Medi-Cal does cover weight loss programs in California when a doctor documents that the treatment is medically necessary, but the list of what’s actually paid for narrowed in 2026. Behavioral counseling, the Diabetes Prevention Program, and bariatric surgery are still covered for adults who meet the clinical criteria. GLP-1 weight loss medications like Wegovy, Zepbound, and Saxenda are not, effective January 1, 2026. Beneficiaries under 21 have broader access under separate federal rules.
How Medi-Cal Decides a Weight Loss Service Is Covered
Every service runs through a medical necessity test. California law defines a service as medically necessary for adults 21 and older when it is reasonable and necessary to protect life, prevent significant illness or disability, or relieve severe pain.1California Legislative Information. California Welfare and Institutions Code 14059.5 State regulations require fully documented medical justification before any service is authorized.2Cornell Law School. California Code of Regulations Title 22, 51303 – General Provisions
For weight management, body mass index is the main gatekeeper. A BMI of 30 or higher opens the door to obesity-related services like intensive behavioral therapy. Bariatric surgery sits behind a higher bar: a BMI of 40 or above, or 35 or above when paired with a related condition such as diabetes or heart disease.3CA.gov. Surgery: Digestive System – Medi-Cal Provider Manual A BMI number alone rarely carries a prior authorization request. Your doctor needs to show how your weight contributes to or worsens other health problems.
Behavioral Counseling for Obesity
Medi-Cal covers face-to-face behavioral counseling for beneficiaries with a BMI of 30 or higher, following U.S. Preventive Services Task Force recommendations. Sessions focus on diet, exercise, and long-term lifestyle changes. A typical schedule runs weekly for the first month, every other week through month six, then monthly for the remainder of the year if you reach an early weight loss target. The benefit allows up to 22 sessions in a 12-month period, billed under procedure code G0447. More than that requires a Treatment Authorization Request.
The counseling has to happen in a primary care setting, delivered by a primary care doctor, nurse practitioner, or other qualified provider enrolled with Medi-Cal. A specialist referral by itself doesn’t satisfy the rule. For most beneficiaries, the obesity screening and counseling benefit is free.
The Diabetes Prevention Program
The Diabetes Prevention Program is a separate benefit for people diagnosed with prediabetes. It’s an evidence-based lifestyle change program designed to prevent or delay Type 2 diabetes through diet and exercise.4Department of Health Care Services. Diabetes Prevention Program The provider must hold CDC recognition through the National Diabetes Prevention Recognition Program.5CA.gov. Diabetes Prevention Program Not every Medi-Cal provider offers it, so finding a participating provider may take some searching. Consistent attendance matters, and missing sessions can affect continued eligibility.
What Changed for GLP-1 Weight Loss Drugs in 2026
This is the biggest shift. Effective January 1, 2026, Medi-Cal Rx stopped covering GLP-1 medications prescribed for weight loss. The three drugs removed from coverage for weight-loss indications are:
- Wegovy (semaglutide)
- Zepbound (tirzepatide)
- Saxenda (liraglutide)
Any prior authorizations previously approved for these drugs expired on December 31, 2025. Claims for weight-loss indications submitted on or after January 1, 2026 will be denied.6Department of Health Care Services. Changes to Medi-Cal Rx, Effective January 1, 2026
The same drugs can still be reviewed case by case for non-weight-loss conditions. Wegovy is reviewed for noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH) or cardiovascular disease. Zepbound is reviewed for obstructive sleep apnea.7Medi-Cal Rx. Changes to GLP-1 Drug Coverage – Effective January 1, 2026 Seven other GLP-1 drugs, including Ozempic, Mounjaro, and Trulicity, remain on the Contract Drug List but are restricted to a Type 2 diabetes diagnosis and cannot be prescribed for weight loss alone.6Department of Health Care Services. Changes to Medi-Cal Rx, Effective January 1, 2026
If you were taking one of the affected drugs for weight loss, talk to your prescriber about alternatives. Options may include older weight-management medications still on the Contract Drug List or non-drug approaches like behavioral therapy.
Bariatric Surgery
Medi-Cal covers surgical weight loss procedures like gastric bypass and sleeve gastrectomy, but the approval criteria are strict. You must meet all of the following:
- A BMI of 40 or higher, or a BMI of 35 or higher with at least one obesity-related condition such as Type 2 diabetes, hypertension, or sleep apnea3CA.gov. Surgery: Digestive System – Medi-Cal Provider Manual
- Documented failure of non-surgical weight loss through conservative treatment such as supervised diet and exercise programs
- A comprehensive treatment plan covering pre-operative preparation and post-operative care
- No medical or psychiatric conditions that would make surgery unsafe
The documented-failure requirement is where most people stumble. Reviewers want records of a sustained, supervised weight loss effort, usually involving regular check-ins tracking diet, exercise, and weight over several months. A single visit to a nutritionist followed by a surgery request months later won’t clear the bar. The paperwork has to show real engagement with non-surgical approaches that didn’t work.
After surgery, Medi-Cal expects ongoing follow-up care including nutritional counseling and monitoring. If a revision surgery is needed later, you’ll generally have to show that you complied with the post-operative nutrition and exercise plan before the second procedure will be considered.
Different Rules for Beneficiaries Under 21
Anyone under 21 has broader coverage. Under the federal Early and Periodic Screening, Diagnostic and Treatment benefit, Medi-Cal must cover all medically necessary services for children and young adults, including services to correct or improve conditions like obesity.8Department of Health Care Services. Medi-Cal Coverage for EPSDT Federal guidelines recommend referral to intensive behavioral interventions for children aged 6 and older with a BMI at or above the 95th percentile for their age and sex.9Medicaid.gov. Addressing Childhood Obesity
The 2026 GLP-1 cutoff works differently here, too. Medi-Cal Rx will still consider prior authorization requests for weight loss medications for members younger than 21, evaluated for medical necessity under the EPSDT standard.6Department of Health Care Services. Changes to Medi-Cal Rx, Effective January 1, 2026 All EPSDT services are provided at no cost.8Department of Health Care Services. Medi-Cal Coverage for EPSDT
Skin Removal and Reconstructive Procedures After Weight Loss
Significant weight loss sometimes leaves excess hanging skin that causes infections, mobility problems, or ulceration. Medi-Cal can cover procedures like panniculectomy, which removes the hanging abdominal skin fold, when they meet the medical necessity standard. The excess skin must cause a documented functional impairment, not just a cosmetic concern. Typical approval criteria include chronic skin infections that don’t respond to treatment, interference with mobility, or ulceration beneath the fold. A cosmetic abdominoplasty is not covered.
For people who had bariatric surgery, most coverage criteria require that your weight has been stable for several months and that you’re at least 18 months past the surgical date before reconstructive procedures will be considered. The surgeon submits photographs and medical records showing the functional problems the excess skin is causing.
Getting a Service Approved and What to Do If It’s Denied
Most weight loss services beyond basic behavioral counseling require prior authorization. Your provider submits the request, but you should know what goes in the package: current height, weight, and BMI; records of previous weight loss attempts and outcomes; documentation of obesity-related conditions; and the doctor’s written explanation of medical necessity. Bariatric surgery requests also include supervised non-surgical weight loss records and a complete pre- and post-operative plan.
For Medi-Cal managed care plans, federal rules require a decision on an expedited prior authorization within 72 hours when a delay could seriously harm your health.10eCFR. 42 CFR 438.210 – Coverage and Authorization of Services As of 2026, standard requests must be decided within 7 calendar days, down from the previous 14-day window. Either deadline can be extended by up to 14 additional days if you ask or if the plan can show the extension is in your interest.
A denial isn’t the end of the road, especially for bariatric surgery, where initial denials are common. If you’re in a Medi-Cal managed care plan, the first step is filing a grievance with the plan within 90 days of the Notice of Action. The plan has 30 calendar days to resolve a standard grievance, or 3 calendar days for an expedited one when your doctor says waiting could endanger your health.
If the plan upholds the denial and the decision rested on medical necessity, you can request an Independent Medical Review through the California Department of Managed Health Care. An outside review organization looks at your case fresh, considering your medical records, your doctor’s recommendation, and any additional information you submit within ten business days of the review starting.11eCFR. 45 CFR 147.136 – Internal Claims and Appeals and External Review Processes If the reviewer overturns the denial, the plan must provide the service.
You can also request a State Fair Hearing within 90 days of the denial notice, or up to 180 days with good cause.12Medi-Cal Rx. State Fair Hearing Request Form Request a hearing by calling the State Hearings Division at 1-800-743-8525, filing online at cdss.ca.gov, or mailing the form to the California Department of Social Services. You cannot pursue both an Independent Medical Review and a State Fair Hearing for the same denial at the same time.13eCFR. 42 CFR Part 431 Subpart E – Fair Hearings for Applicants and Beneficiaries
One timing detail is worth knowing. If you file a grievance or hearing request within 10 days of the action date on your denial notice, you can keep receiving the denied service on a temporary basis, called aid paid pending, while the appeal is processed. Miss that window and the service stops until the appeal is decided.
