Yes, MassHealth does cover abortion. Both medication and surgical abortion are covered as part of the program’s family planning services, and members pay nothing — no copays, no deductibles, and no other cost-sharing through MassHealth or any MassHealth health plan.1Mass.gov. Sexual and Reproductive Health Service Insurance and Cost Because federal Medicaid dollars generally cannot pay for abortion, Massachusetts funds this coverage with state money.2Guttmacher Institute. What’s Covered
MassHealth covers abortion by medication or by surgery.3Mass.gov. MassHealth Sexual and Reproductive Health Services for Members No referral is required for pregnancy-related care, including abortion.4Mass.gov. Information for Pregnant MassHealth Members
Coverage does not stop at the procedure. MassHealth continues healthcare coverage for 12 months after a pregnancy ends, whatever the outcome.4Mass.gov. Information for Pregnant MassHealth Members Members who had an abortion in the last three months may be eligible for 12 months of continuous coverage starting from the date the pregnancy ended.3Mass.gov. MassHealth Sexual and Reproductive Health Services for Members
Doula support during an abortion, miscarriage, or delivery is also covered, and that benefit runs for up to 12 months after the pregnancy ends. The doula program launched in spring 2024 and is open to all pregnant and postpartum members without a provider recommendation.5Mass.gov. MassHealth Announces Coverage of Doula Services
When MassHealth Pays
Under MassHealth regulations, the program pays for an abortion when one of two conditions is met: a provider determines it is medically necessary “in light of all factors affecting the pregnant individual’s health,” or the pregnancy resulted from rape or incest that was reported to a law enforcement agency or public health service within 60 days.6Cornell Law Institute. 130 CMR 433.455 The medical necessity standard is broad, and Massachusetts is grouped among the states that cover all or most abortions for Medicaid enrollees under that designation.2Guttmacher Institute. The paperwork is on the provider’s side. Providers must complete a Certification for Payable Abortion form and keep it in your record. In some circumstances they need more: two providers must certify the risk of severe, long-lasting physical health damage, and claims for rape or incest cases require documentation from law enforcement or a public health agency.6Cornell Law Institute. 130 CMR 433.455 Providers are prohibited from using any form of coercion, and your decision about abortion cannot affect your eligibility for other MassHealth benefits.7Cornell Law Institute. 130 CMR 484.002
Who Qualifies
MassHealth eligibility usually turns on income, residency, and citizenship or immigration status. Pregnancy is a notable exception. Pregnant individuals can qualify for MassHealth Standard — the comprehensive coverage tier — regardless of immigration status, as long as household income is at or below 200% of the federal poverty level.8Mass.gov. MassHealth Information for Noncitizens That full coverage, including abortion services, continues through 12 months postpartum.4Mass.gov. Information for Pregnant MassHealth Members
Noncitizens who otherwise would be limited to MassHealth Limited (emergency-only coverage) can therefore access abortion services under MassHealth Standard during pregnancy.
Rules for Minors
Coverage is one question; consent is another. Massachusetts requires parental consent for abortion for minors under 16. The 2020 ROE Act removed the parental consent requirement for 16- and 17-year-olds.9American Journal of Public Health. Impact of Removing Parental Consent Requirements on Abortion Timing A minor under 16 who cannot or does not want to get a parent’s permission can petition a court through a judicial bypass process.10Planned Parenthood. Gestational Limits
MassHealth coverage tracks what state law allows. In Massachusetts, a physician, physician assistant, nurse practitioner, or certified nurse midwife may perform an abortion if, in their best medical judgment, the pregnancy has existed for less than 24 weeks.11Mass.gov. Mass General Laws c.112 Section 12M After 24 weeks, abortion is permitted in cases of fatal fetal diagnoses under the 2020 ROE Act.12Mass.gov. Finding a Provider
You do not need a referral. Members can locate providers through the sexual and reproductive health service map on Mass.gov or by calling MassHealth Customer Service at (800) 841-2900.3Mass.gov. MassHealth Sexual and Reproductive Health Services for Members
Planned Parenthood League of Massachusetts accepts MassHealth. Because federal law blocks Planned Parenthood from receiving federal Medicaid reimbursement, it uses a mix of state funding and donor support to cover the cost of care for MassHealth patients. You should not have to pay out of pocket.14Planned Parenthood. Medicaid Information
One warning from MassHealth: “crisis pregnancy centers” and “pregnancy resource centers” are not part of the MassHealth network and do not offer the reproductive health services the program covers, including abortion.3Mass.gov.
