Health First Colorado does not cover weight loss medication when it is prescribed solely to lose weight. Under the January 2026 Preferred Drug List, any prior authorization request for a GLP-1 drug used only for weight management is denied.1Colorado Department of Health Care Policy and Financing. Preferred Drug List January 1, 2026 Two medications, Wegovy and Zepbound, can still be approved when they treat a specific condition that happens to involve excess weight, and two separate changes could expand coverage during 2026.
What the Preferred Drug List Says Today
The Preferred Drug List governs which medications Health First Colorado will pay for and under what conditions. Several GLP-1 receptor agonists sit on the preferred list, but they are approved for diabetes, not weight loss. The PDL puts the restriction in plain language: “Prior Authorization for GLP-1 analogues prescribed solely for weight loss will not be approved.”1Colorado Department of Health Care Policy and Financing. Preferred Drug List January 1, 2026
That is the blanket rule. The two exceptions below are where coverage actually opens up.
When Wegovy Can Be Approved
Wegovy (semaglutide) can be authorized when it is prescribed to reduce cardiovascular risk, not to pursue weight loss on its own. All of the following must be true:
- You are 18 or older.
- You have established cardiovascular disease, meaning a history of heart attack, stroke, or symptomatic peripheral arterial disease.
- Your BMI is 25 or higher.
- You do not have a diagnosis of Type 2 diabetes. If you do, other GLP-1 medications are covered for that condition instead.
- The prescription is specifically to reduce the risk of cardiovascular events.
- You have been counseled on diet modification and exercise.
If your doctor can document every one of these points, this is currently the most accessible route to Medicaid-covered semaglutide in Colorado.1Colorado Department of Health Care Policy and Financing. Preferred Drug List January 1, 2026
When Zepbound Can Be Approved
Zepbound (tirzepatide) is listed as non-preferred, which means extra scrutiny, and it is only approved for moderate to severe obstructive sleep apnea. The requirements are stricter:
- You are 18 or older.
- You have moderate to severe obstructive sleep apnea confirmed by an FDA-approved sleep test, with an Apnea-Hypopnea Index of 15 or more events per hour.
- Your BMI is 30 or higher.
- You have tried CPAP therapy for at least six months without success, or you have a medical reason you cannot use it.
- The medication is prescribed by, or in consultation with, a neurologist, pulmonologist, or other sleep medicine specialist.
- You have no personal or family history of medullary thyroid cancer or Multiple Endocrine Neoplasia syndrome type 2, and you are not pregnant or planning to become pregnant.
- You have been counseled on diet and exercise to promote weight loss.
A baseline sleep study has to be submitted with the prior authorization request.1Colorado Department of Health Care Policy and Financing. Preferred Drug List January 1, 2026 Zepbound’s pathway is narrow, and most people asking about weight loss coverage will not qualify through it.
Getting the Prior Authorization Approved
Every weight-related GLP-1 on the Preferred Drug List requires prior authorization. Your prescribing doctor starts the process by contacting Health First Colorado at 1-800-424-5725.2Health First Colorado. My Pharmacy Told Me My Medication Is Denied Because a Prior Authorization Was Needed – What Should I Do? The submission has to include clinical documentation that you meet every criterion for the drug being requested: BMI, relevant diagnoses, and test results such as a sleep study for Zepbound. Processing takes up to 24 hours.3Health First Colorado. Pharmacy Benefits Frequently Asked Questions
The most common reason these requests fail is framing. If the paperwork reads as a prescription for weight loss, it will be denied. If it reads as a prescription for cardiovascular risk reduction or sleep apnea, with the weight criterion listed alongside the primary diagnosis, it has a chance. Make sure your doctor’s documentation ties the medication to the covered condition first.
If you arrive at the pharmacy and learn the medication was denied for lack of prior authorization, ask your doctor’s office to submit the request and come back the next day.
If Your Request Is Denied
A denial comes with a Notice of Action explaining the reasons and your next steps.4Health First Colorado. Appeals The appeal process runs in stages. If you are in a managed care plan, you usually appeal first to your health plan or to the company that manages your pharmacy benefits. Follow the directions in the letter closely, because deadlines vary and a missed one ends the appeal.
If the health plan appeal does not succeed, you can request a state fair hearing through the Office of Administrative Courts, generally within 60 days of the date on your Notice of Action.4Health First Colorado. Appeals To keep receiving benefits while the hearing is pending, the Office of Administrative Courts must get your request within 10 days of your health plan’s final decision. Before the hearing, gather any additional records your doctor can provide that connect the medication to a covered diagnosis rather than to weight loss on its own.
What You’ll Pay
Since July 1, 2023, Health First Colorado members pay no copays for prescription drugs.5Colorado Department of Health Care Policy and Financing. Health First Colorado Pharmacy Benefits If your prior authorization is approved, you owe nothing at the pharmacy counter.
Coverage Changes Coming in 2026
Two developments could broaden what Colorado Medicaid pays for.
The first is state law. The Colorado General Assembly passed SB 25-048, the Diabetes Prevention and Obesity Treatment Act, which directs the Department of Health Care Policy and Financing to cover weight-loss medication for obesity treatment, along with screenings, behavioral and lifestyle therapy, and bariatric surgery.6Colorado General Assembly. SB 25-048 Diabetes Prevention and Obesity Treatment Act – Fiscal Note The practical change at the pharmacy counter depends on when HCPF updates its formulary and prior authorization criteria. Until that happens, the current denial rule stays in force. Check the Preferred Drug List on the HCPF website periodically for updates.
The second is federal. The Centers for Medicare and Medicaid Services launched the BALANCE model (Better Approaches to Lifestyle and Nutrition for Comprehensive Health), which state Medicaid agencies can opt into starting May 2026. Under BALANCE, CMS negotiates lower GLP-1 prices directly with manufacturers for participating states. Covered drugs include Mounjaro, Ozempic, Rybelsus, Wegovy, and the KwikPen formulation of Zepbound.7Centers for Medicare & Medicaid Services. BALANCE Model
BALANCE eligibility is broader than Colorado’s current rules. For adults 18 or older, GLP-1 coverage for weight management is available at one of three tiers:
- BMI of 35 or higher with lifestyle modification.
- BMI of 30 or higher with at least one qualifying condition such as heart failure with preserved ejection fraction, uncontrolled hypertension, chronic kidney disease stage 3a or above, or moderate to severe obstructive sleep apnea.
- BMI of 27 or higher with pre-diabetes, a previous heart attack or stroke, or symptomatic peripheral artery disease.
Beneficiaries covered through BALANCE also get a lifestyle support program from the drug manufacturer at no cost.7Centers for Medicare & Medicaid Services. BALANCE Model States have until July 31, 2026, to respond to the request for applications. Colorado has not publicly confirmed whether it will opt in, though the General Assembly is studying its GLP-1 coverage options.
