” tool at novocare.com will walk you through manufacturer pricing. Novo Nordisk also provides a sample coverage request letter that a provider can submit to your employer’s HR department asking that the plan add Wegovy.¹¹ If you want to formally challenge a denial, the route is BCBSM’s internal appeal first, then external review through DIFS under MCL 550.1915, with the understanding that the state has upheld the exclusion every time so far.¹⁴
