Health First Colorado, the state’s Medicaid program, does cover vision for adults, but the coverage is narrower than most people expect. Adults 21 and older get one comprehensive eye exam a year at no cost. Eyeglasses and contact lenses are only covered if you’ve had eye surgery at some point in your life. If you just need glasses for poor eyesight and have never had eye surgery, the program won’t pay for them.
The Annual Eye Exam
Every adult 21 and older on Health First Colorado can get one comprehensive eye exam each year at no cost. The exam covers assessment, diagnosis, and management of conditions affecting your eyes and visual system, including systemic conditions like diabetes that show up in the eyes. A comprehensive eye exam typically runs $100 to $250 or more out of pocket without insurance, so this benefit is worth using every year even if you think your vision hasn’t changed. Eye exams catch conditions like glaucoma and macular degeneration before symptoms appear.
When Glasses and Contacts Are Covered
Here’s the biggest limitation. Health First Colorado only pays for eyeglasses and contact lenses for adults who have had eye surgery. The surgery can have happened at any point in your life. If you qualify, the covered eyeglasses include one or two single-vision or multifocal clear plastic or polycarbonate lenses and one standard frame. Contact Lenses After Surgery

Replacement Cycle
If you qualify for post-surgical eyeglasses, you can get a new pair once every 24 months. What Is Not Covered
The exclusions are short, but they hit the things most adults actually want:
- Routine eyeglasses and contacts. If you need vision correction but have never had eye surgery, Health First Colorado will not pay for them. This is the single biggest gap in adult vision coverage.

If You Need Glasses but Don’t Qualify
Most adults on Health First Colorado land in this frustrating position: they can get the exam, but not the glasses their exam says they need. A few paths are worth exploring. Nonprofit organizations like New Eyes run voucher programs that purchase glasses for qualifying adults with a current prescription. Retail optical chains and online eyewear companies sometimes offer basic single-vision glasses starting under $20. Your eye care provider may also know about local charitable programs or sliding-scale clinics. The annual exam gives you the prescription; buying affordable frames and lenses separately is usually the most practical route.
Using the Benefit
Once you’re enrolled, getting your exam is straightforward. The Health First Colorado website at HealthFirstColorado.com has a provider search tool where you can filter by specialty and location. Enter your zip code, select optometry or ophthalmology, and the tool returns nearby providers. Always call the office to confirm they’re currently accepting new Medicaid patients, because the directory isn’t always up to date.
Bring your Health First Colorado member ID card to the appointment. Since no prior authorization is needed, you can schedule directly with the provider’s office. If a Service Is Denied
