The Healthy Indiana Plan does not cover Wegovy when it’s prescribed for weight loss. Indiana Medicaid excludes “anorectics or any agent used to promote weight loss” from its covered drug categories, and all three HIP managed care plans — MHS, Anthem, and CareSource — enforce that exclusion.1Indiana Medicaid. Pharmacy Benefits There is one narrow opening: Wegovy can be approved through prior authorization to reduce cardiovascular risk in members who meet strict clinical criteria.2OptumRx / Indiana Medicaid. Why HIP Excludes Wegovy for Weight Loss
Federal law treats anti-obesity drug coverage as optional for state Medicaid programs, and Indiana has chosen not to cover it.3KFF. Medicaid Coverage of and Spending on GLP-1s The Indiana Health Coverage Programs rule (405 IAC 5-24) covers FDA-approved prescription drugs except for a short list of excluded categories, and weight-loss drugs are on that list.4OptumRx / Indiana Medicaid. Each HIP plan reflects the exclusion in its own documents. The MHS pharmacy FAQ lists “drugs for weight loss” as not covered, and the MHS Preferred Drug List confirms the exclusion with a limited exception for orlistat.5MHS Indiana. The One Path to Approval: Cardiovascular Risk Reduction
Indiana Medicaid’s prior authorization criteria say plainly that Wegovy is “not covered exclusively for weight loss,” but can be authorized for members with established cardiovascular disease who are overweight or obese and need to lower their risk of heart attack, stroke, or cardiovascular death.2OptumRx / Indiana Medicaid. GLP-1 RA and GIP Combinations Prior Authorization Criteria
To get an initial one-year authorization, a member has to meet every one of the following:
- Age 45 or older.
- BMI of at least 27 kg/m², documented in chart notes from the prior three months.
- Documentation within the past year of a prior heart attack, a prior stroke (ischemic or hemorrhagic), or symptomatic peripheral arterial disease (evidenced by amputation, revascularization, or intermittent claudication with an ankle-brachial index below 0.85 at rest).
- Already taking guideline-directed heart therapy — beta-blockers or renin-angiotensin system inhibitors along with lipid-lowering drugs — or a prescriber explanation of why those are not appropriate.
- No diagnosis of type 1 or type 2 diabetes.
- Prescriber attestation that Wegovy will be used alongside a reduced-calorie diet and increased physical activity.
- No concurrent use of Ozempic or any other semaglutide or GLP-1 receptor agonist.
- A dose no higher than 2.4 mg per week.
Renewal requires consistent use (at least 84 days of fills in the previous 112 days) and continued satisfaction of every original criterion. In practice, this pathway is built for older adults with serious heart disease, not for members looking for help with weight management.
Other Weight-Loss and GLP-1 Drugs Under HIP
The exclusion extends past Wegovy. Saxenda, Contrave, Qsymia, and phentermine are not listed in the GLP-1 prior authorization criteria, and the broad weight-loss exclusion applies to them.4OptumRx / Indiana Medicaid. GLP-1 RA and GIP Combinations Prior Authorization Criteria – April 2025
Wegovy itself now has an FDA-approved MASH indication, but Indiana Medicaid’s Wegovy criteria do not include MASH as an approved use. That pathway exists for Ozempic and Victoza, not Wegovy.2OptumRx / Indiana Medicaid. GLP-1 RA and GIP Combinations Prior Authorization Criteria
If Your Prescription Is Denied
Appeal Through Your Plan
HIP members can appeal a denial through their managed care plan’s internal process. Appeals must be submitted in writing, there is no filing cost, and the denial notice will spell out appeal rights.9Indiana Medicaid. Member Appeals Because the weight-loss exclusion is a categorical policy rather than a clinical judgment, an appeal has the best chance when the prescriber reframes the request around a covered indication such as cardiovascular risk reduction and documents that every required criterion is met.
Self-Pay Pricing
Novo Nordisk has reduced self-pay pricing for Wegovy. The 1.5 mg and 4 mg injection doses run $149 per month through August 31, 2026. New patients on lower doses can start at $199 per month for the first two months, then $349 per month after that. Without any discount, the list price is roughly $1,349 for a 28-day supply.10Novo Nordisk. What to Pay for Wegovy
Patient Assistance Programs
Novo Nordisk’s Patient Assistance Program is for uninsured patients and excludes anyone enrolled in or eligible for Medicaid. A Medicaid-eligible person would have to show proof of a Medicaid coverage denial before being considered.11NovoCare. What Could Change
CMS proposed a rule (CMS-4208-P) that would have required state Medicaid programs to cover anti-obesity drugs, but the Trump administration did not finalize that provision in the Contract Year 2026 final rule, stating it was “not appropriate at this time.”12CMS. Contract Year 2026 Policy and Technical Changes Final Rule Fact Sheet
A separate voluntary demonstration called BALANCE (Better Approaches to Lifestyle and Nutrition for Comprehensive Health) would expand Medicaid coverage of GLP-1 drugs for obesity in participating states beginning May 1, 2026. Participation is optional, with a state application deadline of July 31, 2026, and the program is scheduled to run through December 2031.13KFF. What to Know About the BALANCE Model for GLP-1s in Medicare and Medicaid Whether Indiana will apply has not been reported. At the state level, House Bill 1202, “Medicaid coverage for treatment of obesity,” was introduced in Indiana’s 2025 legislative session and referred to the Committee on Public Health, with no further action reported.14LegiScan.
