Does HMSA Cover Weight Loss Medication? GLP-1 Rules and Exceptions

HMSA does not cover weight loss medication under most of its plans. Weight loss programs appear on the excluded services list across HMSA’s commercial, employer, and federal plan documents, and anti-obesity drugs generally fall outside standard pharmacy benefits. There are a few openings: a GLP-1 prescribed for a different diagnosis may be covered, the Federal Plan 87 has a defined anti-obesity drug pathway, prior authorization exceptions exist, and Medicare Advantage members gain access to specific GLP-1s through a new federal program starting July 1, 2026.

What HMSA’s Plans Exclude

HMSA’s Summary of Benefits and Coverage documents list “weight loss programs” as an excluded service across its main plan types. That includes the Platinum PPO on the individual and employer market,1HMSA. Platinum PPO Summary of Benefits and Coverage the plans offered through the state Employer-Union Health Benefits Trust Fund (which carves out an exception for anything “required by Health Reform Law”),2EUTF Hawaii. A review of HMSA’s state employee coverage by the GW Stop Obesity Alliance found that anti-obesity drugs are generally not covered, though coverage is “possible with prior authorization.” The same review noted that behavioral therapy for weight reduction, along with related exams, lab tests, and supplies, is excluded regardless of medical necessity.4GW Stop Obesity Alliance. GLP-1 Drugs: The Diagnosis Changes the Answer

Semaglutide and tirzepatide carry different brand names depending on what they’re approved to treat. Semaglutide is sold as Ozempic for type 2 diabetes and as Wegovy for weight management. Tirzepatide is sold as Mounjaro for diabetes and Zepbound for weight management. These are the same molecules, but HMSA’s formularies treat them differently based on the condition being treated.

When a GLP-1 is prescribed for type 2 diabetes or cardiovascular risk reduction, it may fall under standard pharmacy benefits. When the same class of drug is prescribed specifically for weight loss, the anti-obesity exclusion typically applies. HMSA does maintain a dedicated “Anti-Obesity Medication” policy, though it is written for the HMSA Federal Plan 87 and the Postal Service formulary, suggesting those federal plans have a defined coverage pathway even where the general rule is exclusion.5HMSA. Medicare Advantage: Bridge Program Starting July 2026

Federal law has long barred Medicare Part D from paying for drugs used only for weight loss. That restriction is loosening through a temporary program that covers members of Medicare Advantage plans with drug coverage, including HMSA Akamai Advantage.

Starting July 1, 2026, the Medicare GLP-1 Bridge program covers Wegovy (injection or tablet), Zepbound (KwikPen only), and Foundayo (tablet). The copay is $50 per one-month supply, and that amount does not count toward the Part D deductible or out-of-pocket maximum.6Medicare.gov. Weight Loss Drugs

Eligibility is tied to BMI and other health conditions:

  • BMI of 35 or higher qualifies on its own.
  • BMI between 30 and 34.99 qualifies with at least one listed condition, such as uncontrolled hypertension, chronic kidney disease (stage 3a or higher), prediabetes, diastolic heart failure, or a history of heart attack or stroke.
  • BMI between 27 and 29.99 qualifies only with prediabetes, a history of heart attack or stroke, or symptomatic peripheral artery disease.

You cannot use the Bridge program if you already receive a covered GLP-1 through your regular Part D plan, or if you have type 2 diabetes, moderate-to-severe sleep apnea, or fatty liver disease. The prescriber must submit a prior authorization certifying that the medication is being used alongside a diet and exercise program. Approved authorizations run through December 31, 2027.6Medicare.gov. Weight Loss Drugs

How To Check Your Plan and Request an Exception

HMSA runs dozens of plan variations, and formularies differ. To find out what your specific plan covers:

  • Log in to My Account at hmsa.com and use the Check Drug Cost tool. Enter the drug name and dosage to see coverage status, cost, and any restrictions.7HMSA. Check Drug Cost Tool
  • Identify your formulary using the drug code on your membership card, then look it up on hmsa.com.8HMSA. Your HMSA Drug Formulary List

For Medicare plans, HMSA generally decides within 72 hours, or within 24 hours on an expedited request when a delay could harm your health.9HMSA. If Coverage Isn’t Available: Manufacturer Self-Pay Options

Without insurance, GLP-1s can run over $1,000 per month. Manufacturers offer direct-pay programs that bring the cost down. Novo Nordisk’s NovoCare program offers Wegovy starting at $199 per month for new patients. Eli Lilly’s LillyDirect prices Zepbound between $299 and $499 per month depending on dosage. An oral Wegovy tablet launched in January 2026 at cash pricing starting at $149 per month, and a generic version of liraglutide (Saxenda) reached pharmacies in August 2025.
