MassHealth does cover Zepbound right now, but only with prior authorization, and the coverage ends for most members on July 1, 2026. After that date, MassHealth will stop paying for Zepbound and other medications prescribed solely for weight loss. Coverage continues only for members who have specific medical conditions beyond obesity itself, and even those members will need to try Wegovy before Zepbound is approved.
Coverage Through June 30, 2026
Zepbound is currently a preferred drug on the MassHealth Drug List. It requires prior authorization and is limited to four pens per 28 days.1MassHealth Drug List. Therapeutic Class Detail – Zepbound
To get the initial approval, your prescriber has to document a qualifying diagnosis, confirm you’re at least 18, record a recent baseline weight, and show you’ve been counseled on diet and physical activity. BMI requirements depend on your health picture: a BMI of 35 or higher qualifies on its own, while a BMI of 27 or 30 qualifies only if you also have a weight-related condition such as type 2 diabetes, a prior heart attack, hypertension, or sleep apnea.1MassHealth Drug List. Therapeutic Class Detail – Zepbound
Since October 2025, members who have both diabetes and obesity have had to try Mounjaro first, or document why it isn’t appropriate. MassHealth treats the two drugs as interchangeable because they share the same active ingredient, dosing, and route of administration.2Mass.gov. MassHealth Pharmacy Facts, Issue 12
Renewals require evidence that the drug is working: at least 5% weight loss from baseline, improvement in a secondary measure like blood pressure or blood sugar, or documented improvement in obstructive sleep apnea symptoms. The weight on file must be from within the last 90 days.1MassHealth Drug List. Therapeutic Class Detail – Zepbound
What Changes on July 1, 2026
Starting July 1, 2026, MassHealth will no longer pay for drugs used solely to treat obesity or overweight. The change sweeps in a long list of anti-obesity medications: Zepbound, Wegovy, Saxenda, Contrave, Qsymia, Xenical, and others all lose their weight-loss coverage.3Point32Health. MassHealth Coverage Change – Anti-Obesity Medications
Zepbound also loses its preferred status and becomes non-preferred. For the narrower set of covered indications that remain, Wegovy becomes the sole preferred drug, so anyone who wants Zepbound for a covered medical condition has to try Wegovy first and show that it didn’t work or isn’t appropriate.4Mass.gov. MassHealth Pharmacy Facts, Issue 7
Who Still Qualifies After July 1, 2026
Coverage doesn’t disappear entirely. If you have a qualifying medical condition, your prescriber can submit a new prior authorization for a GLP-1 medication for that specific indication. The conditions MassHealth will still cover are:
- Established cardiovascular disease (prior heart attack, stroke, symptomatic peripheral arterial disease, or prior revascularization) with a BMI of 27 or higher, to reduce major adverse cardiovascular events.5Mass.gov. June Update on Vida Health Program and GLP-1 Coverage
- Moderate to severe obstructive sleep apnea with a BMI of 30 or higher.5Mass.gov. June Update on Vida Health Program and GLP-1 Coverage
- Non-cirrhotic metabolic dysfunction-associated steatohepatitis (MASH) with moderate to advanced fibrosis, with no BMI requirement.5Mass.gov. June Update on Vida Health Program and GLP-1 Coverage
- Members under 21, reviewed for medical necessity under federal Early and Periodic Screening, Diagnostic, and Treatment rules.3Point32Health. MassHealth Coverage Change – Anti-Obesity Medications
If you have diabetes or prediabetes, MassHealth expects you to switch to a diabetes-indicated GLP-1 like Mounjaro or Ozempic, which stay covered under their own criteria.3Point32Health. MassHealth Coverage Change – Anti-Obesity Medications Adolescents aged 12 to 21 who are new to GLP-1 therapy face an additional step: a trial of phentermine before any GLP-1 agent is approved.6Mass General Brigham Health Plan. What Happens to a Current Authorization
If you already have an approved prior authorization for Zepbound, you can keep filling it through June 30, 2026.6Mass General Brigham Health Plan. Formulary Updates May 2026 Newsletter – Medicaid Prior authorizations issued before February 17, 2026, are being end-dated as of June 30, and affected members will get letters notifying them of the change.3Point32Health. If your current Zepbound authorization was approved for one of the qualifying medical conditions and extends past July 2, MassHealth will replace it with an approved authorization for Wegovy for the rest of the original period. That transition does require a new prescription for Wegovy from your prescriber.4Mass.gov. MassHealth Pharmacy Facts, Issue 7
If Your Prior Authorization Is Denied
You have options after a denial. Your prescriber can submit a new prior authorization with additional clinical evidence, including peer-reviewed literature explaining why standard step therapy isn’t appropriate for you.7Mass.gov. MassHealth Appeal Decision 2412410 If the denial comes from a managed care plan, you have to exhaust that plan’s internal appeal first. After that, MassHealth regulations entitle you to a fair hearing, and a denial at the fair hearing can be appealed to Superior Court within 30 days.8Mass.gov. MassHealth Appeal Decision 2511025
Paying Out of Pocket
Members who lose coverage have narrow choices if they want to stay on Zepbound. Eli Lilly’s savings card is off limits: federal law bars manufacturer coupons for drugs covered by government programs, and Lilly’s own terms exclude anyone enrolled in Medicaid, Medicare, or other government-funded insurance.9Eli Lilly. Zepbound Savings The Lilly Cares patient assistance program does not currently cover Zepbound.10NiceRx. Zepbound Patient Assistance Program
Lilly does sell Zepbound vials directly through its LillyDirect platform on a cash-pay basis, priced between $349 and $699 per month depending on the dose. These are vials that require self-injection with a syringe rather than the autoinjector pen. Insurance cannot be applied to the cash price.11NPR. Zepbound Eli Lilly Direct Cost
Other Obesity Treatments MassHealth Still Covers
Bariatric surgery remains a covered benefit, subject to prior authorization and the medical necessity criteria in MassHealth’s clinical guidelines. Managed care members should check their plan’s own medical policies, which can differ from the general MassHealth standard.12Mass.gov. MassHealth Guidelines for Medical Necessity Determination for Bariatric Surgery Medical nutrition therapy is also covered, including nutritional diagnostic services and counseling delivered by licensed dietitians, nutritionists, or other qualified practitioners.13Cornell Law Institute. 130 CMR 405.475 – Medical Nutrition Therapy
If you’ve been using a GLP-1 through the state’s Vida Health program, that program will continue after July 1, 2026, with health coaching, nutrition support, and lifestyle guidance. Vida will no longer prescribe anti-obesity medications or schedule visits with its own clinicians for that purpose, so members who need ongoing medical management will be referred back to their regular prescriber or to the GIC network directory.5Mass.gov.
