Does Blue Shield of California Cover Wegovy? Rules by Plan Type

Blue Shield of California covers Wegovy only in narrow circumstances. Since January 1, 2025, the insurer has required prior authorization for Wegovy on its commercial plans and will approve it only when it is medically necessary to treat Class III (morbid) obesity, generally a BMI of 40 or higher. Members who don’t meet that threshold pay the full price. The answer shifts further depending on whether you’re on an employer plan, an individual or Covered California plan, Medi-Cal, or Medicare Advantage.

The 2025 Prior Authorization Rule

In November 2024, Blue Shield announced that effective January 1, 2025, or on a member’s next plan renewal after that date, Wegovy and 12 other weight loss drugs would no longer be covered without prior authorization showing medical necessity. The rule applies to commercial HMO and PPO plans, including plans purchased directly from Blue Shield and plans purchased through Covered California. Members who already held a valid prior authorization when the policy took effect were allowed to keep filling their prescriptions until that authorization expired.1Blue Shield of California. What You Have to Show to Get Approved

Under the 2025 criteria, a member seeking Wegovy coverage must have a current or previous diagnosis of Class III (morbid) obesity. The member must also be participating in a comprehensive weight loss program that includes a reduced-calorie diet, physical activity, and behavior therapy. That participation is required both before starting Wegovy and while taking it.1Blue Shield of California. How the Answer Changes by Plan Type

Employer-Sponsored Plans

For large group commercial plans, pharmacy benefits run through a set of prescription drug riders that the employer picks. One option, the Enhanced+ Rx rider, includes enhanced coverage for weight loss drugs. Other riders, including Basic Rx, Enhanced Rx, and Premier Rx, do not appear to include that coverage.2Blue Shield of California. Large Group Prescription Drug Riders

Starting January 1, 2026, Blue Shield said coverage of weight loss drugs for large group commercial plans “will vary.” Depending on how the employer configured the plan, the BMI threshold for a new Wegovy prescription may be 40 or higher, 30 or higher, or 27 or higher with certain conditions. Some employers may choose not to cover weight loss drugs at all.3Blue Shield of California. Provider News and Education Two co-workers at different companies can get completely different answers. Individual and Covered California Plans

The 2025 restriction applies in full. Wegovy is excluded unless prior authorization confirms it is medically necessary to treat Class III obesity. Medi-Cal

Blue Shield administers Medi-Cal in several California counties. Effective January 1, 2026, Medi-Cal discontinued coverage of GLP-1 medications prescribed for weight loss or weight-related indications for adults aged 21 and older. Wegovy is classified by Medi-Cal as a weight loss medication and is not covered for any indication under the standard benefit. Prior authorization requests may still be considered for noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH) or cardiovascular disease, and patients under 21 may remain eligible for GLP-1 weight loss therapy with an approved prior authorization.5CMA Docs. Medicare Advantage

Blue Shield’s Medicare Advantage and Medicare prescription drug plans explicitly exclude drugs used for weight loss from their formularies,6Blue Shield of California. Medicare Formularies which tracks the federal rule in the Medicare Prescription Drug, Improvement and Modernization Act of 2003 barring Part D plans from covering anti-obesity drugs. Part D will cover Wegovy when it’s prescribed for an FDA-approved use other than weight loss, such as cardiovascular risk reduction or MASH.7AARP. A separate federal path opens July 1, 2026. The Medicare GLP-1 Bridge is a temporary CMS program running through December 31, 2027, that offers eligible Medicare beneficiaries Wegovy, Zepbound (KwikPen formulation), and Foundayo for weight management at a fixed copay of $50 per monthly fill. It operates outside the standard Part D benefit, so Blue Shield doesn’t need to opt its members in. Blue Shield Medicare Advantage members enrolled in an MA-PD coordinated care plan are eligible, and prior authorization is handled through a central processor run by Humana rather than the Part D plan.8CMS. Medicare GLP-1 Bridge

To qualify for the Bridge, beneficiaries must be at least 18, use the drug alongside lifestyle modifications, and meet one of these thresholds:

  • BMI of 35 or higher, with no additional diagnosis required.
  • BMI of 30 or higher, with heart failure (preserved ejection fraction), uncontrolled hypertension, or chronic kidney disease (stage 3a or higher).
  • BMI of 27 or higher, with pre-diabetes, previous heart attack, previous stroke, or symptomatic peripheral artery disease.

The $50 copay does not count toward Part D deductibles or out-of-pocket limits, and the Extra Help low-income subsidy cannot be applied to it.9CMS. Medicare GLP-1 Bridge – Information for Providers10Medicare Rights Center. GLP-1 Weight Loss Drug Demonstration Begins July 2026

What You’ll Pay Without Coverage

The list price for a 28-day supply of Wegovy is roughly $1,350, or about $16,200 a year. Novo Nordisk sells reduced self-pay pricing directly: the 1.5 mg and 4 mg doses run $149 a month through August 31, 2026, after which the 4 mg dose rises to $199. Starter doses (0.25 mg and 0.5 mg) cost $199 a month for the first two months and $349 a month after that.11Wegovy. What to Pay for Wegovy

Novo Nordisk also offers a savings card for patients whose commercial insurance does cover the drug, cutting the copay to as little as $25 a month with a maximum savings of $100 per fill. The card is not available to patients on government-funded programs such as Medicare, Medicaid, or Tricare.11Wegovy. What to Pay for Wegovy

Appealing a Denial

If Blue Shield denies Wegovy, you have three overlapping routes:

  • File an internal grievance within 180 days of the denial. You can submit online, by mail, or by calling (800) 393-6130. Standard grievances are resolved within 30 calendar days, and an expedited appeal can be resolved within three calendar days when waiting poses a serious health risk.
  • Request an Independent Medical Review (IMR). When a denial is based on medical necessity, you can ask for a free external review. The internal appeal generally has to come first. Skipping an eligible IMR can forfeit your right to take legal action over the denied service.
  • Complain to the California Department of Managed Health Care at (888) 466-2219 or dmhc.ca.gov if your grievance is unresolved after 30 days or involves an urgent situation.

You can also designate a physician, attorney, family member, or friend to act on your behalf using an Appointment of Representative form.12Blue Shield of California. Grievance Process

Appeals are worth taking seriously because regulators have found problems with how Blue Shield handles Wegovy denials. In a 2025 enforcement action, the DMHC concluded that Blue Shield violated the Knox-Keene Health Care Service Plan Act after denying a Wegovy refill despite having medical records confirming the member’s weight and adherence to diet, exercise, and behavioral requirements. One response letter simultaneously upheld the denial for lack of medical necessity and told the member that an internal specialist had approved the request. Blue Shield acknowledged it had “overlooked” the peer review report during the grievance process. The drug was ultimately authorized nearly two months after the initial request. The DMHC assessed a $30,000 penalty, which Blue Shield agreed to pay in a settlement signed in August 2025.13DMHC. Enforcement Matter No. 24-732

Pending California Legislation

No California law currently forces insurers to cover anti-obesity medications. Assembly Bill 575, the Obesity Prevention Treatment Parity Act, would have required plans to cover at least one FDA-approved anti-obesity medication and eliminated prior authorization requirements, but it failed in February 2026.14LegiScan. California AB575 Amended

Senate Bill 1089, the Preventive Treatment Health Care Act, is still moving. As of June 2026, it had been re-referred to the Assembly Appropriations Committee. In its current form it would require CalPERS health plans to cover at least one FDA-approved GLP-1 medication for chronic weight management from January 1, 2028, through 2032. It would not affect commercial Blue Shield plans or Medi-Cal enrollees.15CalMatters Digital Democracy. SB 108916CHBRP. SB 1089 Preventive Treatment Health Care Act Analysis
