Medi-Cal does cover a psychiatrist, and for most beneficiaries the visits cost nothing out of pocket. Coverage includes diagnostic evaluations, medication management, individual and group therapy, crisis intervention, and telehealth appointments. You do not need a referral from a primary care doctor to get your first assessment with a network provider, which is the single most common misunderstanding people have when they try to start care.
How to Actually Get an Appointment
Medi-Cal splits psychiatric care between two systems based on how severe your condition is. If your symptoms are mild to moderate and can be managed with outpatient therapy and medication, your Medi-Cal Managed Care Plan handles the service. If your condition is serious enough to significantly impair daily functioning, you move into “specialty mental health services” run by your County Mental Health Plan. The county and the managed care plan have a formal agreement that governs how people transition between the two systems in either direction.1DHCS – CA.gov. Medi-Cal Mental Health Services Referral Processes
For the managed care side, call the member services number on the back of your Medi-Cal card or use the plan’s online provider directory. Managed care plans cannot require a PCP referral or prior authorization before your first mental health assessment with a network provider. They are required to tell members this.2DHCS – CA.gov. APL 22-006 Medi-Cal Managed Care Health Plan Requirements for Mental Health Services If a representative tells you otherwise, they are wrong. Your PCP can still refer you if you prefer that route, but it is not required.
For specialty mental health services, every county operates a toll-free access line you can call directly to request an assessment. There is no single required referral path: you can self-refer, your managed care plan can refer you, or your PCP can start the process.1DHCS – CA.gov. When you call a psychiatrist’s office to schedule, confirm two things: that they currently participate in your specific Medi-Cal plan, and how long the wait is for new patient evaluations. High-demand specialists often have wait lists of several weeks. If the wait is unreasonable, call your plan and ask them to find a closer provider or authorize an out-of-network one. Plans have obligations to maintain adequate networks.
What a Psychiatrist Can Do Under Medi-Cal
Psychiatrists bill Medi-Cal for several distinct services:
- Diagnostic evaluations, where the psychiatrist reviews your mental status, history, and symptoms to reach a diagnosis.
- Medication management, which covers prescribing, adjusting, and monitoring psychiatric medications, tracking side effects, obtaining informed consent, and explaining risks and alternatives.3DHCS – CA.gov. Specialty Mental Health Services Medi-Cal Billing Manual Version 1.4
- Individual and group therapy aimed at reducing symptoms and restoring daily functioning.
- Crisis intervention for acute psychiatric emergencies, with the goal of stabilizing you and preventing hospitalization.
Telehealth is a valid option for all of this. You can see a psychiatrist by secure video or audio, and the provider must meet the same clinical and documentation standards as an in-person visit.3DHCS – CA.gov. Specialty Mental Health Services Medi-Cal Billing Manual Version 1.4 Telehealth slots usually open up sooner than in-person appointments with the same provider, so if your plan’s directory lets you filter by telehealth availability, use it.
One clinical boundary matters: coverage depends on medical necessity, meaning the psychiatrist must document a specific diagnosed condition and connect your symptoms to functional limitations in your daily life. A vague complaint of stress is not enough on its own. The diagnosis comes from the Diagnostic and Statistical Manual of Mental Disorders, the standard classification system mental health professionals use.
Psychiatric Medications
When a psychiatrist writes a prescription, Medi-Cal pays through the Medi-Cal Rx program. The Contract Drugs List functions as the formulary. Common antidepressants such as sertraline, fluoxetine, and escitalopram are covered, as are widely used antipsychotics including aripiprazole, quetiapine, and risperidone.4DHCS Medi-Cal Rx. Some drugs on the list carry restrictions. A medication with a quantity limit will be rejected at the pharmacy if the prescription exceeds the allowed amount unless your psychiatrist obtains prior authorization. Others have age limits or labeler restrictions. Drugs that are not on the list can still be covered, but the prescriber must get authorization from a Medi-Cal consultant, which usually involves showing that you tried and failed preferred alternatives first. Controlled substances, including certain anti-anxiety and sleep medications, face additional scrutiny. If a prescription is denied at the pharmacy, ask the psychiatrist’s office to submit a prior authorization before assuming the drug is not covered.
What It Costs
Most Medi-Cal beneficiaries pay nothing for psychiatric services. Mental health and substance use disorder services are specifically excluded from the copayment categories that apply to other Medi-Cal benefits. If a provider asks you for a copay at a psychiatric visit, verify with your managed care plan before paying.
If you also have Medicare, Medicare pays first and Medi-Cal picks up what Medicare does not, including premiums, deductibles, and coinsurance.5CMS.gov. Beneficiaries Dually Eligible for Medicare and Medicaid Providers who treat Qualified Medicare Beneficiaries must accept the combined payment as payment in full and cannot bill you for Medicare cost-sharing. One place Medi-Cal becomes especially important for dual-eligible beneficiaries: Medicare has a 190-day lifetime limit on inpatient psychiatric hospital stays, and Medi-Cal takes over as payer once that cap is reached.
What to Bring to Your First Visit
Bring your Medi-Cal Benefits Identification Card. Providers use it to verify your active coverage and bill the program; if you do not tell the office you have Medi-Cal, you could end up paying the bill yourself.6DHCS – CA.gov. Medi-Cal Help Center
Prepare a list of every medication you currently take, with dosages and prescribing doctors, so the psychiatrist can screen for interactions and see what you have already tried. Records from previous mental health treatment or hospitalizations are not required, but they help the psychiatrist avoid repeating medications that did not work.
Before the appointment, write down how your symptoms affect your daily life. Concrete examples carry more weight than general descriptions. “I have not been able to work for two months because I cannot concentrate” is more useful to the clinician than “I feel bad.” Clear documentation of functional impairment also strengthens the medical necessity case if your plan later reviews the authorization.
Getting to the Appointment
If you have no way to reach your psychiatric appointment, Medi-Cal covers transportation.7DHCS – CA.gov. Transportation Services There are two forms:
- Non-Medical Transportation, meaning rides by private or public vehicle for beneficiaries who simply have no other way to get there. Managed care members call the plan’s member services department; fee-for-service members contact DHCS directly.
- Non-Emergency Medical Transportation, meaning ambulance, wheelchair van, or litter van service for people who cannot physically use regular transportation. This requires a prescription from a licensed provider.
Both cover travel to mental health appointments. Request rides in advance so you do not miss the visit.
If Your Service Is Denied
If your managed care plan denies a psychiatric service, you will receive a Notice of Action explaining the decision. You have 90 days from the date the notice is mailed to request a State Fair Hearing.8DHCS – CA.gov. Medi-Cal Fair Hearing You can file by completing the form on the back of the notice and mailing it to your county welfare department, faxing it to the State Hearings Division at (833) 281-0905, filing online through the California Department of Social Services, or calling (800) 743-8525.
One timing detail matters. If you request the hearing before the effective date of the denial and within ten days of the notice, your benefits continue while the appeal is pending. Miss that window and your services may be cut off during the review, which is exactly the wrong time to lose access to psychiatric care. The state must issue a final decision within 90 days of receiving your request, or within seven working days if you qualify for an expedited hearing because delay could jeopardize your health.9eCFR. Crisis Help
Medi-Cal covers crisis intervention without prior authorization. Crisis services can be delivered in person, by phone, or through telehealth, and can happen anywhere, not only in a clinical setting.3DHCS – CA.gov.
