Whether Kaiser covers weight loss drugs in California depends on which Kaiser plan you have and why the drug is being prescribed. On most standard Kaiser commercial and marketplace plans, GLP-1 medications like Wegovy and Zepbound are not covered when the only reason for the prescription is weight loss. Coverage opens up when the same drug is prescribed for an underlying condition like type 2 diabetes, cardiovascular disease, or obstructive sleep apnea, and a few plan types, notably federal employee plans, cover weight loss use directly with strict requirements.
Standard Commercial and Marketplace Plans
As of January 2025, Kaiser dropped base coverage for GLP-1 medications prescribed solely for weight loss for commercial and ACA members in California with a body mass index under 40.1Becker’s Hospital Review. The GLP-1 Dilemma Persists Into 2025 A Kaiser business flyer puts it plainly: GLP-1s and other anti-obesity medications are “no longer covered when prescribed for the sole purpose of weight loss,” and patients are responsible for the full cost.2Kaiser Permanente Business. Members with a BMI of 40 or above may have limited access subject to strict step therapy requirements, and some regions provide Zepbound coverage for patients with severe obstructive sleep apnea who meet specific criteria.3Find Honest Care. Employer-Sponsored Plans
If you get Kaiser through work, whether a weight loss drug is covered depends on what your employer bought. Weight loss drugs are excluded from the base pharmacy benefit, but employers can purchase an optional rider that adds coverage for weight management medications.6Kaiser Permanente Business. Federal Employee and Postal Service Plans
Kaiser’s Federal Employees Health Benefits (FEHB) and Postal Service Health Benefits (PSHB) plans do cover GLP-1 drugs and oral anti-obesity medications for weight management, at 50% coinsurance of the plan allowance.8Kaiser Permanente. Kaiser Permanente FEHB Weight Management Programs and GLP-1 Requirements Prior authorization is heavy. You generally need:
- A baseline BMI of 27 or greater with a weight-related comorbidity.
- Documentation of intolerance or failure to lose at least 5% body weight after a three-month trial of other weight management medications.
- Enrollment in a lifestyle intervention and intensive behavioral program.
- Monthly in-office exams with weight checks, plus attestation that you are following a reduced-calorie diet and exercising.
Initial approval runs three to six months, and the plan reassesses at the three-month mark to decide whether you can continue.9Kaiser Permanente. Kaiser Permanente PSHB Weight Management Programs and GLP-1 Requirements
Kaiser Medi-Cal
California eliminated Medi-Cal coverage for GLP-1 drugs prescribed for weight loss effective January 1, 2026. The change was enacted through state budget law (SB 101) and applies to both fee-for-service and managed care Medi-Cal, including Kaiser Medi-Cal plans.10Los Angeles Times. Coverage continues in two situations. Wegovy remains covered for noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH) or cardiovascular disease, Zepbound for obstructive sleep apnea, and diabetes-indication GLP-1s like Ozempic and Mounjaro for type 2 diabetes.12California Medical Association. Kaiser Medicare Advantage
Federal law prohibits Medicare Part D plans from covering medications prescribed specifically for weight loss, which includes Kaiser’s Medicare Advantage plans in California. No plan has the authority to override the exclusion.14Medicare Rights Center. GLP-1 Weight Loss Drug Demonstration Begins July 2026 GLP-1 drugs prescribed for diabetes or other covered conditions can still be covered through Part D.
A limited federal workaround exists. The Centers for Medicare and Medicaid Services launched the Medicare GLP-1 Bridge Program, running from July 1, 2026, through December 31, 2027. It operates outside of standard Part D, is managed centrally by Medicare rather than by individual plans, and charges a fixed $50 monthly copayment that does not count toward Part D out-of-pocket limits. Part D plans are not required to participate.14Medicare Rights Center. GLP-1 Weight Loss Drug Demonstration Begins July 2026
When the Same Drug Is Covered for Another Condition
Across plan types, the key distinction is why the drug is being prescribed. If you have type 2 diabetes, cardiovascular disease, or obstructive sleep apnea and your doctor prescribes a GLP-1 for that condition, coverage is generally available on the plans that otherwise exclude weight loss use. This is why two Kaiser members can both be prescribed Zepbound and get different answers from the pharmacy: one has an obstructive sleep apnea diagnosis on file, the other does not.
Prior Authorization: What To Expect If Your Plan Does Cover It
Even when coverage exists, Kaiser requires prior authorization, and the criteria are detailed. Kaiser Permanente Northwest’s criteria for Wegovy, effective February 2026, show what members typically face:15Kaiser Permanente. KP Criteria Wegovy NW Commercial
- BMI of 30 or above, or BMI of 27 or above with hypertension, diabetes, or high cholesterol.
- Prior trial and failure of at least two other weight management medications (such as phentermine, topiramate, or naltrexone/bupropion) for three months each, plus a trial and failure of semaglutide (Ozempic).
- Current participation in a diet and exercise program, with weight and BMI documented in the last 30 days.
- To continue the drug, you must show at least 5% weight loss has been achieved and maintained.
For adolescents aged 12 to 17, the criteria require a diagnosis of class 2 or class 3 obesity (BMI of 35 or above, or at least 120% of the 95th percentile) and failed trials of multiple prior medications. California plan criteria may differ in detail, so ask Kaiser for the specific criteria that apply to your plan.
If Kaiser Denies Coverage
You have a meaningful appeal path in California. After completing one level of Kaiser’s internal grievance process, you can request an Independent Medical Review through the California Department of Managed Health Care. The review is free, and the assigned reviewer has no affiliation with Kaiser. If the reviewer finds the denial was not medically appropriate, DMHC can order Kaiser to reverse it. Urgent cases can get an expedited decision within three business days.16Muni Health. Kaiser Permanente Appeal Guide 2026 The DMHC Help Center is at 1-888-466-2219.
One boundary worth knowing: no California law currently requires commercial health plans to cover weight loss medications. AB 575, which would have imposed that mandate on state-regulated plans, failed in February 2026.17CalMatters Digital Democracy. AB 575 For now, your plan documents and the appeal process are what determine your access.
