Yes, Florida Blue does cover physical therapy. Rehabilitative services are one of the ten essential health benefits required under the Affordable Care Act, so every ACA-compliant Florida Blue plan, whether purchased on the marketplace, offered through an employer, or structured as Medicare Advantage, includes some level of physical therapy coverage.1Florida Blue. What You’ll Pay Per Visit

How Many Visits You Get
Most Florida Blue individual and employer group plans cap physical therapy at a set number of visits per benefit period. The most common limit in current plan documents is 35 visits per year, and that cap typically covers rehabilitation services as a whole, meaning physical therapy, occupational therapy, and spinal manipulations combined.10Florida Blue. myBlue 2325S Summary of Benefits and Coverage, 202511Florida Blue. Do You Need a Referral or Prior Authorization?
Two different gates can stand between you and a covered visit, and they work independently.
On referrals, Florida law lets you see a physical therapist directly for up to 21 consecutive days of treatment. If therapy needs to continue beyond 30 days and the condition hasn’t been assessed by a physician, chiropractor, podiatrist, or other qualifying practitioner, that practitioner must review and sign the plan of care before treatment continues.13The Florida Legislature. Chapter 486, Florida Statutes – Physical Therapy Practice Act Your insurance can be stricter than state law. Florida Blue HMO plans generally require a primary care physician who may need to issue a referral to a specialist, including a physical therapist.14Florida Blue. Types of Health Plans myBlue HMO members are a notable exception and don’t need a referral for physical therapy.15GuidewellGroup. Florida Blue Provider Manual PPO plans generally let members see a specialist without a referral, though out-of-network costs will be higher.
Prior authorization is a separate requirement, and it applies to many Florida Blue plans for rehabilitation services. Several plan SBCs flag it for physical therapy, and the BlueMedicare Premier plan lists both referral and prior authorization as possibilities.5Florida Blue. When Florida Blue Considers Physical Therapy Medically Necessary
Coverage runs through Florida Blue’s Medical Coverage Guidelines. To qualify as medically necessary, physical therapy must be supported by a written plan of care that includes measurable functional goals, the procedures being used, planned frequency and duration, and a discharge plan.17Florida Blue. Medical Coverage Guideline – Outpatient Rehabilitation Therapy
The services must also be complex enough to require a licensed physical therapist, and the patient should be expected to show significant, objective improvement within a predictable time frame. Therapy for general conditioning or fitness, or therapy that isn’t expected to produce a practical functional improvement, generally doesn’t clear the standard. Clinical progress has to be documented, and the plan of care updated when goals aren’t being met or the condition changes.17Florida Blue. Medical Coverage Guideline – Outpatient Rehabilitation Therapy
HMO vs. PPO for Physical Therapy
Plan type affects coverage in three ways: network, referrals, and cost.
HMO plans restrict you to in-network providers for non-emergency care. An out-of-network physical therapist visit on an HMO almost certainly won’t be covered. In return, premiums and per-visit costs tend to be lower when you stay in network. HMOs also typically require a PCP and may require a referral.14Florida Blue. Types of Health Plans
PPO plans are more flexible but can be sharply more expensive out-of-network. On the State of Florida Employees’ Standard PPO, out-of-network rehabilitation runs 40% coinsurance after a $750 individual deductible, plus any balance-billed amount above the plan’s allowed charge. The balance-billed amount doesn’t count toward the out-of-pocket maximum, and the out-of-network out-of-pocket limit on that plan is unlimited.6Florida Blue. Telehealth Physical Therapy
At least some Florida Blue plans cover physical therapy delivered by telehealth. The BlueMedicare Premier HMO covers telehealth physical therapy at the same copay as in-person visits.8GuidewellGroup. BlueMedicare Premier HMO Summary of Benefits, 2026 Sessions are generally conducted by video and include evaluation, exercise demonstration, and progression of a treatment plan, with standard copays or coinsurance applying as they would for a clinic visit.
Finding an In-Network Physical Therapist
Search for in-network physical therapists at providersearch.floridablue.com. Logging in to your member account filters results to providers in your specific plan’s network; as a guest you can enter your zip code and pick your plan from a dropdown. Listings refresh nightly, but Florida Blue recommends calling the provider’s office to confirm they accept your specific plan before scheduling.18Florida Blue. Provider Search

What’s Not Covered
Physical therapy that fails the medical necessity test is excluded. That includes therapy that is educational, vocational, or recreational, and treatment where the therapeutic benefit isn’t documented.17Florida Blue. Medical Coverage Guideline – Outpatient Rehabilitation Therapy General conditioning and fitness programs are typically out, even when a physical therapist runs them. Acupuncture and weight loss programs are commonly excluded from rehabilitation benefits as well.3Florida Blue. myBlue 2325S Summary of Benefits and Coverage, 2026 Because plan terms vary, check your benefit booklet or the customer service number on the back of your member ID card for the full list that applies to you.
