Does Medi-Cal Cover GLP-1 in California: Drugs, Appeals, Options

Medi-Cal does cover GLP-1 medications, but only for specific medical conditions. As of January 1, 2026, Medi-Cal no longer pays for GLP-1 drugs prescribed for weight loss or weight-related conditions in adults 21 and older. Coverage continues for type 2 diabetes and, through prior authorization, for a short list of other FDA-approved uses. Members under 21 can still get GLP-1 drugs for weight loss through the federal EPSDT benefit.1Medi-Cal Rx (DHCS). Important Update: GLP-1s for Weight Loss Not a Covered Benefit

Which GLP-1 Drugs Medi-Cal Still Pays For

Seven GLP-1 medications remain on the Medi-Cal formulary for type 2 diabetes: Ozempic, Rybelsus, Mounjaro, Victoza, Byetta, Bydureon, and Trulicity. The pharmacy claim is denied if the diagnosis code on the prescription does not indicate type 2 diabetes.2Medi-Cal Rx (DHCS). Changes to Medi-Cal Rx Effective January 2026

If a provider believes one of these drugs is medically necessary for a condition other than diabetes or weight loss, they can submit a prior authorization request. Medi-Cal Rx reviews those on a case-by-case basis and continues to cover GLP-1 drugs for FDA-approved uses such as atherosclerotic cardiovascular disease and chronic kidney disease when medical necessity is documented.1Medi-Cal Rx (DHCS). Which GLP-1 Drugs Were Removed

Three drugs used primarily for chronic weight management are gone from the Medi-Cal Rx Contract Drugs List entirely: Wegovy (semaglutide), Zepbound (tirzepatide), and Saxenda (liraglutide). Any pharmacy claim for these three is auto-denied regardless of the diagnosis code. Every prior authorization that had been approved for weight-loss use expired on December 31, 2025.1Medi-Cal Rx (DHCS). Important Update: GLP-1s for Weight Loss Not a Covered Benefit

Two of them can still be approved for non-weight-loss conditions:

  • Wegovy for noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH, a form of fatty liver disease) or cardiovascular disease. As of April 2026, Wegovy for MASH no longer needs a prior authorization at all. The provider just includes the correct diagnosis code.3California Pharmacists Association. • Zepbound for obstructive sleep apnea, through prior authorization.4Medi-Cal Rx (DHCS). GLP-1 Coverage Considerations

The rule is uniform. No Medi-Cal managed care plan — not L.A. Care, Health Net, Kaiser, or any other — covers these drugs for weight loss as a separate or supplemental benefit.5California Medical Association. GLP-1 Medications for Weight Loss Will No Longer Be Covered by Medi-Cal

Members Under 21

The weight-loss exclusion does not apply to Medi-Cal members younger than 21. Federal law requires state Medicaid programs to provide comprehensive benefits to children and young adults through the Early and Periodic Screening, Diagnostic, and Treatment program, and California has confirmed that GLP-1 drugs for weight loss remain available in that age group with an approved prior authorization.6Medi-Cal Rx (DHCS). How To Appeal a Denial

If you receive a Notice of Action denying a GLP-1 prescription, you can request a State Hearing through the California Department of Social Services. The deadline is 90 days from the date on the notice. You can request a hearing by:

There is an extra protection for members who were already taking a GLP-1 drug before January 1, 2026. File the hearing request within 10 days of the denial notice and you can ask for “aid paid pending,” which keeps the medication coming while the appeal is decided. The continued coverage lasts until the hearing decision, the expiration of the existing prior authorization, or the appeal’s withdrawal, whichever happens first.7Medi-Cal Rx (DHCS). If You Lose Coverage, What Are the Options

The Department of Health Care Services has told providers to work with affected patients on individualized care plans. Those may include switching to a different treatment, gradually tapering the GLP-1 drug, or shifting to non-drug approaches like dietary changes, increased physical activity, and counseling.1Medi-Cal Rx (DHCS). Paying out of pocket is possible but expensive. The FDA approved an oral tablet form of Wegovy in December 2025, with cash prices starting around $149 per month for the lowest dose. Why Medi-Cal Dropped Weight-Loss Coverage

The change was a budget decision. Governor Gavin Newsom signed the 2025–26 Budget Act eliminating the coverage to help close a projected $12 billion deficit. Federal law does not require state Medicaid programs to cover weight-loss drugs; California had previously included them as an optional benefit.10CalMatters. Medi-Cal Coverage of Weight Loss Drugs

The numbers behind the decision are large. Medi-Cal spending on Ozempic and Wegovy alone reached about $733 million in 2023. Wegovy prescriptions through the program jumped from 15,000 in 2022 to 181,000 in 2023, and Ozempic prescriptions rose from 178,000 to 480,000 over the same period.10CalMatters. If you have a Medi-Cal denial in hand and think your prescription falls within one of the medically necessary categories still covered, the fastest move is to ask your prescriber to resubmit with the correct diagnosis and, if needed, a prior authorization request. If the denial stands, the State Hearing deadline is 90 days, and the 10-day window for aid paid pending matters if you were already on the drug.
