Does Blue Cross Blue Shield of Tennessee Cover Wegovy?

Whether Blue Cross Blue Shield of Tennessee covers Wegovy depends entirely on which BCBST plan you carry. Some plans include weight-loss medications, others exclude them outright, and Wegovy does not appear on the insurer’s standard 2026 commercial formularies. The practical answer for most members is to confirm coverage against their specific benefit document before expecting the pharmacy to fill it.

Why the Answer Depends on Your Plan

BCBST’s own member materials note that “some plans may exclude coverage for certain categories of drugs, such as those for weight loss.”¹ Weight-loss drug coverage is decided plan by plan, so two people holding BCBST cards can get opposite answers at the counter.

Wegovy also does not appear on BCBST’s 2026 Essential Formulary (effective June 2026) or the 2026 Preferred Formulary used by other commercial plan types.²³ The Essential Formulary does include an “Anti-Obesity/Anorexiants” category, but the drugs listed under it are stimulants and ADHD agents rather than GLP-1 medications. Large employer groups sometimes negotiate custom formularies that go beyond the standard lists, so the gap is not absolute. For members on individual marketplace or standard small-group plans, however, the absence strongly suggests Wegovy is not covered without an exception.

Coverage by BCBST Plan Type

Commercial Employer and Marketplace (PPO) Plans

Third-party coverage databases show that where BCBST commercial and Health Insurance Exchange PPO plans do cover Wegovy, there is no default prior authorization, step therapy, or quantity limit at the plan level.⁴ The catch is upstream of that: an employer that excluded weight-loss drugs when designing the benefit will still trigger a denial, no matter what the plan rules otherwise permit.

BlueCare Tennessee (Medicaid)

TennCare expanded coverage for obesity medications to beneficiaries 21 and older effective August 1, 2025. Before that date, only patients under 21 were eligible.⁵ For members in BCBST’s BlueCare Tennessee managed Medicaid plan, Wegovy coverage requires prior authorization and is subject to a quantity limit.⁴ Prescribers can find the anti-obesity PA forms through the CVS Caremark portal that handles BCBST pharmacy management.⁶

BlueCare Plus (Medicare-Medicaid Dual-Eligible)

BlueCare Plus Tennessee is restrictive on this drug class. A December 2025 provider bulletin states that GLP-1 medications are covered only to treat Type 2 Diabetes and are not covered when prescribed solely for weight loss.⁷ Wegovy, which is FDA-approved for weight management rather than diabetes, does not appear in the 2026 BlueCare Plus formulary. Members in these dual-eligible plans who need a weight-loss medication may need to look at the federal Medicare GLP-1 Bridge program or contact TennCare’s separate pharmacy provider, Optum Rx, which handles certain drugs not on the BlueCare Plus drug list.⁸

State of Tennessee Employee Plans

State employees and retirees have a pharmacy benefit administered by CVS Caremark, not BCBST directly. These plans cover weight-loss medications, but as of January 1, 2026, members pay 25% coinsurance for them.⁹ The state cited rising medication costs and utilization as the reason for the change.¹⁰ Weight-loss prescriptions are limited to a 30-day supply. Given Wegovy’s list price, 25% coinsurance can translate to a substantial monthly bill, so confirm formulary status and any PA requirement with Caremark before filling.

Medicare Members and the GLP-1 Bridge

Medicare Part D has traditionally been barred from covering drugs prescribed for weight loss. On July 1, 2026, CMS launched the Medicare GLP-1 Bridge, a temporary nationwide demonstration that provides access to Wegovy (injection and tablet forms), Zepbound, and Foundayo.¹¹¹² Participants pay a flat $50 per one-month supply, and that payment does not count toward Part D deductibles or out-of-pocket limits.¹³

To qualify, a beneficiary must have Medicare prescription drug coverage, be 18 or older, and meet BMI and health-condition criteria, such as a BMI of 35 or higher, or a lower BMI combined with prediabetes, high blood pressure, or a history of heart attack or stroke.¹³¹¹ The prescriber must submit a prescription and complete a prior authorization certifying the drug is part of a lifestyle program focused on diet and exercise.

How to Check Your Specific Plan

Four ways to confirm what your plan actually covers:

Read the Evidence of Coverage or member handbook, which spells out whether weight-loss medications are included or excluded.²
Ask HR if your plan is employer-sponsored; they can confirm whether the employer elected weight-loss drug coverage.¹
Run Wegovy through the Drug Price Estimating Tool in the BCBST member portal, which shows coverage under your plan code.¹
Call Member Service at the number on the back of your ID card for a drug-specific lookup.²

If Wegovy Is Not Covered: Requesting an Exception

Members or their prescribing physicians can ask BCBST for a formulary exception. For marketplace and individual plans, a standard request receives a response within 72 hours, and an expedited request filed for an urgent health situation gets a decision within 24 hours. The prescribing physician must provide a statement explaining why all covered formulary alternatives are ineffective, less effective, or likely to cause adverse effects. A denied exception can be escalated to an independent external review panel, which must rule within the same timeframes.¹⁴

For commercial and employer plans, physicians can submit requests electronically through CoverMyMeds or SureScripts, or by faxing the Pharmacy Medication Review Request form to 1-888-343-4232.¹⁵ If a non-formulary drug is approved through this process, the member’s cost-sharing counts toward their out-of-pocket maximum.¹⁴

Manufacturer Savings and Self-Pay Options

Whether or not BCBST covers Wegovy, Novo Nordisk offers a savings program that can reduce out-of-pocket costs. Commercially insured patients can pay as little as $25 per month, with the program covering up to $100 per one-month fill, $200 per two-month fill, or $300 per three-month fill. The card works whether insurance covers Wegovy or not. If the primary insurer rejects the claim, the pharmacy submits it to the savings program as the secondary payer.¹⁶

Patients enrolled in Medicare, Medicaid, TRICARE, or VA benefits are not eligible for the savings card.¹⁷ Federal Employees Health Benefits and ACA exchange plans are not treated as government programs for the offer. For patients without any insurance coverage, Novo Nordisk offers self-pay through NovoCare Pharmacy at an introductory $199 per month for the lower doses during the first two fills, rising to $349 per month afterward.¹⁶ You can enroll by texting “SAVE” to 81627 or visiting the NovoCare website.¹⁸
