Indiana Medicaid does not cover Zepbound for weight loss. The only route to coverage is a narrow prior authorization for adults who have obesity along with moderate to severe obstructive sleep apnea. If you are hoping to get tirzepatide paid for as a chronic weight management drug, Indiana’s program will not approve it, and that applies across Hoosier Healthwise, the Healthy Indiana Plan, and the state’s managed care plans.
Why Weight Loss Prescriptions Are Excluded
Indiana’s Medicaid pharmacy benefit page states plainly that “medications used for weight loss” are not covered.1Indiana Medicaid. Pharmacy Benefits Federal law lets states opt out of covering anti-obesity drugs under the Medicaid Drug Rebate Program, and Indiana has opted out.2KFF. Medicaid Coverage of and Spending on GLP-1s
Zepbound and Wegovy are both flagged in Indiana Medicaid’s prior authorization criteria as “not covered exclusively for weight loss.”3Optum Rx Indiana Medicaid. GLP-1 RA/GIP Combinations Prior Authorization Criteria The word “exclusively” is what opens the one door. Zepbound can be authorized, but only when the prescription targets a specific medical condition beyond weight loss.
Managed care formularies reflect this the same way. CareSource lists Zepbound as a Tier 2 drug requiring prior authorization, with coverage terms matching the statewide criteria.4CareSource. Indiana Medicaid Preferred Drug List Managed Health Services is blunter: its formulary excludes drugs prescribed for weight loss (with the exception of Orlistat) and does not list Zepbound at all.5Managed Health Services Indiana. The Obstructive Sleep Apnea Exception
Since April 2025, Indiana Medicaid has authorized Zepbound for adults with moderate to severe obstructive sleep apnea (OSA) who also have obesity. The FDA approved Zepbound for this indication in December 2024, and Indiana built a prior authorization pathway to match.3Optum Rx Indiana Medicaid. GLP-1 RA/GIP Combinations Prior Authorization Criteria To qualify, you must meet every one of the following:
- Age 18 or older.
- Moderate to severe OSA with obesity, confirmed by a sleep study showing an apnea-hypopnea index (AHI) of 15 or more events per hour, or a documented history of AHI at that level together with current CPAP or BiPAP use.
- BMI of 30 kg/m² or higher, documented within the past three months.
- No Type 1 or Type 2 diabetes diagnosis, and an HbA1c below 6.5%.
- Prescriber attestation that you will use Zepbound alongside a reduced-calorie diet and increased physical activity.
- Dose capped at 15 mg per week.
- No concurrent use of another GLP-1 receptor agonist or tirzepatide product.
Approvals have been scarce. An Indiana Medicaid preferred drug list evaluation covering July 2024 through June 2025 reported a combined 24 prior authorizations approved for Wegovy and Zepbound during the full year, generating 244 claims and about $322,000 in spending.6Indiana General Assembly. Keeping Coverage After the First Year
Initial authorization lasts one year. To renew it, you have to show you’ve been taking the medication consistently (at least 84 of the prior 112 days), that you’ve lost at least 5% of your baseline weight, and that your AHI has dropped or you are still using CPAP or BiPAP therapy.3Optum Rx Indiana Medicaid. GLP-1 RA/GIP Combinations Prior Authorization Criteria Miss the adherence threshold or the weight-loss benchmark, and renewal can be denied even if you still meet the clinical criteria that got you approved in the first place.
If You Have Type 2 Diabetes
The diabetes exclusion in the Zepbound criteria is strict, but it is not the end of the road for tirzepatide. If you have Type 2 diabetes, Indiana Medicaid covers Mounjaro, the diabetes-branded version of the same molecule. The catch is that you first have to try and fail two preferred GLP-1 medications.7Optum Rx Indiana Medicaid. Children and Teens
Zepbound is not available to Indiana Medicaid members under 18. Federal Medicaid law requires states to cover medically necessary treatment for members under 21 through the Early and Periodic Screening, Diagnostic and Treatment (EPSDT) program, and Indiana does use EPSDT to cover Wegovy and Saxenda for minors, according to a 2023 IHCP bulletin.8Indiana Medicaid. IHCP Bulletin BT2023148 Zepbound is not on that list. The FDA has approved it only for adults, and a clinical trial in adolescents ages 12 to 17 is not expected to produce final results until 2027.9FDA. Zepbound NDA Approval Letter
