Yes, Medi-Cal does cover acupuncture, but only for one purpose: treating severe, persistent chronic pain caused by a recognized medical condition.1Medi-Cal Providers. Acupuncture Services The benefit is capped at two sessions per month under a shared outpatient limit, and under fee-for-service Medi-Cal you do not need a prescription or a referral from a doctor to use it.
What Conditions Qualify
The Medi-Cal provider manual limits covered acupuncture to procedures that prevent, modify, or relieve the perception of severe, persistent chronic pain from a generally recognized medical condition.1Medi-Cal Providers. Acupuncture Services Conditions like chronic back pain, arthritis-related pain, or fibromyalgia pain fall within that definition.
Both standard needle acupuncture and acupuncture with electrical stimulation of the needles are included.2Department of Health Care Services. Acupuncture Fact Sheet If your acupuncturist performs both during the same visit, that counts as a single service.3Legal Information Institute. California Code of Regulations Title 22 Section 51304 – Benefit Limitations
A good deal of acupuncture practice falls outside what Medi-Cal will pay for. The following are not covered:
- Nausea or vomiting, including chemotherapy-induced nausea, post-surgical nausea, or morning sickness.
- Mental health conditions such as anxiety, depression, PTSD, or insomnia.
- General wellness, stress management, sleep support, or preventive care.
- Fertility support or weight management.
- Experimental or investigational techniques not established as effective for chronic pain.
If a visit falls outside the covered benefit, you could be billed for the full cost. Tell the acupuncturist clearly that the treatment is being billed to Medi-Cal and that it is directed at your chronic pain condition.
The Two-Session Monthly Limit
Medi-Cal covers two acupuncture sessions per calendar month, but the cap is not acupuncture-specific. Under California’s Medi-Service reservation rule, you have a combined maximum of two services per month across six outpatient therapy types: acupuncture, audiology, chiropractic, occupational therapy, podiatry, and speech therapy.3Legal Information Institute. California Code of Regulations Title 22 Section 51304 – Benefit Limitations A chiropractic visit and an acupuncture visit in the same month exhausts your allowance for all six.
One “service” means all the care a single practitioner provides during a single visit.3Legal Information Institute. California Code of Regulations Title 22 Section 51304 – Benefit Limitations So a session that includes both needle and electrical stimulation acupuncture is still one service against the cap.
Getting More Than Two Sessions a Month
When two sessions are not enough to manage severe pain, your acupuncturist can file a Treatment Authorization Request (TAR) asking the state to approve additional visits. The TAR must include clinical documentation showing why more sessions are medically necessary.1Medi-Cal Providers. Acupuncture Services The provider files it, not the patient, so raise the issue with your acupuncturist if your pain is not controlled within the standard limit. Approval is not automatic; the state reviews the medical evidence and decides whether the request meets the threshold for medical necessity.
Do You Need a Referral?
Under fee-for-service Medi-Cal, no. The provider manual is explicit that no prescriptions are required and no prior authorization is needed for standard acupuncture services within the monthly limit.1Medi-Cal Providers. Acupuncture Services You can schedule directly with an enrolled acupuncturist.
Managed care works differently. If your Medi-Cal is delivered through a managed care health plan, the plan may require its own referral or authorization steps before you see an acupuncturist. These rules vary by plan. Check your plan’s member handbook or call the number on your Medi-Cal card to find out what applies to you. The underlying benefit and the two-session cap stay the same regardless.
Finding a Provider Who Accepts Medi-Cal
The acupuncturist must hold a valid California acupuncturist license from the California Acupuncture Board and must be actively enrolled as a Medi-Cal provider.4Department of Health Care Services. Acupuncturist Application Information A session with someone who is licensed but not enrolled will not be reimbursed, and that is one of the more common reasons claims get denied.
If you are in managed care, start with your plan’s provider directory or call the plan for a list of contracted acupuncturists. For fee-for-service, the Medi-Cal Health Care Options provider search can help you locate enrolled practitioners. Either way, call the office before booking to confirm they currently accept your specific Medi-Cal coverage.
What You Pay
Nothing, for a covered visit. Medi-Cal beneficiaries have no copayment for covered acupuncture services, and the office bills Medi-Cal or your managed care plan directly. Each claim must include the diagnosis of the condition causing your pain.1Medi-Cal Providers. Acupuncture Services
If an enrolled provider asks you to pay out of pocket for a session that should be covered, that is a warning sign. Providers who accept Medi-Cal must bill the program directly and take the state’s reimbursement as full payment. They cannot balance-bill you for the difference between their standard rate and what Medi-Cal pays.
If Coverage Is Denied
When Medi-Cal or a managed care plan denies acupuncture coverage or a TAR, you will get a written Notice of Action explaining the decision and your appeal rights. Managed care members typically appeal through the plan’s internal grievance process first. If the plan upholds the denial, or if you are in fee-for-service Medi-Cal, you can request a state fair hearing. The deadline is 90 days from the date you receive the Notice of Action.5Department of Health Care Services. Medi-Cal Fair Hearing
You can request a hearing by mail, by fax, online through the California Department of Social Services, or by calling the toll-free hearing line at (800) 743-8525.5Department of Health Care Services. Medi-Cal Fair Hearing The back of the Notice of Action has a hearing request form you can fill out and return. Note any need for language assistance on the form.
If you were already receiving acupuncture and your plan moves to reduce or stop coverage, you can keep getting treatment at the previously approved level while your appeal is pending. This is called aid paid pending. To qualify, request continuation of benefits by the effective date of the denial or within 10 days of the notice date, whichever is later.5Department of Health Care Services. Medi-Cal Fair Hearing One catch: if you lose the appeal, the state can ask you to repay the cost of services received during that period.
