Does MassHealth Cover GLP-1? Exceptions and Your Options

MassHealth does cover GLP-1 medications, but coverage depends on why the drug is prescribed. GLP-1s prescribed for type 2 diabetes and prediabetes, including Ozempic, Mounjaro, Trulicity, and Rybelsus, remain covered without interruption. GLP-1s prescribed for weight loss, including Wegovy, Zepbound, and Saxenda, stop being covered on July 3, 2026, unless the member qualifies under a narrow list of other medical conditions through prior authorization.1Mass.gov. MassHealth Pharmacy Bulletin, Issue 7, May 2026

What MassHealth Still Covers

Diabetes care is unaffected. GLP-1 and GIP/GLP-1 drugs with an FDA-approved indication for type 2 diabetes stay on the MassHealth Drug List under the antidiabetic formulary. That includes Ozempic (semaglutide injection), Mounjaro (tirzepatide), Trulicity (dulaglutide), and Rybelsus (semaglutide tablets).2MassHealth Drug List. Antidiabetic Agents – GLP-1 Agonists

If a MassHealth member is currently taking a weight-loss GLP-1 to manage type 2 diabetes or prediabetes, the state’s instruction to prescribers is to switch the patient to a diabetes-indicated GLP-1 under the existing antidiabetic drug criteria rather than seek a weight-loss authorization.3Point32Health. What Stops Being Covered on July 3, 2026

Under 130 CMR 406.413(B), MassHealth will no longer pay for any drug used solely to treat obesity or overweight. The rule applies across all MassHealth plan types, including fee-for-service, managed care organizations, One Care, and Senior Care Options. Any drug prescribed off-label for weight loss is also non-payable after that date.1Mass.gov. MassHealth Pharmacy Bulletin, Issue 7, May 2026

The drugs affected when prescribed for weight loss are:

  • GLP-1 and GIP/GLP-1 agents: Wegovy (semaglutide), Zepbound (tirzepatide), and Saxenda (liraglutide).
  • Other anti-obesity medications: benzphetamine, diethylpropion, orlistat (Xenical), phendimetrazine, phentermine (Adipex-P, Lomaira), and phentermine/topiramate extended-release.

The change affects roughly 22,000 MassHealth members currently taking a GLP-1 for weight loss.4WBUR. Mass Cutting GLP-1 Coverage Medicaid Wegovy Zepbound

Exceptions That Keep GLP-1 Coverage

MassHealth will continue to authorize a GLP-1 after July 2026 for members who have one of several specific medical conditions beyond obesity. A prescriber must submit a prior authorization request with clinical documentation of the qualifying indication. Approvals last up to six months.5Mass.gov. MassHealth Pharmacy Bulletin, Issue 5, March 2026

The qualifying conditions are:

  • Established cardiovascular disease (history of heart attack, stroke, symptomatic peripheral arterial disease, or revascularization) with a BMI above 27 kg/m², for reduction of major adverse cardiovascular events.1Mass.gov. MassHealth Pharmacy Bulletin, Issue 7, May 2026
  • Moderate to severe obstructive sleep apnea in adults with a BMI above 30 kg/m². The prior authorization must include the member’s apnea-hypopnea index score.5Mass.gov. MassHealth Pharmacy Bulletin, Issue 5, March 2026
  • Non-cirrhotic MASH (metabolic dysfunction-associated steatohepatitis) with moderate to advanced fibrosis at stage F2–F3. The request must document the fibrosis stage and include consultation notes from a gastroenterologist or hepatologist.6MassHealth Drug List. Anti-Obesity GLP-1 Receptor Agonists Prior Authorization
  • Members under 21. All pediatric requests are reviewed for medical necessity under the federal Early and Periodic Screening, Diagnostic, and Treatment requirement, a protected pathway that applies regardless of the broader exclusion.3Point32Health. One point about recertification matters here. MassHealth evaluates eligibility using the member’s baseline BMI from before they started any GLP-1, not their current weight. The agency has acknowledged that some members who are currently stable on a weight-loss GLP-1 “may no longer be eligible under these revised criteria and will be denied coverage at the time of recertification.”6MassHealth Drug List. What Happens to a Current Prior Authorization

Every prior authorization submitted before February 17, 2026 will be end-dated for July 2, 2026, regardless of what indication it was originally approved for. To keep coverage after that date, the prescriber must submit a new prior authorization with documentation of one of the qualifying conditions above. If the clinical documentation is missing, the authorization will not be extended.1Mass.gov. MassHealth Pharmacy Bulletin, Issue 7, May 2026

There is also a change in which GLP-1 MassHealth prefers. As of July 3, 2026, Wegovy becomes the sole preferred GLP-1 for the remaining covered indications, and Zepbound loses preferred status. Members with an approved Zepbound prior authorization will have it automatically replaced with one for Wegovy for the rest of the original authorization period, and the prescriber must submit a new prescription. Continuing on Zepbound after that requires documentation that the patient first tried Wegovy.1Mass.gov. MassHealth Pharmacy Bulletin, Issue 7, May 2026

If You Lose Coverage, What Are Your Options

Start with the prescriber. If a doctor can document established cardiovascular disease, moderate-to-severe sleep apnea, or MASH with stage F2–F3 fibrosis, the member may be able to keep the medication through the prior authorization pathway described above.5Mass.gov. MassHealth Pharmacy Bulletin, Issue 5, March 2026 Members with type 2 diabetes or prediabetes should ask about switching to a diabetes-indicated GLP-1 like Ozempic or Mounjaro, which stay on the antidiabetic formulary.3Point32Health. Without insurance, these drugs run around $450 per month at cash price.4WBUR. The program excludes anyone enrolled in a government insurance program, so a current MassHealth member would only qualify after losing MassHealth and not enrolling in any other government plan.7Novo Nordisk. Why MassHealth Made This Change

Cost. Prescription drug spending at MassHealth rose 18 percent in 2025, accounting for more than a quarter of total per-member cost growth, and GLP-1s drove nearly half of that drug-spending increase.10Mass.gov.
